           TECHNICAL MEETING REQUEST FORM (TMRF) FOR COOPERATION
                             Request for Computer Society Cooperation with a
                                        Conference or Symposium

1. ABOUT THIS FORM
This form is to be used to request that the Computer Society (CS) "Cooperate in" a Technical
Meeting. A TMRF should be submitted 9 to 12 months before the proposed conference. Allow
4 to 6 weeks for the review process. If all information is not completed on the TMRF or attached
as requested, the review process may be longer.

For meetings for which cooperation is being sought, additional information should be attached as follows:
1) Sponsors: If there is reason to believe that the Computer Society might not know a
sponsoring or cooperating entity, include a brief description of the sponsor, and indicate if it
is a for-profit organization.
2) Computer Society Member Involvement: Please indicate if members of the Steering Committee
or Technical Committee are members of either IEEE or the IEEE CS.
3) Call for Papers: draft or final version and any other relevant information about the conference or
the committee.
4) Statement of Benefit: The Vice President for Conferences & Tutorials, who has final decision
on cooperation status, requests a written "Statement of Benefit" to the Society for granting
cooperation with the conference. Please complete this new section in the TMRF. It must be
completed before the TMRF can be reviewed.

Please follow the guidelines in the Computer Society Technical Meeting Handbook. They are
keyed to this form. The handbook is available on our home page, http://www.computer.org, under
"Conference Organizer Tools" in the "Conference" section.

Since a number of copies will be made of this completed form, please use a typewriter or felt pen
to make the entries. This form is available on 3 1/2" or 5 1/4" diskettes using an Excel or Lotus
spreadsheet. Please contact the Volunteer Services Department for a diskette. Or you may request
that the spreadsheet be sent to you via email. Both the Excel and Lotus spreadsheets are available
on the home page under "Conference Organizer Tools" in the "Conference" section.

Please make registration income entries in U.S. dollars. For meetings held outside the U.S.,
indicate here the local currency (e.g., Swiss Francs) and the conversion rate used to compute the
U.S. dollar entries.

2. COOPERATION STATUS
The Computer Society prefers to participate in meetings as sponsors or cosponsors to provide

the best benefit for society members. The Computer Society will cooperate in a meeting where the
society participates but accepts no financial or legal liability for the meeting. For cooperation, there
must be member interest and participation, technical quality, and clear advantages for the
Computer Society and its members.

If your conference previously was granted cooperation and received an approval letter stating that
 "cooperation has been extended for one additional year" please complete a TMRF for
cosponsorhsip or sponsorship. Only in rare circumstances will the society grant cooperation
for more than one year. Cosponsorship from the society can be as little as 5% or as much as 50%
depending on the other sponsors. For more information on cosponsorship or sponsorship,
please refer to the Technical Meeting Handbook, which can be requested via the Volunteer Services
Department at (202)371-1013, or contact the Conference Finance Coordinator in the Volunteer
Services Department.
