    A 24-year-old woman was referred to our dermatology clinic for assessment of vulvar papules. They had been present for at least 2 years, during which time she had tried a course of imiquimod 5% cream, a wart treatment. The lesions, aside from causing significant embarrassment, had not changed in number or size and were asymptomatic. The patient had no known history of sexually transmitted infections. Examination revealed multiple tiny, filiform, rosy papules, symmetrically distributed on the inner sides of her labia minora (Figure 1). The papules were not spreading. The projections were easily separated from each other on examination. The affected area was not painful to touch.Lesions of vulvar intraepithelial neoplasia (Figure 2B) are clinically variable and nonspecific. They closely resemble non-neoplastic epithelial abnormalities or shallow verrucocities. They may be dull red and granular, or they may present with a variegated red and white pigmented appearance with interspersed warty areas. The diagnosis can be made only by biopsy. In our case, we decided against biopsy because the clinical presentation was typical for benign disease: all of the papules were separate and symmetrically distributed, whereas neoplasia often presents as a unique patch.

    The most specific findings on CT include air around the distal esophagus and aorta, pleural effusions, esophageal wall thickening and pulmonary infiltrates.4 A fluoroscopic esophagram performed with water soluble contrast is the diagnostic procedure of choice in patients with suspected perforation of the esophagus. Dilute barium can be used if the water soluble contrast is not diagnostic.5 Findings on esophagram that are indicative of perforation include direct visualization of the submucosal tear or extravasation of contrast. 4 While visualization of the fistula or the actual tear is a definitive finding, there has been a =10% false-negative rate reported with esophagography.6 Helical CT esophagography using oral solution composed of 10% IV iodinated contrast material, effervescent granules and water has shown promising results for demonstrating esophageal extravasation.7 The treatment of choice consists of emergent primary surgical repair of the esophageal tear and removal of debris from the mediastinum.

    Other malignant diseases Cavitary lesions in patients with lymphoma are infrequent, however, pulmonary lymphoma can cavitate (Figure 6).28 These cavities are usually multiple with thick walls and have an upper-lobe predominance.29, 30 However, in lymphoma patients with human immunodeficiency virus (HIV) infection, cavitation is not rare, and =25% of patients can have a cavitary lesion detected on CT images.31 Nonmalignant cavitary diseases Tuberculous, nontuberculous, bacterial and fungal infections can present with cavity formation and ascertaining the most likely diagnosis is vital in the management of the oncology patients, especially the post bone-marrow transplant population.\n In addition, it has been reported that the presence of a "halo sign," defined as a nodule surrounded by ground-glass attenuation, is reasonably sensitive and specific for invasive aspergillosis in high-risk patients.89 At pathologic examination, the nodules represent foci of infarction, and the halo of ground-glass attenuation results from alveolar hemorrhage.90 However, this CT appearance has been observed in patients with lung metastases from angiosarcoma, choriocarcinoma and osteosarcoma.91 Pneumocystis jiroveci pneumonia is a common opportunistic infection in HIV-infected patients but can also occur in patients with hematologic and solid malignancies.

    An analysis of Medicare data shows that by 1992, two-thirds of all radiologic imaging involved modalities that did not exist 20 years earlier or that existed only in embryonic form.4 As a result of this rapid technological development, several multidetector computed tomography (MDCT), positron emission tomography and computed tomography (PET-CT), and magnetic resonance imaging (MRI) techniques became available to address particular clinical questions.5-10 Part of the task of the radiologist is to be adept at selecting the study with the greatest likelihood of providing the correct diagnostic answer safely and at an acceptable cost. The most commonly used cross-sectional modality is CT and images can be acquired at the same time on combined PET-CT scanners.39 Integrated PET-CT adds important clinical information in comparison with PET alone, CT alone or separate comparison of PET and CT images that were acquired at different times-owing to better lesion identification and localization.\n Invasion of mediastinal vascular structures may also be assessed by a double inversion recovery MRI technique, because it nulls the adjacent blood pool signal.35-37 Biologic, metabolic and functional activity The use of PET-CT assesses the biologic, metabolic and functional activity of the tumors and allows diagnosis of lymph node and distant metastasis, increasing the sensitivity, specificity, and accuracy when compared with CT alone.74 But FDG-PET imaging has its own limitations since some benign or non-aggressive lesions may demonstrate FDG avidity and inflammatory lesions usually are intensely FDGavid.
