    From:William Glover

    Document Overview:
A limited liability company (LLC) is a separate legal entity that can conduct business just like a corporation with many of the advantages of a partnership. It is taxed as a partnership. Its owners are called members and receive income from the LLC just as a partner would. There is no tax on the LLC entity itself. The members are not personally liable for the debts and obligations of the entity like partners would be. Basically, an LLC combines the tax advantages of a partnership with the limited liability feature of a corporation. This LLC Membership Certificate shows the name of the member to whom the interest in the LLC is being transferred, the Seller or Transferor, and the amount of the interest.

    Amendment to Prior Affidavit

    $14.99

    $14.99

    From:William Glover

    Document Overview:
To amend means to correct, rectify and/or to remove the faults or errors in a document. An affidavit is statement of facts which is sworn to (or affirmed) before an officer who has authority to administer an oath (e.g. a notary public). The person making the signed statement (affiant) takes an oath that the contents are, to the best of their knowledge, true. It is also signed by a notary or some other judicial officer that can administer oaths, affirming that the person signing the affidavit was under oath when doing so. An affidavit generally consists of statements of fact regarding the issue at hand, with a section at the bottom for the affiant to swear to the truth of the statements made and affix his/her signature, which is then notarized in a jurat. A jurat is the bottom part of an affidavit where the officer certifies that the document was "sworn" before him.

    $19.99

    From:William Glover

    Document Overview:
A trust is a legal agreement in which a trustor transfers assets to a trustee. A joint trust is a living trust created by a married couple that designates both spouses as grantors of the trust. There are several advantages and disadvantages in regards to entering into a joint trust, so each couple will need to decide if this type of arrangement is suitable for their needs. A joint trust generally contains provisions that benefit both spouses while they are living as well as the surviving spouse in the incident of death. Provisions may also be included for descendants or beneficiaries that the couple requests to receive assets in the event of death. Joint trusts typically serve as the primary estate planning document and are used in the place of standard wills. The use of a joint revocable living trust as the primary estate planning instrument can be appropriate for certain married couples whose assets are uncomplicated and whose combined estates are not subject to the estate tax. A joint living trust can, however, result in significant gift and estate tax problems in certain estates for which tax planning is required.

Letter from Attorney to Trustee of Trust Informing Trustee of Duties and Liabilities

    $19.99

    $19.99

    From:William Glover

    Document Overview:
A trust involves any arrangement by which legal title to property is transferred from one person to be administered by a trustee for another person’s benefit. The person who manages a trust has a legal duty to manage the trust's assets in the best interests of the beneficiary or beneficiaries. Typical trustee duties include managing rental properties, investing funds or paying income to the beneficiary. Generally, trustees are paid for their services because of the amount of work involved in managing a trust and the threat of potential liability if assets are mismanaged. It is advisable to agree in advance upon how much a trustee is to be paid.

  • Plaintiff Medical Damages / Data Sheet
Plaintiff Medical Damages / Data Sheet

    $19.99

    $19.99

    From:Rick Barton

    Document Overview:
Plaintiff Medical Log / Data Sheet in spreadsheet format that arranges medical expenses for a claimant in a manner that can be scaled by provider and/or date of service. The Data Sheet is an evolving tool that can be increased in size to accommodate for several individual dates of service and can be used by a paralegal and/or legal practitioner to keep track of medical records/billings as a personal injury case is being developed. The Data Sheet calculates per-treatment charges horizontally with sub-totals (by provider) and grand total charges vertically. This enables the user to capture insurance deductions for settlement negotiations with insurance providers and insurance adjusters/defense counsel. Finally, the Data Sheet is arranged so that only Dates of Service/Charges/Grand Total can be selected and printed for demand correspondence and/or submission to courts without including insurance deductions. Or, the entire document can be printed for hard-copy inclusion into the file for reference/preservation of evidence. This data arrangement has been tested in several personal injury matters and has been very successful in detailing claimant damages, the driving metric in any personal injury matter, to both insurance adjusters and courts of law. Technical support is available at any time by calling (203) 885-5377.
