 This survey must be completed by a relevant and competent person within your organisation. This person should have detailed knowledge of the activities carried out on-site in
2009.
 Please report waste data for 2009 only. All quantities must be reported in TONNES.
 Enter information in the white boxes only. Do not type in the shaded or coloured boxes,
 Start by filling in the "Company Information" sheet. These questions will determine which of the sheets A to F you will need to complete.
COMPANY INFO
Please fill in all sections of this sheet. Provide the name and contact details of the relevant person who will be able to assist with any queries on the data submitted.
Mass balance - this section fills in automatically, based on the data you enter into the relevant sheets. It calculates the difference between waste entering and leaving your site in
2009.
If the difference in the mass balance is large (greater or less than 5%) please provide any possible explanation for the difference between incoming and outgoing tonnages.
SHEET A. COLLECTION
Pleae complete this sheet only if your company collected municipal household waste at kerbside in 2009.
For 2009, the EPA is asking for information on kerbside household waste collections directly from waste collectors.
The reason is that collection runs may cross county boundaries, and waste collecters will find it easier to report a national versus a regional tonnage.
SHEET B. INCOMING
The EPA is obligated to report on waste generation and waste treatment by SOURCE; for example municipal vs. construction & demolition (C&D). For this reason, the list of incoming
wastes is grouped according to source (EWC Chapter codes) on the Incoming sheet.
Please include details of all waste accepted onto your premises in 2009.
For "Method of Quantity Assessment", please state how you obtained the tonnage figure reported for each waste stream.
SHEET C. OUTGOING
Include all waste sent off-site to third-party facilities for recovery or disposal.
If a single waste stream (e.g. mixed municipal waste, EWC 20 03 01) went to more than one destination, please provide the tonnage sent to each destination in a separate row.
For wood, plastic, metal, glass and paper/cardboard, please provide a packaging/non-packaging split. This breakdown is vital for determining Ireland's progress towards National and
EU packaging recycling targets.
SHEET D. STORAGE
Please enter the tonnages of material in stock at the start and end of 2009. The figure for "net storage" will calculate automatically.
SHEET E. BROKERED
The term "brokered waste" here refers to waste your company handled during 2009 but which did not enter any of your company's sites.
This includes waste sent (i) directly to landfill, (ii) directly to a third party Irish recovery facility, or (iii) directly abroad.
As this waste did not enter your company's site(s), it is not normally included in weighbridge reports but is recorded separately.

SHEET F. ONSITE RECOVERY
Please report here only waste that is actually recovered or recycled on your premises. Do not report in this section waste that only undergoes pre-treatment (e.g. sorting,
segregation, trommelling, repackaging) at your site.
Waste that is recovered stops being a waste - it is either used on-site (e.g. as a fuel) or is sent off-site as a product (e.g. compost) or raw material. As this material is not sent off-
site as a waste, it should not be included in SHEET C. OUTGOING.

In order to select the R code which fits the recovery operation carried out on your premises, please refer to the document "Explanation of Recovery and Disposal Codes " which is
available on our website at www.wastesurvey.ie. This document gives useful examples of the types of operations that fall under each of the R and D codes.
