Event and Causal Factor Chart - Enter the sequence of events before, during, and after the accident. - Enter any conditions that affected the events. - As they become known, enter any causes for each event. EVENT Fixed ladder installed on building in 1977. Employee climbed ladder. Employee fell from ladder. CONDITIONS Ladder was not compliant with OSHA requirements. Rungs were wet. CAUSAL FACTORS (to be entered later) Barrier Analysis Barriers generally fall in the following categories: equipment, design, administrative (procedures and work processes), supervisory/management, warning devices, knowledge and skills, and physical. What were the barriers (to prevent the accident)? B1: Slip-resistant rungs. B2: Proper climbing technique. How did the barrier perform? The rungs were smooth. Not used. Why did the barrier fail? Did not exist. Employee was carrying tools. Evaluation of Effect The rungs did not provide traction. Second hand was not available to stop fall. Change Analysis Identify and examine all changes and determine the significance or impact of the changes. Accident Situation Prior, Ideal, or Accident-free Situation Worker started day the same time as coworkers. Employee met with supervisor to discuss the day’s work activities. Difference Evaluation of Effect C1: Employee came to work early, so was working alone, carrying tools. C2: Because employee came to work early, job hazards were not discussed. Worker came in early to avoid the heat. Employee did not meet with supervisor the morning of the accident. No co-workers were available to help with the job. Work activities were not discussed. 1 Event and Causal Factor Chart - after completion of Barrier Analysis and Change Analysis Causal Factor: An event or condition in the accident sequence necessary and sufficient to produce or contribute to the unwanted result.
