Registration                                                This attachment is recognised
                                      as a training position for specialist qualification. In
                                      addition there will be CTA-recognised training
                                      positions.

Functional Relationships:             Health care consumer
                                                               Hospital and
                                      community based health care workers.

Primary objective:         To facilitate the management of patients under the
                           care of the Children’s Health Service

Clinical Duties                      Paediatric registrars will rotate through several
                              positions on a 3-monthly basis under the supervision of
                              the consultant attached to that area. These areas will be:
                                                   1) Neonatal intensive care 1
                                                   2) Neonatal intensive care 2 +
                              Postnatal delivery
                                                   3) Children’s Outpatients Clinic
                              (alternating weekly with evening registrar)
                                                   4) Paediatric ward
                                                   5) Night reliever
                                                   6) Evening Registrar (alternating
                              weekly with clinic registrar)
                                                   7) Leave reliever/rotator
                                                   Amended are timetables for each
                              role.

                                                    When on acute duty the registrar will
                              be responsible for the reception of GP enquires for
                              admissions and arrange to assess patients if necessary.
                              A registrar will attend caesarean sections and be on call
                              to attend any birth in the hospital where there is concern

                              regarding the neonate’s condition. A registrar will be
                              available to attend neonate retrievals.

                                                     Notify the specialist on call of all
                              acute admissions, as soon as possible in the case of
                              seriously ill patients and at mutually agreed times for non-
                              urgent cases.

                                                     NICU 1 & 2 registrars will attend
                              consultant ward rounds (Mon, Wed, Fri) and complete
                              registrar rounds (Tues, Thurs). NICU 2 will attend
                              neonatal consultant outpatient clinic (Broadbent) with
                              outpatient registrar taking Delivery Suite tracer during this
                              time. It is expected that the NICU registrars work
                              together as a team to cover the neonatal service.
                                                     NICU 1 registrar to cover hot room
                              and as above.
                                                     NICU 2 to cover cool room,
                              Postnatal, Delivery Suite and Outpatients as above.
                                                     In addition on Fridays the NICU 2
                              registrar will cover the job of the person going onto nights
                              – with NICU 1 taking the Delivery Suite tracer and
                              covering (with the SHO) the neonatal service.
                                                     They will provide assistance to the
                              neonatal consultants chronic patients as directed by
                              them.
                                                     The shift ends at 1700 hours and
                              until then the NICU registrars will continue to complete
                              work in their respective areas after handing over.

                                                      Ward registrar will attend consultant
                              ward rounds (Mon, Wed, Fri) and complete registrar
                              rounds (Tues, Thurs). They will have a current
                              knowledge of the progress of the inpatients under their
                              care.
                                                      They will review babies in Ward 1A
                              at admission of the parent or caregiver.
                                                      They will also review children on
                              outlying wards and ICU each shift.
                                                      From 1100 onwards (Mon, Wed,
                              Thurs) the ward registrar will take the acute call pager to
                              answer GP and Emergency Department enquiries (prior
                              to this it will be carried and attended to by the clinic
                              registrar). They will be responsible for the running of the
                              ward and PAU with SHO assistance and all acute, and
                              arranged, admissions. The ward registrar will carry the
                              acute pager all day on Tues and Friday.
                                                      The shift ends at 1700 hours and
                              until then the ward registrar will continue to complete their
                              work in the ward and PAU after handing over the acute
                              tracer to the evening registrar.

                                                    Clinic registrar will take the acute
                              children’s pager from handover until 11am (Mon, Wed,
                              Thurs) attending to calls and seeing children in PAU.
                              They will also take arranged venipuncture (0845
                              Outpatients) and place lines as prearranged and required
                              required (lists to be kept at Outpatients desk).
                                                    1100-1200 (Tue – Thurs) will attend
                              a review clinic.
                                                    1200-1230 (Mon – Thurs) the
                              registrar has set aside for oncology reviews (assisted by
                              oncology nursing staff).
                                                     The clinic registrar will attend
                              rostered outpatient clinics each afternoon (Mon – Thurs)
                              and out-of-town clinic once per week (usually Fri) for
                              which registrars need a current drivers’ license.
                                                    They will provide assistance to
                              consultants with chronic children (not covered by the
                              neonatal consultants) as directed and have the
                              opportunity to attend specialist clinics (such as CF and
                              diabetic clinics).
                                                    Outpatients not previously seen in
                              the department, or those who are to be discharged, will
                              normally be discussed with a consultant paediatrician.
                                                    The day shift ends at 1700 hours and
                              until then the daytime registrar will continue to complete
                              children’s outpatient work.

                             Evening registrar will start at 1600 and take handover
initially from NICU and then the ward registrar (taking both acute tracers). They will
then attend to all acute calls until 2300 and handover to the night registrar – with the
assistance of an evening SHO. They will do alternate weeks with the clinic registrar
and provide continuity as much as possible for patients being cared for.

                             Night registrar will attend handover at 2300 hours from
the evening registrar and will take acute call for all areas until mane handover. On
Fri and Sat they will complete the postnatal baby checks and reviews – so to leave
less to cover by the weekend team.
                             When there is not a regular SHO on Friday, the night
registrar will do the baby checks and reviews on Thursday night so to reduce the
work load for the NICU team.

                            Night rotator registrar will cover the person who is
currently doing nights and rotate through the areas doing the jobs as required.

                           Leave relief registrar will cover the person who is on
leave and rotate through the areas doing the jobs as required.

                              Attend:

                              a)      X-ray meeting 1100-1200 Monday

                              b)      Journal club 1300-1400 Tuesday

                              c)      Starship rounds 0800 Wednesday

                              d)      Registrar teaching 1300-1400 Wednesday

                              e)      Department meeting 1300-1400 Thursday

                              f)      Telepaeds seminars 1300-1400 Friday


Administration                        Legible notes will be written in patients’ charts on
                              admission and whenever management changes are
                              made. This may in part be delegated to the house
                              officer. Date, name and time of entry is required.

                                                 Letters will be written to the patient’s
                              GP after each outpatient visit.

                                                    The results of all outpatient
                              investigations will be sighted and signed by a member of
                              the medical team before they are filed in the patient’s file.

                                                     The registrar will dictate a discharge
                              letter to the patient’s G.P following discharge from
                              hospital, within 2 working days where other duties permit
                              as required/specified.


Education                                  The following educational activities will be
                              regarded as part of normal duties:

                              a)      Registar Teaching, Wednesday 1300 - 1400


                                                    Performance will be discussed at
                              formal meetings at the beginning of the attachment and
                              again at 3 months and six months. If deficiencies are
                              identified during the attachment, the consultant to whom
                              the registrar is responsible will bring these to the
                              registrar’s attention and discuss how they may be
                              corrected and give the registrar the opportunity to correct
                              such deficiencies.


Roster                                       Ordinary hours
                                                    Monday to Friday the ordinary hours
                              of work will be 0815-1700hrs. The number of registrars
                              on the out-of-hours roster(1700-2315 Mon-Fri, 0815-2145
                              Sat/Sun) will be 6 on a 1:6 generic roster. A night shift
                              on-call duty will operate from Friday to Friday (2130-0830
                              Sat/Sun and 2300-0830 Monday to Friday) and this will
                              be on a 1:6 basis according to the generic roster.

                                     Outside of ordinary hours the registrar on duty
                              shall be responsible for all medical GP calls, referrals and
                              paediatric admissions.

                              Will be determined once a run review is completed.

Cover for Leave               Cover for leave is provided by the leave reliever.


Salary                                      The salary for this attachment will be as
                              detailed in Category B. Those in relief positions are paid
                              2 categories above.

                                                   Additional hours worked related to
                              out-of-town clinics will be an exclusion to the run hours
                              and will be paid at MECA additional duties rates.
