Mmm dd, 2009 STRICTLY PRIVATE AND CONFIDENTIAL First LAST 99 King Street E Hamilton, ON L1M 2N3 Dear Mr LAST: I am pleased to offer you the temporary position of Research Assistant in the Department of Mechanical Engineering with McMaster University. Reporting to me, you will be responsible for those duties I discussed with you. These duties may be reviewed and modified from time to time based on departmental/organizational needs. Your temporary appointment will commence on Mmm dd, 2009 and will end on Mmm dd, 2010. The scheduled hours of work for this position are: 8:30 a.m. to 4:30 p.m., Monday through Friday for a total of 35 paid hours per week. A one 60 minute unpaid lunch break will be provided. You will be given two week’s written notice of any change in this schedule. Your employment may be ended sooner than Mmm dd, 2010 by yourself or the University upon two week’s written notice (or two week’s pay in lieu of notice) without just cause. Your rate of pay for this assignment will be $##.##/hour, less applicable deductions. Also, as per the Employment Standards Act, S.O. 2000, you agree that any vacation pay owing to you will be paid on every wage payment made to you. Your pay will be deposited directly into a bank account of your choice. A statement of earnings will be sent to your office address each pay. Please complete the attached TD1 government forms and Employee Contact and Deposit Form, and submit to me along with your signed acceptance of this offer letter and a void cheque for direct bank deposit. Please bring your Social Insurance Number card with you on your first day of work. You are responsible to follow University policies, procedures and guidelines as they may apply to you. We will be reviewing the list of main activities involved with your job and will be completing a Job Hazard Analysis Summary. You will be required to complete all mandatory training and any other safety training as identified on the JHA. In addition you must also complete health and safety orientation training at http://www.ltrc.mcmaster.ca/eohss/healthsafety.htm.
