  INFORMATION & INSTRUCTIONS FOR THE APPLICANTS


                            Important Instructions
                                                for
                                  APPLICANTS
  Applications are required to go through the document carefully and note the procedure for
submission of the documents. The applications should be filled up completely and carefully and
sign the documents wherever it is prescribed.

  The entire physical and financial turnover must be supported by the clients certificates not
below the rank of Executive Engineer duly attested should be submitted. The details without
client’s certificate will not be considered and the applicant would be disqualified for which he
himself would be responsible.

              Information & Instructions for Applicants
   1.0     General :
   1.1     Forms for Pre- qualification and letter of transmittal are attached herewith.

   1.2     All information required for, in the enclosed forms should be furnished against the
           respective columns in the forms, if information is furnished in separate document;
           reference to the same should be given against respective columns. If information is
           ‘NIL’ it should also be mentioned as ‘NIL’ or ‘No such case’. If any particular
           query is not applicable in case of the applicant, it should be stated as not
           applicable. However, the applicants are cautioned that not giving complete
           information called for in the application, in the form required or not giving it in
           clear terms of making any change in the prescribed forms may result in the
           applicant being summarily disqualified. Applications made by telegram or telex
           and those received late will not be considered.

   1.3     The application must be type-written and the applicant’s name must appear on
           each page of the application.

   1.3.1   All financial / monetary figures to be furnished in any of the pages/forms in these
           pre-qualification documents shall be in’ Rupees‘

   1.4     Over-writing should be avoided. Mistake should be scored through and corrections
           incorporated and attested with the applicants initials written in ink. All pages of
           the Pre-qualification document shall be numbered and submitted as a package with
           the signed letter of transmittal.

   1.5     All the information must be submitted in English.

   1.6     Reference, information and certificate form the respective clients certifying the
           suitability, technical knowledge and capacity of the applicant should be signed by
           the client.

   1.7     The applicant is advised to attach any additional information which he thinks
           necessary in regard to his capabilities. No further information will be entertained
           after submission of pre-qualification documents unless it is called for by the Water
           Resources Department.

   1.8     The applications are invited for the work as detailed in the brief note. The Pre-
           qualification documents in prescribed form duly completed and signed shall be
           submitted.

   1.9     The copies of the documents must be submitted in a sealed cover. The sealed
           cover must be superscribed:-

“Pre-Qualification documents for CONSTRUCTION OF R.B.C. & L.B.C. MAIN CANAL,
MINOR & CANAL STRUCTURES, BARREL CUM TROUGH ESCAPE, DRB, AQUADUCT,
HEAD CUM CROSS REGULATOR, V.R.B., C.D. WORK AND HEAD REGULATOR OF
BHADAR DIVERSION SCHEME, BLOCK-MANPUR

   And shall be submitted to the Engineer-in-Chief, W.R.D. Bhopal or Chief-Engineer,
   GANGA BASIN REWA (M.P.) or Superintending Engineer , WATER RESOURCES
   CIRCLE, SHAHDOL (M.P.).
          Documents submitted in connection with the pre-qualification will be treated as
         confidential and shall not be returned.

1.10.    The cost incurred by the Applicant in preparing this application, in providing
         clarification or attending discussion, conferences in connection with document
         shall not be reimbursement under any circumstances.

1.11     An affidavit in the prescribed format as per Annexure-E should be furnished by
         the Applicant ( Tenderer ) regarding correctness of the information furnished at
         Annexure F-2 and Annexure – A & B

2.0      Method of Applying.

2.1      If the application is made by an individual, it shall be signed by the individual
         above his full name and the full name of his firm with its current address.

2.2      If the application is made by a firm in partnership, it shall be signed by all partners
         of the firm above their full names and current address. Or by a partner holding the
         power of allorney which shall accompany the application. A Certified copy of the
         partnership deed, current address of the firm and the full name and the current
         address of all the partners of the firm shall also accompany the application.

2.3      If the application is made by a limited Company or a limited Corporation, it shall
         be signed by a duly authorised person holding the power of attorney for signing
         the application, in which case, a certified copy of the power of attorney shall
         accompany the application. Such limited company or corporation may be required
         to furnish satisfactory evidence of its existence before pre-qualification is filed.
         They should also furnish article of Memorandum of Association.

2.4      If the application is made by a group of firms, the sponsoring firm shall submit
         complete information pertaining to each firm in the group and state in covering
         letter attached to the application as to which of the firm have the responsibility
         of applying for tender, submission of tender and for completion of contract
         documents and furnish evidence admissible in law in respect of the authority
         assigned to such firm on behalf of the group of firms for applying for tender,
         submission of tender and for completion of contract documents. The full and
         satisfactory evidence pertaining to the participation and responsibility of each
         member of the group of the firm in the tender shall be furnished along with the
         pre-qualification documents duly signed by each firm.

2.5      Witnesses and sureties shall be, person of status and probity and their full names,
         occupations and addresses shall be stated below their signatures.

2.6    If the application is made by a proprietary firm it shall be signed by the proprietor
       above his full type written name and the full name of his firm with its current
       address.

2.7    Any change in the legal status of the applicant, subsequent to submission of Pre-
       qualification Application will render the application as non responsive.

3.0    Final Decision Making Authority:

3.1    Government of Madhya Pradesh, Water Resources Department reserves the right
       to accept or reject any of the Application for pre-qualification without assigning
       any reasons thereof and their decision shall be final.

4.0    Particulars Provisional:
4.1    The particulars of the proposed work given as well as in the brief note are
       provisional and must be considered only as advance information to assist
       applicants.

5.00   Site Visit:

5.1    The site or work may be inspected by the Applicant or his Representative at his
       own cost in consultation with the Executive Engineer, WATER RESOURCES
       DIVISION, UMARIA (M.P.) indicated vide statement under Para 1.1 of the
       invitation for pre-qualification applications. The same is detailed at Appendix -1

6.0    Eligibility for Pre-qualification:

6.1    The application in the same name and styles must be a well established Civil
       Engineering Contractor with five years experience and capability in construction
       of earthen dam and masonry and concrete work of structures or similar civil
       engineering works involving excavation in all types of soil and rock, earth work,
       pitching, rock-toe, structures of stone masonry and reinforced and mass concrete
       work and other items, connected with construction of an earthen dam.

6.2    The applicant in the same name and style must give evidence of having adequate
       financial capability for mobilizing equipment and personnel for similar value of
       contract and in the deployment of suitable equipment for excavation of all type of
       soil and rock, earth work, pitching, rock toe, structures of stone masonry and
       reinforced and mass concrete work stone masonry works and jobs as indicated
       vide 6.1 above.

7.0    Minimum Qualification Criteria:

7.1    To qualify, each applicant in the same Name and style should have in last five
       financial years 2006-07 to 2010-11

(a)       Executed similar work as indicated vide Para 6.1 in any one year during the last
          five financial year 2006-07 to 2010-11 which shall not be less than the minimum
          physical turnover fixed for the work in quantum vide Appendix –II

(b)       Achieved Annual Financial Turnover in any one of the last five financial years
          2006-07 to 2010-11 price level not be less than minimum financial turnover fixed
          for the work in quantum vide Appendix-II

Note:     Turnover of previous years shall be given an additional weightage 10% per year to
          bring them 2010-11 price level to account for price escalation as illustrated below:

7.2       The Physical and financial requirement for Pre-qualification, as per Para 7 are
          given in Appendix – II

7.3       To be eligible for Pre-qualification , applicants shall provide evidence of Madhya
          Pradesh Water Resources Department of their meeting eligibility indicated in Para
          7.1 & 7.2 above .For this the applicants are required to furnish along with their
          applications, certificates obtained from the concerned authorities/employers
          towards proof of their having executed the contracts satisfactorily including
          details of quantities executed . All other certificates should be countersigned by
          the Engineer-in-charge, not below the rank of Executive Engineer towards proof
          of there having executed the contracts satisfactory including details of quantities
          executed physically and financially.

7.4       The application submitted shall also include the following:-

          (a)      Letter of transmittal.

          (b)       Copies of original documents defining the construction or legal status,
                structure and organization, place of registration and principal place of
                business of applicant in For- F-1

          (c)      Report on the financial standing of the Applicant in Form-F-2

          (d)      Details of experience in form F-3 and past performance of the applicant for
                construction of earthen dam and its appurtenant works with masonry and
                P.C.C./R.C.C. works of structures and of similar nature during the last five

              financial year 2006-07 to 2010-11 in Annexure-A , and details of Plant and
              Equipment in Annexure- ‘C’ & ‘D’.

        (e) Any other relevant additional information in Form – F 4

7.5     Statement No. S-1 to S-4 for indicating quantities of various items executed during
        last five financial years 2006-07 to 2010-11 are to be enclosed for furnishing the
        desired information while submitted the pre-qualification applications. Similarly
        Statement No.S-5 is meant for indicating cost of these works completed and in
        hand during the said five financial years 2006-07 to 2010-11

7.8     Even though, the applicants satisfy the above requirements, they are subjected to
        be disqualified if they have.

        (a)       Made signature or false representations in the forms, statements and
              attachments required in pre-qualification documents and or have;

        (b)     Record of poof performance such as abandoning the work, not properly
              completing contracts etc. or financial failures.

7.7     Evaluation Criteria for Pre-qualification.

7.8     The details provided by the applicants will be scrutinized as per physical and
        financial criteria prescribed under Para 7 above and the applicant’s eligibility for
        pre-qualification for the work evaluated.

8.0     Bid Capacity:

8.1     Applicants who meet the minimum qualifying criteria in the evaluation as stated
        above are to be evaluated further for bid capacity as under:

        Bid Capacity = (1.5 A X B)      C

        Where

  A =    Maximum value of civil engineering works executed in any one year during the
         last five year (10% weightage per year shall be given to bring the value of work
         executed at present price level)

  B =    Proposed contract period in years.

  C =    Amount of work in hand at present.

8.2      The applicant will be pre-qualified only, if this bid capacity. (Total cost of work
         as per Annexure-1) is considered adequate for the work.

8.3   At the bid evaluation stage, a careful check of appropriate reference with
      reference to updated information on pre-qualification will be done and in no
      event a contract will be awarded to a bidder lacking the financial and technical
      resources to undertake the work regardless of pre-qualification.

9.0   Pre –qualification applications received after the time and date stipulated will not
      be considered. Water Resources Department will not be responsible for delay in
      receipt due to postal delay or loss, if applicant is sent by post.
