                               Debutante Cotillion Application
                                               2010-2011


The Cincinnati Alumnae Chapter of Delta Sigma Theta Sorority, Inc is seeking young ladies in their
junior or senior year of high school to participate in our 2010-2011 Debutante Cotillion program.
Eligible candidates who have attained a minimum 2.5 G.P.A. and have demonstrated the potential to
succeed are encouraged to apply. In addition, these young ladies must also demonstrate an active
interest and involvement in public service and community related activities.

Historically, a cotillion was characterized as an elaborate, formal dance. In recent years, it is most
often recognized as a formal presentation of young ladies referred to as debutantes. These young
ladies are presented to society after they have developed social etiquette skills and acquired a sense of
responsibility and achievement that prepares them to master life’s challenges.


For Delta Sigma Theta Sorority, Inc. this concept is embraced as an opportunity to introduce young
African American females to society through an African cultural experience. The program is designed
to develop and enhance their personal, educational, cultural, spiritual, and social lives as African
American young ladies.

In order to participate in the 2010-2011 Cotillion you must complete the application package which
consists of the following components:

   1. Completed Cotillion Application (application attached)
   2. Official High School Transcript (transcripts must come directly from your school and should
      be in a sealed envelope)
   3. Signed Cotillion Statement of Criteria (criteria attached)
   4. Personal Statement expressing your career goals, describing how your involvement as a
      debutante will enhance your personal character, and identify the one person that has influenced
      you the most and why?
   5. Letter of Recommendation from a teacher, school administrator, clergy, community leader, or
      volunteer organization leader

Submit the completed Cotillion Application Package and Official Transcript to:

                                           Statement of Qualification

   I                         confirm that I meet all of the Cotillion participation criteria
   listed below and understand that I must maintain these standards throughout the 2010-
   2011 Cotillion season.

       1. Junior or Senior between the ages of 16 and 18 years of age.
       2. Cumulative grade point average of 2.5 or better (verified by official transcript).
          Transcripts will need to come directly from your school and should be in a sealed
          envelope with your schools seal and/or appropriate signature.

       3. Demonstrated interest and involvement in public service and community related
          activities.
       4. Demonstrated goals, aspirations, and plans to pursue an education beyond high
          school.
       5. Evidence of good moral character, which includes but not limited to:
             A. No record of poor discipline at school or in the community
             B. No criminal, misdemeanor, or court record of any kind
             C. No indication or public hint of pregnancy
             D. Must not be a parent

       6. Must not have previous or current employment in places of
          entertainment including bars, clubs, etc…

       7. Must be able to commit to attending necessary rehearsals and activities that occur
          throughout the Cotillion season.
