					            FAMILY MATH TRAINING REGISTRATION FORM
At Rutgers University, New Brunswick from 8:30 a.m. to 3:00 p.m. on:

•    October 16,17, 20, 2008
•    January 13, 14, 16. 2009
•    April 20, 21, 23, 2009


FAMILY MATH training prepares educators to establish and conduct FAMILY MATH classes at their schools.
These classes include materials and activities for parents to use in helping their children with mathematics;
these classes may be conducted in English or Spanish.

FAMILY MATH training provides 18 hours of professional development. The Center for Family Involvement in
Schools (NJDOE Provider #1660) and the Rutgers Center for Mathematics, Science, and Computer Education
(NJDOE Provider #2), which sponsor the training workshops, are registered as professional development
providers with the New Jersey Department of Education.

FAMILY MATH training will include:
     Math activities for use with parents and children from all elementary math skill and concept areas.
     Career activities that demonstrate the importance of mathematics in children’s futures.
     Information on how to set up FAMILY MATH classes.
     One copy of the FAMILY MATH **book and other materials necessary to conduct FAMILY MATH classes. (The
     books and materials are provided in English or Spanish. Please indicate your preference below, and add $25.00 to
     the fee per additional book you wish to order.)

FEE: $1,235 per team of two teachers.

The Rutgers FAMILY MATH program meets equity and/or parental involvement requirements for Title I, Title II, Title VII, and
Eisenhower funding.

FAMILY MATH is a professional development activity endorsed by both the NJEA Professional Development Institute and the New
Jersey Statewide Systemic Initiative and reinforces both the National as well as the NJ State Core Curriculum Content Standards in
mathematics.

*We strongly recommend that Family Math Programs be administered by teams of two. Single registrations will be accepted on the
basis of replacing team members in existing programs or completing teams when two cannot attend the same session.
