REGISTRATION AND PAYMENT COLLECTION INSTRUCTIONS FOR LEADERS
1. A leader from each church needs to be in charge of collecting registrations and payments from everyone. Please ensure that people intending to attend the conference but that are not apart of a church (i.e. friend’s of friends) contact the designated church leader. 2. Fill out the attendance form attached to help calculate the lump sum payment needed to be paid to Melbourne Church. 3. Send a copy of the attendance form with the registration forms to Melbourne church and keep a photocopy of the attendance form for yourself. Send the signed registration forms altogether (with the attendance form) to: PO Box 2114 Footscray, Victoria 3011 by the 16th of August 2009. 4. Deposit the lump sum payment into the following account by the 1st of October 2009. If possible make a reference/note when you make the deposit, for example (NYC, your church, state): Account Name: Matthew Pham BSB: 733017 Account Number: 562657 5. Email Trinh Van on: capri_88_ttv@hotmail.com your name, church, date deposited and lump sum payment amount. *A reminder that registration forms are due in on the 16th of August, and payments are due on the 1st of October
