									            Bid Package 4A – Masonry & Stone
The successful Subcontractor acknowledges and agrees to the requirements set forth by
this bid package and its descriptions and in conjunction with all other Construction
Documents, as required to provide, furnish, install, and/or maintain all items necessary
for a complete scope of work as listed below. The scope of work shall be inclusive of,
but not limited to the following specifications, information, and requirements:

I)       Specifications
      1) See specification bid package responsibility list.
      2) The successful Subcontractor is responsible for all specification Divisions and
         Sections included within the Construction Documents as required to provide a
         complete scope of work for this project. The individual Divisions and Sections
         listed above are provided as a courtesy only.
      3) Subcontractor herein acknowledges that he/she has examined the project site
         and is fully informed as to the nature of the work and conditions relating to its
         completion. Any special precautions required in order not to disturb existing
         items to remain are included.


II)      Scope Specific Items:

      1) Include Davis-Bacon wage scale rate.

      2) Provide all labor and material to perform all operations necessary for the
         complete execution of masonry and stone scope as indicated in the Contract
         Documents and as further defined in this scope of Work.

      3) SKANSKA is fully engaged with providing an Injury Free Environment for the
         workers on the jobsite as described in the Safety and Health Management
         Program. Included with this philosophy is the SKANSKA Stretch and Flex
         program beginning each morning of work at 7:00 am. Subcontractor agrees to
         this philosophy and shall endeavor to meet requirements as set for by SKANSKA
         Management team. If Subcontractor, at anytime, is against this environment,
         SKANSKA may elect to terminate this agreement.

             a) Subcontractor is to provide a site-specific safety plan as detailed in
                 Skanska’s Contract Exhibit. Any regulation and/or safety procedure
                 omitted from the Safety Plan shall not relieve the Subcontractor from the
                 responsibility of providing the necessary safety requirements as required
                 to perform and complete this scope of work.

             b) Each Subcontractor shall maintain first-aid provisions on-site for use by
                 their employees in case of minor injures.

          c) Subcontractor’s Safety Manual and Material Safety Data Sheets (MSDS)
              required for the work and materials furnished under this Subcontract are
              to be on the jobsite prior to the delivery of the materials.

   4) Concrete masonry units (CMU).

   5) Mock up panels/wall as indicted on the contract documents.

   6) Furnish and install wall cavity insulation as shown on the contract documents.

   7) Furnish and install all interior stone work as shown on the contract documents.

   8) Furnish and install all masonry anchors and reinforcement required for this
       scope of work.

   9) Reinforcing steel for masonry work will be installed by this bid package.
       (furnished by others)

   10) Precast/cast-stone lintels, sills, copings, caps, accent bands, bases and other
       special shapes.

   11) Site-related masonry, inclusive of unit pavers and bedding materials (see Civil
       drawings).

   12) Install all metal lintels and angles for masonry work, including but not limited to
       loose lintels, anchored lintels, shelf angles, lintels anchored into masonry,
       concrete, etc.

   13) Installation of items to be set in masonry whether furnished by this
       Subcontractor or others (i.e. reglets, anchor bolts, sleeves, boxes, etc.).

   14) Ensure exterior block and brick walls are properly reinforced and prepared for
       window/louver/door frame attachment per wind-loading and governing code
       requirements.

   15) Provide cut-outs, block-outs, and openings as required for doors, ductwork, fire
       extinguisher cabinets, toilet accessories, etc…

   16) Expansion joints, covers, and control joints within this work.

   17) Concrete, mortar and/or sand fill in masonry.

   18) Installation and grouting of hollow metal frames in masonry.

   19) Through-wall flashing, special flashing at window and door rough openings
       weeps, flashing around any other rough openings, mortar nets, cavity vents,
       etc…

   20) Provide hoisting equipment and operator as required completing this scope of
       work. Lifting and hoisting shall be performed in strict accordance with all
       regulatory guidelines. Subcontractor shall provide equipment and operator
       certifications and documentations as required.

   21) Pointing and cleaning of all masonry.

   22) Layout and engineering as required.

   23) Scaffolding and material or man lifts as required for the installation of this work.

   24) Weather protection as required.

   25) Provisions for cold weather work as required including hot water, temporary
       heat, enclosures, blankets, etc, to ensure normal flow of work in cold weather
       conditions.

   26) Caulking, sealants, compressible fire rated filler, firestopping, and steel bracing
       angles required for this scope of work will be provided by others.

   27) Subcontractor to coordinate masonry installation with other exterior trades to
       ensure proper installation.

   28) Subcontractor is required to provide all requirements for Quality Assurance/
       Quality Control.

   29) Clean up shall be performed continuously to keep work areas clean and free of
       debris. Subcontractor to include cleaning of all work installed as a part of this
       Subcontract. Subcontractor shall perform all final cleaning prior to turn over to
       Owner.

   30) Each subcontractor agrees to furnish all associated rigging, hoisting, unloading,
       handling, unpacking, and services necessary for erection and delivery into the
       premises of all necessary material, equipment and apparatus. Subcontractor
       agrees to remove same from premise when no longer required for the
       performance of this work.

   31) Each subcontractor is responsible for including all temporary measures,
       including traffic control, cleaning of trucks prior to leaving site, flagging
       personnel, barricades, bracing, shoring, re-shoring, underpinning, and
       protection as required for the performance of their scope of work.

   32) A hard copy of a three week Look-Ahead schedule is to be submitted to the
       Skanska superintendent every Monday by 2:00 PM. Schedule is to be separated
       by area, manpower and major deliveries.

   33) All Subcontractors are required to submit a written punch list to the Skanska
       superintendent after each phase of completed work.

   34) Subcontractor shall be responsible for all permits, fees, and licensing associated
       with the performance of this work, with the exception of the General Building
       Permit.

   35) Clinical Services Building (CSB)

       a) Include $5,000 for overtime/schedule enhancement. This is to be used at
            the sole discretion of Skanska.

   36) Medical Office Building (MOB)

       a) Include $5,000 for overtime/schedule enhancement. This is to be used at
            the sole discretion of Skanska.

       b)

   37) Central Energy Plant (CEP)

       a) All door frames are to be set by this bid package.

       b) Include $5,000 for overtime/schedule enhancement. This is to be used at
            the sole discretion of Skanska.

   38) Medical Intensive Care Unit (MICU)

       a) Include $2,500 for overtime/schedule enhancement. This is to be used at
            the sole discretion of Skanska.

   39) Cardiovascular Intensive Care Unit (CVICU)

       a) Include $2,5000 for overtime/schedule enhancement. This is to be used at
            the sole discretion of Skanska.

       b) Provide matching coping as detailed.

       c) Repair exterior brick as required by new work. See example on sheet A-3.0

       d) Provide & install brick vent as shown on 1/A-4.4

III)   Labor Units:
       Contractor/Buyer may elect to have Subcontractor/Seller provide labor (or additional labor) in
       connection with its Scope. All additional labor shall be billed at the applicable rate as required by
       law. Each specified rate is inclusive of all of Subcontractor’s/Seller’s costs and expenses (labor
       burden, taxes, overhead and profit, etc.) to furnish the additional labor in accordance with all
       requirements of the Subcontract/Purchase Order.

V)        Notes of Exclusion:
