					                          MAYFIELD HIGH SCHOOL JUNIOR/SENIOR PROM

On Saturday, May 14, 2011, the Junior/Senior Prom will be held at Signature of Solon. In order to inform
parents of the activities and guidelines pertaining to this special event and to help promote a safe and enjoyable
evening, we are providing you with the following information:

       WHEN:           Saturday, May 14, 2011, 6:00 – 11:00 p.m. THEME: “Out Of This World”
       WHERE:          Signature of Solon
                       39000 Signature Drive
                       Solon, Ohio 44139
       COST:           $100 per couple ticket or $50 for a single ticket.
                       Tickets will be sold during lunch periods Wednesday April 27th through Friday, May 6th.
                       Checks should be made payable to “Mayfield High School.”

ADDITIONAL COSTS:              Photographs – Picture packages start at $22 per couple (payable that evening).
Pictures will be taken all evening starting at 6:00 p.m. There is no need to stand in line.
       a.      Doors will open at 6:00 p.m. – no earlier.
       b.      You must sign up for a dinner table when you purchase your prom ticket. Tables are set for 10 guests.
               To assure sitting with your group, buy your tickets early and at the same time. Seats may not be held or
               saved. Your ticket must be purchased in order to put your name at a table.
       c.      Dinner will be served at 7:00 p.m.
       d.      Crowning of the prom queen and king will follow dinner.
       e.      Non-Mayfield dates must complete the “Guest Permission Slip” PRIOR to the purchase of tickets.
               Guests must follow all Mayfield rules, regardless of their ages.
       f.      Students entering the prom will have their names checked for accountability purposes and will not be
               permitted to leave and return. Once the student leaves the prom, it is assumed that a parent will know of
               his or her child’s whereabouts. The school chaperones cannot be responsible for students after they leave
               the prom.
       g.      Parents are encouraged to discuss safety and proper behavior for this occasion with their son or daughter.
       h.      Students under the influence of alcohol or any other chemical substance will not be permitted to enter the
               prom and will be suspended from school and may forfeit the privilege of attending commencement
       i.      Parents are expected to not serve alcoholic beverages of any kind prior to or after the prom. Student
               safety while driving is of the utmost importance. Remember, the legal drinking age in Ohio is 21 for any
               alcoholic beverages, and the state law specifies that any driver under 21 years of age who tests as low as
               .01 on a breathalyzer will be charged with a DUI.
       j.      Students are expected to dress in proper attire throughout the entire prom. Good taste is the order of the
AFTER-PROM ’10 – “Game On!”
WHEN:       12:00 A.M. – 4:00 A.M. ON Sunday, May 16, 2010
WHERE:      Mayfield Middle School, 1123 S.O.M. Center road, Mayfield Heights
COST:       $10 per person – Tickets must be purchased in advance.
            Tickets will be on sale during lunch periods May 10th through May 14th

NO TICKETS WILL BE SOLD AT THE DOOR. See the flier posted for additional information.
Remember, you don’t have to attend prom to go to after-prom. Our previous after-prom parties have all been
HUGE SUCCESSES, and we expect this year’s to be even better!!

ATTENTION SENIOR SEARCH STUDENTS: It is your responsibility to purchase advance tickets.
Suggestion: Have a friend or parent purchase your ticket during lunch periods.
