					                      Professional Soccer Training Program in Springfield MO

Dear Coach, Parent(S) and Players,

With the increasing demands for professional training over the last few years by so many Teams, I have decided to
expand the soccer academy. I will be focusing my time on a new service dedicated to a professional coaching program
that may be of interest to you and/or your son/daughter to be part of USTA Soccer Academy or USTA Soccer Select
Team.

This Professional Coaching Program is new and just making its way to the USTA soccer scene. It will be available for
two different time periods.

Season Requirements
The full program is based on a ten-month cycle, 5 months - November to March- Indoor Season, 5 months - April to
October (not including June, July Soccer Camps optional)

Program Levels: There are two levels of professional coaching program available.
Level 1 -1 session per week (Recreational)                 Level 2 -1 session per week (Competitive)

Agreement: Each player’s parent will be asked to sign an agreement that will contract them to the level of service.

Level 1 Program (1 session per week non- Competitive soccer experience) will focus on fundamentals of soccer like
passing, trapping, dribbling, and shooting all basic of the soccer game. Practice Time will be 1:15 minutes per session.
For all soccer players with no competitive or travelling soccer experience, each player will be evaluated by a
Professional Trainer, this may change player’s age or group depends on the USTA Trainer decision. Some groups may
be mixed between boys & girls depend on the number of player’s and ability.

Level 2 Program (1 session per week only for Competitive Player’s) will focus on the entire game more than basics of
the game; we will train player’s positions, including how to play at an organized as a team. For Soccer players with
traveling or competitive experience, a USTA professional soccer trainer will evaluate all the players to make the
appropriate placement level decision.

Academy Date’s and Time:

Monday: 5:30pm to 6:45pm U-9, 10 boys (Level 2) 6:45pm to 8:00pm U-13, 14 girls (Level 2) 8:00pm to 9:15pm U-13, 14 girls and
boys (Level 1).

Tuesday: 5:30pm to 6:45pm U-11, 12 girls (Level 2) 6:45pm to 8:00pm U-11, 12 boys (Level 2) 8:00pm to 9:15pm U-11, 12 girls
and boys (Level 1).

Wednesday: 5:15pm to 6:30pm U-7, 8 girls and boys (Level 2) 6:30pm to 7:45pm U-9, 10 girls (Level 2) 7:45pm to 9:00pm U-13,
14 boys (Level 2) 9:00pm to 10:15pm high (school girls Level 2).

Thursday: 4:45pm to 6:00pm U-5, 6 (boys & girls Level 1 & 2) 6:00pm to 7:45pm U-7, 8 girls and boys (Level 1) 7:45pm to 9:00pm
U-9, 10 girls and boys (Level 1) 9:00pm to 10:15 (high school boys Level 2).

                                                 Who run this Soccer Academy?
                                                 Julio Reyes: Technical Director: impressive playing career spanned 14 years
                                                 1988-2003. He participated in three CONCACAF Champions clubs. In
                                                 December 2007, Julio proudly received certification of Completion of the
                                                 world-renowned Real Madrid International training course, sanctioned by
                                                 the Spain Soccer Federation. Julio was in high demand as he trained many
                                                 top clubs’ teams in New Jersey. Julio served as the assistant Coach for the
                                                 NJ Stallion Professional Soccer Team for a span of two years. In June of
                                                 2009, Julio Reyes completed the “B” national license course and currently
                                                 USSF Soccer license. In 2010 Julio work with Pulaski United Soccer Club us a
                                                 technical director, later in 2010 he move to Springfield to work with Total
                                                 Demize us the Youth Academy Director, also he was the Trainer and coach
                                                 for the U-16 High School Boys. In January 2011 Julio Reyes was name us the
                                                 Springfield Demize Head Coach. The Demize is one of Team’s that
                                                 participated in the United States Soccer Development League.

Coach Reyes Staff will be only people with Professional back ground, like Maximo Reyes Asistant Technical Director.
Maximo Reyes 16 Year’s Professional Soccer Player Career playing in different countries participated in Libertadores
Cup in four occasions. Also Coach Maximo was a former Peruvian National Team Player. Maximo is a certificated USSF
license.

Alfredo Quesada: “Trainer” Former Peruvian International Professional Player. Alfredo has played the “A” League
Professional in USA; actually he is one of the Springfield Demize Top Players. Alfredo is a certificated USSF license.

USTA requires two month payment up front, make check payable to USTA. By signing this contract you are
committing to 5 months of training with the obligation to pay for the entire 5 months. For more information call

Kearny all the way to Grand make right on Grand drive about two block’s to you see W Talmage St. the school gym
is behind the houses.

Cost $80 per player monthly (Please make check payable: to United Soccer Training Academy)

Indoor Training starts November 1, 2011 ends March 22, 2012 outdoors practice starts April 3, 2012 ends October
25, 2012 (During the Months of Jun and July I will be not practice, only soccer camps.) Training will be on Monday,
Tuesday, Wednesday and Thursday USTA will decide which day your son or daughter will practice.
As the parent/guardian of the above name child, I hereby give my/our permission for his/her participation in USTA Winter Soccer
training. I know of no illness or physical impairment that will prevent him/her from participating. I assume all risks and hazard
incidents to such participation, including transportation to and from all soccer related activities. I agree to waive all claims resulting
from or in connection with United Soccer Training.
