BOOKING AGREEMENT
It is understood that (“Client”) has agreed to retain the services of Mark Douglass, Mark A. Douglass, LLC (collectively “Company”) for , (“Rental Date”).

2. Payment and Terms. Client agrees to pay Company $ Per hour Flat Rate An initial nonrefundable deposit of $ Is due upon signing this agreement. Final payment is due in full, two (2) weeks prior to the Rental Date. In the event of a postponement or cancellation of the agreement, there will be absolutely no refunds on the deposit paid. Payments may be made via cash, check, or credit cards. All credit card payments will be processed through PayPal and subject to a 5% service charge. 3. Conditions. This Agreement will not take effect, and Studio will have no obligation to provide any services, until Client returns a signed copy of this Contract and pays the deposit specified in section 2. 4. Confirmation of Rates and Booking. Please sign and return this contract to Company, along with the deposit specified in section 2. Rental Date cannot be guaranteed until the signed contract and deposit is received by Company. Please make checks payable to: Mark A. Douglass. Contract and deposit should be mailed to: Mark A. Douglass, LLC, 1888 Kalakaua Avenue, #C307, Honolulu, Hawaii, 96815. If any changes must be made to this contract, please notify Company immediately.
