      Dear Mr. Fox:

      The Stipulated Settlement relative to a fourteen (14) day SUSPENSION of Kwanj it Ketchampa's
      Massage Therapist/Off-Premises police permits at 1020 South Ramona Street, San Gabriel, CA 91776,
      which was agreed upon and signed by Ms. Ketchampa prior to the scheduled hearing will be considered
      by the Police Permit Review Panel on:

                                              100 W. 1 st Street, Los Angeles, CA 90012

      You may, but need not, be represented by counsel for the Panel to render a decision; your presence is not
      required. However, it is highly recommended that you appear. A request for continuance must be submitted
      in writing to the Panel Secretary at least six (6) business days prior to the hearing date. A request for
      continuance must include proof of hardship and rarely will be honored on the day of the hearing due to
      scheduling conflicts. Once a continuance request has been granted, no subsequent requests will be accepted.

      When the Panel considers the permit at its public meeting, it may at its discretion, entertain discussion, oral
      argument, or taking of evidence.

SUBJECT: REQUEST TO ACCEPT STIPULATION FOR KWANJIT KETCHAMPA,
          FOR MASSAGE THERAPIST/OFF-PREMISES, PC NO. 135889, LOCATED AT
          1020 SOUTH RAMONA STREET, SAN GABRIEL, CA LOS ANGELES, CA 90013


Permittee, Kwanjit Ketchampa applied for renewal of her Massage Therapist/Off-Premises police permit.
On her renewal form, she failed to list her arrest in the city of Burbank, CA for prostitution, and in her
ensuing court case, her subsequent plea to a lesser charge of operating without a permit. Permittee
Ketchampa chose to enter into a stipulated agreement in lieu of a formal hearing. In this stipulated
agreement, she would surrender her police permit for a period of 14 days. The stipulation which was
signed by permittee Ketchampa is attached with this document.

The Department requests that you adopt the stipulation, and order the surrender of the permittees
Massage Therapist/Off-Premises police permit for the period stated on the stipulation.

Pursuant to Section 102.19 of the Los Angeles Municipal Code (LAMC), I, Kwanjit Ketchampa agree
as follows:

1.     To waive any and all rights to a hearing that I may now. have, or in the future accrue,
       under the Constitution, laws or ordinances of the City of Los Angeles, the State of
       California or the United States with regard to the matter set forth in the Accusation for
       Revocation Action, dated March 2, 2010, to the factual accuracy of the counts listed
       below.

A computer inquiry through the City of Los Angeles, Office of Finance, LA TAX Database,
revealed that Ms. Kwanjit Ketchampa possesses a Massage Therapist/Off-Premises permit. The
permit is active as of January 2, 2008.

                                                       ACCUSATION
Complainant makes and files this Accusation in his official capacity as Commanding Officer,
Commission Investigation Division, and not otherwise.

Lieutenant Chris Waters, for causes of discipline against Respondent, alleges:


                                                         COUNT 1

 On or about November 19, 2008, permittee Kwanjit Ketchampa filed a renewal form with the Los
 Angeles Police Department, Commission Investigation Division, Permit Section, attempting to
 renew her Massage Therapy permit. Section 6 of the renewal form is a question which requires a
 yes or no answer. It states, "Have you within the five years immediately preceding the date of the
filing of this application been convicted of a charge of violating any lesser included or lesser
 related offense, including California Penal Code Section 415, in satisfaction of, or as a substitute
for an original charge of any offense listed above." Permittee Kwanjit Ketchampa failed to list her
 March 17, 2008 arrest, where after being arrested and charged of a violation for PC 647(b), she pled
 to a-       charge of operating without a permit, Burbank Municipal Code 8-2204(A). This
 violation is defined in Section 103.35(a) of the Los Angeles Municipal Code.


2.      That I will accept a 14-day suspension of my Massage Therapist/Off-Premises Police
        Permit.

3.      That no appeal will be made by me regarding the specifics of this stipulation.
