ADDENDUM TO CONTRACT FOR CONSTRUCTION MANAGEMENT OF ___________________________________ This Addendum to Contract for _________________________ (“Addendum”) is made and entered into on the dates shown below by and between ________________________ (“______________”) and _________________ (“____________”). WHEREAS, ____________ and ___________ entered into a _______________________________________ dated _____________, 20____, which contract contained several attached exhibits (collectively the “Agreement”), pursuant to which __________ agreed to provide construction management services for construction of __________________________ (the “Project”) located at ___________________ (the “Facility”) on the terms and conditions set forth in the Agreement; and WHEREAS, completion of the Project has exceeded the time frames estimated by __________ and _________ at the time of entering into the Agreement; and, WHEREAS, _____________ and ____________ desire to supplement and amend the Agreement in order to include the provisions set forth in this Addendum. NOW THEREFORE, Ten Dollars ($10.00) and other good and valuable consideration, the receipt of which is hereby acknowledged, the parties hereto agree as follows: 1. The foregoing recitals shall be incorporated into this Addendum as if fully re-written herein. 2. This Addendum may be executed in several counterparts, each of which shall be deemed an original, but all of which together shall constitute one and the same instrument, and may be canceled, modified or amended only as set forth herein or by a written instrument executed by each party to be charged therewith. 4. In all other respects, the terms and provisions of the Agreement, as supplemented and amended hereby, shall remain in full force and effect.
