This is to certify that I, as the parent/guardian of:

licensed physician, hospital, or medical clinic, for the player named herein at such time as either parent or
legal guardian cannot be contacted in person or by telephone. The authorization shall include all league
activities, including the period required to travel to and from those activities; and we do hereby waive,
release, absolve, indemnify, and agree to hold harmless Hawaii Kai Youth Baseball, Inc, its Board of
Directors, managers, coaches, supervisors, umpires, organizers, participants, and persons transporting the
player to and from those activities, for any and all claims arising out of an injury to the player.

                       HKYB 2011 Mustang/Bronco Goodwill Tournament
                                  RULES & INFORMATION
General Information:

      Teams may enter the tournament by printing out the necessary forms at www.hkyb.org

      Teams must have all paperwork and fees received prior to the first game. The fee is $275 per team.
       Each team must complete a Team Application Form and each player must complete the Medical Release
       Form.

      Tournament Dates: July 2 – 4, 2010 (Saturday – Monday)

      Time: 8:00 – 6:00 p.m.

      All teams will play a minimum of three games.

      There will be NO pre-tournament meeting. All pertinent information will be sent to you via e-mail prior
       to the tournament.

General Rules:

      Mustang players must be 10 years of age and under. Bronco players must be 12 years of age and under.

      There will be no pre-game infield. Teams can practice one hour prior to scheduled game on the Pony
       Field (field with lights). Soft-toss batting only. Please share the field.

      Mustang base distance is 60 feet, pitching distance is 44 feet. Bronco base distance is 70 feet, pitching
       distance is 48 feet.

      All teams must have a minimum of 10 players and a maximum of 15 players.

      Head Coaches shall flip a coin prior to the start of the game to determine the home team.

      The home team is the official score keeper.

      Players/Coaches ejected will miss the duration of the present game.

      The tournament director has the final say in all tournament matters.

Pitching Rules:

      Pitching limit is 4 innings for Mustang. No limit for Bronco.

      24 hours rest for Bronco pitchers after 7 innings and Mustang pitchers after 4 innings.
Base Running:

      Base runners will make every attempt to avoid a collision with the fielders. Players must slide or avoid
       when the defensive player has the ball and is ready to make a play or it will result in an automatic out or
       possible ejection from the game.

      NO sliding head first into home plate.

Length of Game:

      There is a two-hour “drop dead” time limit. At the two-hour limit, all play will stop, and the game will
       revert back to the last completed inning. If the last completed inning was a tie, the game will revert back
       to the last completed inning with no tie.

      Bronco 10 run rule in effect after the 5th inning. Mustang 10 run rule in effect after the 4th inning.

Player Equipment:

      All teams must be properly uniformed and prepared to play.

      Metal spikes are allowed in the Bronco Division but are not allowed in the Mustang Division.

Player Participation:

      The batting line-up can be the entire line-up or 9 players. If 9 players then teams must substitute
       following PONY baseball rules. If the entire line-up is used then free substitution.

      Coaches must remove a pitcher upon his/her second trip to the mound in the same inning.

      A courtesy runner is allowed for the pitcher or catcher with to outs.

Parking and Concession:

      Park only in designated areas. Illegally parked cars (including those parked on the grass) will be subject
       to ticketing and possible towing.

      A concession stand will be located near the entrance of the baseball complex. Please encourage your
       players and families to patronize the concession.

      Tents can be set up in the potluck area. If you choose to leave your tents up overnight you will be doing
       so at your own risk. There is no overnight security at the park/tournament.


As a reminder, this is a goodwill tournament and not a championship tournament. Games are played for fun in
a competitive atmosphere. Managers are responsible for the team and parent’s conduct. We ask that everyone
please show good sportsmanship. Mahalo!
