The State of Ohio and OHVA both have minimum requirements as to the number of school days (and hours) and testing requirements. Your child’s education is your responsibility. You Must Help with: Attendance, Homework, Makeup work

Hours of Operation:
Normal “Core” hours of operation are 9:00 am to 3:00 pm Monday thru Friday. We are occasionally available for tutoring and/or makeup work. Any tutoring and/or makeup work will be by special arrangement only. Please Call us first. Cost will be determined based upon the arrangements made.
Basic Rates and Payment Policies:
$700 per month - for 9 months ~ or ~ $175 per week - for 36 weeks.
* Save $300.00 if you pay the Full Year's School Fee by August 8, 2009 * Save $150.00 if you pay 1/2 of the School Fee by August 8, 2009 , and the other 1/2 by December 31, 2009. If you are late, or any other defect, prices revert to the “Weekly Rate” except that payment will be due Before The Week Begins.
Payment Schedule:
Special Rates are due by the dates listed above. Monthly Rates are due at the beginning of the month before school begins. Weekly Rates are due on the Monday of the following week, except under defect, where payment is required on the Monday for the current week before the week begins. We accept cash, check, or money order for payment.
Returned Checks:
In the event of a "bounced check", you will be charged the actual bank fee, and this is deemed a a defect.
Closures - Holidays / Inclement weather:
We will be closed on; Thanksgiving Day, Christmas Day, and New Year's Day. School Holidays: Labor Day, Veteran’s Day, Thanksgiving Break (3 days), Christmas Break (2 weeks), Martin Luther King Day, Presidents’ Day, Spring break (a schedule will be given each year with the exact dates – see the calendar on-line.) We will be available during inclement weather, if you can get here.
Illness:
Basically, we do not want to spread communicable illnesses amongst our children. If your child has a fever or other signs of illness, please call and talk with us before you bring them here (or you may be asked to take them back home.) This policy is determined individually, based on the circumstances, so Please Call Us First.

Medical Emergencies:
Minor bumps and scratches are inevitable, but we do make every effort to keep the children safe through supervision and childproofing. Minor injuries receive appropriate first aid, and if a severe injury or illness occurs, you will be contacted as soon as possible. If necessary, your child will be taken to the nearest hospital and you will be asked to meet us there. Parents are responsible for all costs involved in and associated with any emergency medical treatment, including emergency transportation if required. Sheryl Green, Arlene Green, and/or their families will not be held liable for any sickness or injury of either the parent, guardian, or of the child while on these premises, or while the child is in the company of the provider during field trips, outings, or gym class.
Provider Emergencies:
We are prepared with emergency providers in cases of unplanned absences of short duration caused by unanticipated circumstances such as illness, accident, or other family crisis. We have three emergency providers that are good with children and are trusted. If an emergency provider is not available during this time, parents may be requested to pick up their children.
Medication:
Both nonprescription and prescription medications, ointments, and creams can be given to your child if needed. Please supply all medications in their original containers. These must be labeled with the child's name.
Transportation:
Occasionally we need to take children to school activities that they are involved in, or we may take a Field Trip excursion, or the Gym. All children will be placed in safety approved car seats as required by Ohio State Law. Parents may be requested to provide a car seat for the day. We carry copies of the emergency forms with a picture of each child attached. In the event of an emergency away from the provider home, your child will be cared for and you will be notified. You are asked for your permission to allow your child to travel with us in one of our cars. gives his/her permission for (child / children) to travel in the daycare providers' vehicles. • • • • • • • • ∗ ∗ parents are required to notify us by 8 a.m. if their child will not be coming for the day. Parents are required to keep us informed of any change in addresses, telephone numbers, and other pertinent information. Parents are required to inform us if they are at any other location than what is listed on their Enrollment Record and to provide a telephone number for that place. No Smoking is allowed on premises Parent / Provider conferences are held twice a year, in January and in July. Other conferences may be requested by either party. Provider maintains an open-door policy for parents during daycare hours. Parents are encouraged to call us at any time during the day or evening. There will be times when we can not answer the telephone because we are quite busy with the children, so please leave a message and we will call you back as soon as possible. This contract is only 1 of 3 forms that We Are Required To Have to provide for your child. You Are Required to fill out the “Emergency Contact Form” and the “Imunizations Form” for EACH CHILD. A small picture of each child needs to be attached to each “Emergency Contact Form”, for each child, so We Will Take a Picture. We would also like to have the appropriate “Social Form”, to help us better serve you and your child. You can copy these forms from the web site and print them, or ask us for a copy to fill out.
I (we) have read and fully understand this contract. I (we) understand our obligations and limitations as set forth herein and agree to abide by and comply with all terms. I (we) agree that any dispute or legal cause arising out of or from our association with you and/or your service to us has nothing to do with your families, heirs or estates, and that such dispute or cause will be settled by binding arbitration. All signators agree that if any provision of this contract is not enforceable, the remaining provisions will survive and will be enforced.
