Keys to Creating an Effective
Portfolio
        “Education is that which liberates.” Ghandi.


T    his portfolio document should be completed with reference to the UBC School of
     Nursing PLAR student guide, which outlines the process of PLAR application and its
     underlying philosophy.

There are a number of benefits to creating a portfolio including, providing you the
opportunity to demonstrate your achievements; enhancing your ability to assess your own
achievement of educational outcomes; and allowing you to develop your skills in keeping a
portfolio (also a professional mechanism used by nurses to demonstrate their continuing
competence in practice). Finally your portfolio will give Faculty the opportunity to access
your prior learning in terms of its application to a specific program of study at the School of
Nursing. You may consider it as an amalgamation of your educational activities and a diary
or personal journal. It’s a collection of your work that tells the story of your efforts,
progress or achievement in a specific area.

In order to be most effective your portfolio should contain the following elements:

   Section 1: Application Details

    The UBC School of Nursing PLAR Application Forms should be completed for:
             applications for credit made on the basis of nursing courses previously
              undertaken at Canadian schools of nursing
             applications for credit made on the basis of previously accredited Canadian
              professional or continuing education courses
             applications for credit made on the basis of nursing courses taken outside of
              Canada
             applications for credit made on the basis of experiential learning

    These forms contain pages to identify the learning outcomes and the relevance of the
    learning that you are claiming credit for to the specific program or course you are
    applying for credit against. For those courses or learning experiences that you have
    identified in this section, provide a brief summary of how the outcomes from this prior
    learning relate to your chosen program of study in the relevant section (University Transfer,
    Certified Continuing Education or Experiential learning). You may cross-reference this to the
    evidence presented in section 3 of the portfolio.

   Section 2: Biographical Details

    Fill in your biographical details following the UBC School of Nursing PLAR application
    forms in this section. You should include an extended Curriculum Vitae with details of
    your academic and professional education and training which should outline:

                 details of your secondary and higher education to date (most recent
                  first)
                 academic, and professional/vocational qualifications achieved
                  (including where and when achieved)
                 employment history (with addresses and dates – most recent first)
                 details of any community/public service
                 any awards or publications
                 The names and contact details of two references (ideally one academic and one
                  professional referee)

       Section 3: Evidence

        This section contains the evidence to support your PLAR claim. It should be clearly
        laid out and cross-referenced by its page number to the items identified in section 1.
        This section may include evidence of transfer credit, certified learning or experiential
        learning if a claim is made in any or all of these categories. Evidence may include:

                  actual certificates (original certificates will be returned once verified)
                  official transcripts from previous courses you have undertaken (if official
                   transcripts are to be included in the PLAR Application Portfolio rather than
                   being sent directly from the institution do not open the original envelope.
                   The transcript must be clearly issued and sent directly from the institution at
                   which the course/program was taken.)
                  clearly identified learning outcomes and evidence of their achievement (for
                   experiential learning)
                  artifacts (such as published articles or project reports or professional policies
                   you may have developed)
                  testimonies from professional colleagues supporting a particular claim

 School of Nursing Prior Learning Assessment & Recognition Application



 These forms must be completed as a part of your portfolio submission for Prior Learning
 & Assessment Recognition (PLAR) accreditation. Please ensure that you have consulted

 1) University Transfer Credit

 Please note if you wish to apply for transfer credit for other university non-Nursing
 courses, this is undertaken by the UBC Registrar’s Office when the formal admission
 application is made. Therefore an application through the School of Nursing PLAR
 process is unnecessary.

 You should contact Diana Kendall by e-mail to diana@nursing.ubc.ca to request a list of
 transfer credits granted on your admission. These transfer credits should be listed as part
 of your PLAR Portfolio on page 10, University Transfer Credit for Non-Nursing courses.

 To request credit for Nursing courses from other post secondary institutions, the PLAR
 application does need to be completed and include the following material:

         an official transcript showing completion of the course(s)
         a course syllabus/outline showing a summary of content, theory hours and, where
          applicable, clincal/laboratory hours of instruction, and textbook used
         the specific learning outcomes achieved


 2) Certified Learning

 If you are applying for credit for continuing education courses/programs undertaken
 elsewhere you must give the details of each course/program you are requesting credit for
 on the following pages, and cross-reference this to the evidence which must be
 provided in English or French in section 3 of the portfolio, including the following
 material :

         proof of successful completion of the course/program (an original certificate,
          official letter of completion) or
         an official transcript of the course/program
         course/program description and learning objectives
         a summary of the taught hours for the course giving details of class time,
          laboratory time and clinical time
         a description of the assignments undertaken (course assessment/texbook used)

 3) Experiential learning

 If you are applying for credit for experiential learning you must briefly summarize the
 individual learning experiences that you are requesting credit for, their level in academic
 terms and the amount of credit sought for them on the appropriate pages beginning at
 page 15. This must be cross-referenced to the evidence which must be provided in
 English or French in section 3 of the portfolio, including the following material :

         the specific learning outcomes achieved for each identified experience
         proof of completion of the experience and meeting the learning outcomes
          identified previously (testimonies, logs, artifacts etc.)
         a brief description of the experience for which credit is sought
         an identification of the time spent undertaking this experience

 accreditation will not be reviewed.

I   n this section you need to provide evidence that you have met the learning outcomes that you
    have outlined previously. This needs to be clearly laid out with each item of evidence on an
individual page. You may use plastic sleeve inserts for artifacts you do not wish to damage, and
simply label them with a page number for reference. You should provide an index page (or pages)
at the front of the section, and use the page numbers at the bottom of each page of evidence for
referencing these items in the previous sections.
