                      Guidelines for Players Injury
:
<The GAA Player Injury Scheme has been developed to help with the financial burden
some injuries sustained through playing may incur. It is not an insurance scheme but one
where funds are paid into every year and it is from this fund you get your payment.
Please see the following guidelines:

       If you have been injured in a training session and require further treatment e.g.
        physio, scans you must inform your team mentors and the club
        secretary/insurance officer. In this way a letter can be produced by your club to
        state that you were injured during an official training session. This must be done
        with 48 hours of the injury happening.

       If you have been injured during a match you must ensure that the injury is notified
        to the referee either during the match or shortly afterwards. The referees report is
        used by the injury scheme to ensure the claim is legitimate and the report forms
        part of the claim.

       Download the Player Injury Form from www.gaa.ie. The first two pages of this
        must be with the club secretary/Insurance Officer within 14 days of injury.

       If you need to attend the physio you must also inform Mattie Lennon - the clubs
        coaching officer - who will note this.

       If you have attended physio and you have been advised to get a scan you MUST
        take the injury scheme form to the doctor to get it filled in saying that you have
        been referred for a scan. You will probably need a note from the physio stating
        this. If you do not fill in the injury scheme form you will not be reimbursed.

       If your injury means that you will be off work for more than one week you will
        have to provide wage slips or other forms to claim monies back from the scheme.
        You should note that the monies from the scheme can take up to 3 months before
        it is fully processed.

       All information on the forms must be filled in correctly.

       We also recommend that every player takes out their own personal insurance as
        the GAA player injury scheme is not an insurance and will not cover all of your
        wages.

    IT IS EVERY PLAYERS RESPONSIBILTY TO FILL
       IN THE FORMS AS IT IS A PLAYER INJURY
    SCHEME - THE CLUB WILL HELP YOU TO FILL
                     THEM IN
