SUBJECT:         Open Enrollment through Benelogic for all Benefit Eligible Employees

           Open enrollment for the 2011-2012 school year for employee benefits will be
       June 1, 2011 – June 30, 2011. All changes for next school year must be made at this time. No changes
       to health insurance benefits will be allowed after the open enrollment period ending June 30, 2011. If
       you do not need to make any changes, then no action is required on your part. Please note, according to
       IRS regulations, you must enroll in Flexible Spending yearly. These changes will not go into effect
       until September 1, 2011.

           In order to make changes, you can access the website through www.qacps.k12.md.us click on
       “Depts.”, “Human Resources” Benefits then Benelogic. You will need your user id number, which is
       your employee id number (which is on your ID badge), and your temporary password will be the last 4
       digits of your social security number. The system will prompt you to change your password at that time.
       Also, new this year the Benefit Overview can be viewed on the QACPS website by going to Depts.,
       Human Resources and Benefits.

          Also, enclosed are the proposed rates for the health insurance deductions effective September 1,
       2011. Please be aware these rates are subject to change due to ongoing negotiations and funding.

     IMPORTANT INFORMATION-BENELOGIC OPEN ENROLLMENT
         Open enrollment for the 2011-2012 school year will be June 1 through June 30, 2011. Any changes would
include deletion of dependents, the addition of eligible employees, or dependents not currently covered. These changes
will be effective September 1, 2011 through August 31, 2012.

        Please note that additions to coverage are only allowed during the open enrollment period, expect for the addition
of a newborn child within 30 days of birth, or to add a spouse within 30 days of marriage.


          Your online benefits open enrollment period through Benelogic will be June 1 – June 30, 2011.
         Please be aware that you will be unable to make any changes to your benefits after June 30, 2011.
                    The effective date for open enrollment changes will be September 1, 2011.
