					                                 VISA INFORMATION FOR CHINA
A visa is an advanced approval to enter a specific country for a specific purpose. A visa is required to visit the People’s
Republic of China. Two adjacent blank pages must be available in your passport in order to issue a Chinese visa.
Amendment pages in the back of your passport are not valid for visa issuance. The Global Young Leaders Conference’s
visa service provider, Passport Visa Express.com, coordinates the application process for your visa. The following
information provides instructions for assembling and submitting your Visa Application Packet for processing.


I. APPLYING FOR A CHINESE VISA

Please review the enclosed visa application instructions carefully. It is important that you forward all necessary
documents to Passport Visa Express.com as soon as possible and no later than April 1, 2009. Receipt of documents after
this date may delay processing and incur additional fees.

Visa Application Packet
To apply for a visa, you need the following items:
    • Original passport (please make sure that it is signed and that it is valid for six months from application date)
    • One passport-quality photo (do not affix this photo to your application forms)
    • Completed Visa Application Form
    • Completed China—Visa Service Form

If any of these required items is missing, incomplete or completed incorrectly, this will delay the processing of your visa.


II. COMPLETING YOUR CHINESE VISA APPLICATION PACKET

Please carefully follow the checklist and guidelines below when obtaining your photograph, completing the enclosed Visa
Application Form and China—Visa Service Form, and assembling your documents for processing.

Passport
You must include your passport in your Visa Application Packet. To ensure the safety and security of your passport, we
highly recommend that you enclose your passport and application documents in a sturdy padded envelope and send them
via overnight priority mail using a traceable service. Once your visa is processed and placed in your passport, it will be
returned to you via FedEx thirty (30) days prior to your program departure date, if submitted on time and with no errors.
A signature will be required for delivery.

It is the sole responsibility of each scholar to obtain a passport. If you do not have a valid passport, please refer to the
aforementioned instructions in your acceptance materials to assist you in renewing or applying for a new passport. The
Global Young Leaders Conference is not responsible for lost time or additional costs incurred by an individual due to
improper travel documents.

Passport-Quality Photograph
You must submit one passport-quality photograph for your visa. The photograph must meet the following guidelines:
   • 2x2 inches in size
   • Taken within the past six months, showing current appearance
   • Color or black and white
   • Full face, front view with a plain white or off-white background
   • Dimensions should be between 1 and 1 3/8 inches from the bottom of the chin to the top of the head
   • Taken in normal street attire
           a) Uniforms should not be worn in photographs except religious attire if it is worn daily.
           b) Do not wear a hat or headgear that obscures the hair or hairline.

            c) If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn
               for your picture.
            d) Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for
               medical reasons. A medical certificate may be required.

            Important: Do not affix the photograph to any of the forms in your Visa Application Packet.

Visa Application Form
The Visa Application Form must be neatly printed in black ink or typed in BLOCK letters. Applications completed in
pencil are not acceptable. The application must have an original signature. Please read the instructions below for specific
guidelines on how to complete each section of the Visa Application Form:

    1.1 Full Name: Fill in your Surname (Last name), Middle Name and Given Name (First name).
          *Please note that you must write your name exactly as it appears on your passport.
    1.2 Sex: Check the appropriate box.
    1.3 Chinese Name: Fill in your Chinese name in Chinese characters if applicable. This applies to those born in
          China PRC, Taiwan, Macau or Hong Kong.
    1.4 Current Nationality: Complete appropriately.
    1.5 Other or Former Name: If you have a former name, such as a maiden name, fill in this information here.
    1.6 Former Nationality: Complete appropriately if applicable.
    1.7 Date of Birth: Complete with year-month-day.
    1.8 Place of Birth: Complete with state and country where you were born.
    1.9 Passport Type: Do not add or change the information that has been provided for you.
    1.10 Passport Number: Complete appropriately.
    1.11 Date of Issue: Complete with year-month-day.
    1.12 Place of Issue: Refer to your passport’s information page for ‘Issuing Authority’ and complete appropriately.
          If no city name is listed as the issuing authority, write National Passport Center.
    1.13 Expiration Date: Complete with year-month-day.
    1.14 Current Occupation: Do not add or change the information that has been provided for you.
    2.1 Purpose of Your Visit: Do not add or change the information that has been provided for you.
    2.2 Number of Entries: Do not add or change the information that has been provided for you.
    2.3 Date of First Entry into China: Do not add or change the information that has been provided for you.
    2.4 Intended Stay: Do not add or change the information that has been provided for you.
    2.5 Cities to visit: Do not add or change the information that has been provided for you.
    2.6 Express or rush service: Do not check either box; express or rush service is not necessary.
    3.1 - 3.6 Your Health Condition and Previous Overseas Tour: Complete this section appropriately.
    4.1 Name of Your Employer or School: Complete appropriately.
    4.2 Daytime Phone Number: Complete appropriately.
    4.3 Address of Your Employer or School: Complete appropriately.
    4.4 Nighttime Phone Number: Complete appropriately.
    4.5 Home Address: Complete appropriately.
    4.6 E-mail: Complete appropriately.
    4.7 Name of Inviter in China: Do not add or change the information that has been provided for you.
    4.8 Phone Number of Your Contact: Do not add or change the information that has been provided for you.
    4.9 Address of Inviter in China: Do not add or change the information that has been provided for you.
    4.10 Email of Your Contact: Do not add or change the information that has been provided for you.
    5     Other Declaration: Complete appropriately if applicable.
    6.1 Name of Person Completing Form: Complete appropriately if applicable.
    6.2 Relationship to the Applicant: Complete appropriately if applicable.
    6.3 Address and Phone Number of that Person: Complete appropriately if applicable.
    6.4 Signature of that Person: Complete appropriately if applicable.
    7     Applicant’s Signature and Date of Signature: You must return the application with your original signature
          and be sure to include the date.

China—Visa Service Form
In order for Passport Visa Express.com to process your visa and return your passport to you prior to your departure, you
must fully and accurately complete the enclosed Visa Service Form. Please pay particular attention to the “Address to
Return Passport” section. When completing this section, keep in mind both your school and travel calendars and then
select the appropriate address at which you would like to receive your returned travel documents. Your documents will be
shipped to you via FedEx thirty (30) days prior to your GYLC departure. An individual must sign for this package when
it is delivered, so we cannot accept P.O. Box addresses.


III. SUBMITTING YOUR VISA APPLICATION

Once you have assembled all of the items required for your Visa Application Packet, enclose them in a sturdy, padded
envelope and send the packet via traceable overnight delivery service to arrive at Passport Visa Express.com no later than
April 1, 2009.
