Print Name of Organization ___________________________________________________________________ Student organizations are an essential part of the University of Florida community and play a vital role in providing important learning opportunities and a rich and diverse campus life experience for UF students. All student organizations are subject to the rules of the University of Florida including the Student Conduct Code and Academic Honesty Guidelines. Registered student organizations have access to designated University resources (including priority use of University facilities and posting on the website of the J. Wayne Reitz Union/Center for Student Activities and Involvement) pursuant to applicable University rules, regulations, and guidelines, including the Student Organization Handbook. Social sororities and fraternities shall exist at the University of Florida as registered student organizations and shall be classified as social sororities and fraternities, governed by policies established by the Division of Student Affairs and the Center for Student Activities and Involvement/Sorority and Fraternity Affairs (SFA). The governance structure established by the University of Florida includes student input and participation. Student input and participation is accomplished through authority delegated by SFA to designated student governing councils. Social sororities and fraternities must abide by applicable university policies, regulations, and procedures governing student organizations, as well as policies, guidelines, and procedures established by SFA and the designated governing councils. The social sorority and fraternity registration process is administered by SFA. The Registration period is from April 15 through September 15, and must be renewed each year. Social Sororities and Fraternities are not eligible to apply for Student Government Funding. ______ (President’s Initials) This organization acknowledges and agrees to uphold all applicable University of Florida Regulations and policies including but not limited to policies or regulations regarding Hazing (6C1-4.0161), Student Leader Eligibility (6C1-4.003), and Commercial Activity (6C1-4.006). Further, this organization acknowledges and agrees to uphold all applicable policies and guidelines promulgated through Sorority and Fraternity Affairs and the designated governing councils including but not limited to Florida Greek Standards, House Director Policy, Policy on Expansion, Chapter Facility Policy, and the Event Guidelines Policy. ______ (President’s Initials) A registered student organization shall be in full compliance with all federal and state nondiscrimination and equal opportunity laws, orders, and regulations. A registered student organization may not discriminate against a member or prospective member on the basis of race, color, religion, national origin, ancestry, age, marital status, disability, unfavorable discharge from the military, or status as a disabled veteran or veteran of the Vietnam era, except as specifically exempted by law. Likewise, among the individual discrimination prohibited by the University policy, but not by law, is sexual orientation. A student organization whose primary purpose is religious will not be denied registration as a Registered Student Organization on the ground that it limits membership or leadership positions to students who share the religious beliefs of the organization. The University has determined that this accommodation of religious belief does not violate its nondiscrimination policy. Certificate of Insurance Sorority and Fraternity Affairs requires each social sorority and fraternity to have a copy of the certificate of insurance on file. Accordingly, here are the directions on how to obtain this document: 1. Check with the Center for Student Activities and Involvement or Sorority and Fraternity Affairs to see if you currently have a valid insurance certificate on file. If so, you will not need to provide a copy as part of your registration. If not, you will need to follow the instructions below to obtain a copy of your insurance certificate. 2. Call the (Inter)National Office for your organization and discuss this requirement with them. Some offices may then direct you to contact the insurance carrier directly to request for the certificate, while other offices may make the request for you. 3. Some insurance carriers will wish to send a copy directly to the SFA office. If this is the case, please indicate on the registration form that the chapter has requested a copy and one will be sent to the address below. Your registration will be complete once the insurance certificate is received. 4. The Certificate must document that all chapters without a chapter house have a minimum of $1 million in liability insurance. Any chapter with a chapter house must have a minimum of $2 million in liability insurance. 5. It is recommended that you list University entities as additionally insured on the policy for the year. Doing so will insure events that need insurance coverage before a permit can be issued have appropriate documentation now versus trying to obtain insurance documentation closer to the time of the event.
