                                 US CELLULAR CONTRACT SUMMARY
                    The following provides a summary of an existing contract with US Cellular

1.1    SCOPE
1.1.1 Purpose
The purpose of this Contract is the provisioning of cellular wireless services and equipment. Contract shall be for
general Agency use.
1.1.2 Inclusive Items
Items of consideration are telephone and data services using wireless cell technology, telephones, radio
telephone to telephone services, wireless modems and transceivers, USB wireless broadband cards, phone
accessories (batteries, charges, headsets, etc.) and associated equipment/services.
1.1.3 Term of Contract
Contract shall commence upon Governor and Executive Council award. The State shall not be invoiced for the
phones unless used by the State during turn-up. Contract shall end on March 15, 2013 The term of the Contract
may be extended for successive twelve (12) month periods for a maximum of twenty-four (24) additional months
subject to approval by Governor and Executive Council. The terms and conditions of the Contract shall remain
fixed throughout the Contract and any extension thereof.
1.1.4 Resulting Orders
Contractor must provide all services and equipment, labor and training required for installation and service at
sites throughout State offices and associated facilities. Orders shall be placed directly by individual State
agencies. All Contractor activities shall be monitored through the Department of Administrative Services,
Telecommunications Section.
1.1.5 Eligible Participants
Other entities that are eligible to utilize the contracts resulting from this RFB are: counties, cities, towns, school
districts, special districts or precincts, governmental sub-divisions, any non-profit agency certified under the
provisions of section 501(c)(3) of the Federal Internal Revenue Code, and the NH College and University
Council as provided by RSA 21:I:17. The State shall not be liable for any breach of Contract by these entities.
Political sub-divisions and authorized non-profit organizations shall utilize their own individually established
purchasing procedures.
1.1.6 Audits and Accounting
The successful Contractor shall allow representatives of the State of New Hampshire to have complete access
to relevant records for the purpose of determining compliance with the terms and conditions of this Contract.
1.2    PURCHASE OUTSIDE OF CONTRACT
The State reserves the right to purchase product outside of this Contract where it is deemed appropriate by the
State.
1.3    NEWS RELEASES
Public announcements or news releases pertaining to this Contract must not be made without prior written
approval of the State.
1.4    CONTRACTOR CERTIFICATION
Contractor must maintain registration as a Contractor authorized to conduct business in the State of New
Hampshire.

STATE OF NEW HAMPSHIRE CONTRACTOR APPLICATION: Contractor must have a current Alternate W-9 filing filed
with the NH Bureau of Purchase and Property.

NEW HAMPSHIRE SECRETARY OF STATE REGISTRATION: Contractor must maintain a current registration with the New
Hampshire Secretary of State and remain in good standing with the State of New Hampshire.
SECTION 2 CONTRACTOR PERFORMANCE REQUIREMENTS
2.1    TYPE OF SERVICE
         Contractor must provide the following services: New Hampshire based traditional cellular telephone
service, National grouped minute services, as well as Low and high speed data service.
2.1.1 Traditional Cellular Telephone Service, Domestic Use
Traditional cellular telephone services shall allow users to directly dial any telephone number which is available
through the Public Branch Exchanges as available from Local Exchange Carriers, Competitive Local Exchange
Carriers, Long Distance Carriers and competitive Cellular Telephone Service providers. Access to any
telephone subscriber number shall not be restricted. Services shall include typical “off the shelf” features and
telephone operation. Equipment provided for use with this service shall be referred to as Traditional Cellular
Telephone (TCT) equipment. One free voice device shall be offered for each user one time per line. Users shall
be eligible for upgrade at prices included in the Contract.
2.1.2 Access Data Services
Contractor shall provide data transport at multiple speeds, limited by the use of Contractor technology.
Common is CDMA2000 1xRTT for peak transfer of up to 144KBps and CDMA2003x (EV-DO) TIA-856 Rev A for
transfer up to 3.1MBbs and Rev B for up to 4.9 MBps. The primary use of the service is for in Internet access and
data transfer through tunneling using “Wireless Air Cards”. Contractor shall provide all necessary items
including ISP operations, access and equipment when directed by the State. Contractor shall not restrict site
address access.
Equipment provided for use with this service shall be referred to as “Wireless Air Card” stationary or mobile
equipment. One free “Wireless Air Card” device (shall be offered for each user one time per line. Otherwise
devices shall be eligible for upgrade at prices included in the Contract. One free “Wireless Air Card” device
shall be provided to the Department of Information Technology (DoIT) , Director of Technical Support for a 30
day evaluation and use approval prior to distribution to users. Devices shall be returned to the Contractor after
evaluation. Rejected devices shall be replaced with DoIT approved devices.
2.1.3 Smart Phone Services
A Smart phone is defined as a mobile phone with advanced capabilities including PC-like functionality with
access to text messaging, e-mail and web browsing. Phones must be capable of accessing Microsoft
Exchange Server e-mail and include Personal Digital Assistant (PDA) capabilities including calendaring..
Access must be provided throughout the Contractor footprint of the continental United States. Equipment
functionality must include the ability to open MS Excel, MS Word and Adobe Acrobat files. Common phones
with fully functional features include the Palm Centro, Palm Treo, Motorola Moto Q, Samsung BlackJack, and
other similar phones. Microsoft Windows Mobile devices are preferred. Equipment provided for use with this
service shall be referred to as Smart Phone (SM) equipment. One free “Smart Phone” device shall be offered
for each user one time per line. Device users shall be eligible for upgrade at prices included in the Contract.

One free “Smart Phone” sample device shall be provided to the Department of Information Technology (DoIT) ,
Director of Technical Support for a 30 day evaluation and use approval prior to distribution to users. Devices
shall be returned to the Contractor after evaluation. Rejected devices shall be replaced with DoIT approved
devices.

Contractor shall provide synchronization software such as released by Intellisync or similar providers for each
Smart phone distributed by the Contractor. One free sample software package shall be provided to the
Department of Information Technology (DoIT), Director of Technical Support, for a 30 day evaluation and use
approval prior to distribution to State users. Software shall remain as property of the State after evaluation.
Rejected software shall be replaced with DoIT approved software.

2.2    COVERAGE
       The Contractor must maintain respective service coverage of the minimum geographic areas for each
proposed service. The Contractor must also ensure same coverage is available 97% of the time for the
respective geographic area.
2.2.1 Traditional Cellular Telephone Service
80% coverage of the geographic area of New Hampshire;
Nation wide coverage within Contractor service footprint.
2.2.2 ISP Access Data Services
20% coverage of geographic area of New Hampshire;
Nation wide coverage within Contractor service footprint.
2.3     LICENSES
        The Contractor must currently hold and retain throughout the duration of the Contract, all licenses or
certificates required by the State and Federal authorities inclusive of the Federal Communications Commission
and State of New Hampshire Public Utilities Commission. The Contractor must file with the appropriate
regulatory body, any tariff, amendments, or special Contract offerings to ensure that the required terms and
conditions of this Contract are met. The Contractor shall cooperate fully with the PUC to ensure that all time
schedules noted within are met.
        In the event of loss of license or permit to provide services as defined, the Contract shall be nullified;
with the State free to engage in an agreement with any Contractor as becomes necessary to continue
services without retribution to the original Contractor.
2.4     INTRODUCTION OF SERVICES
The Contractor must insure that services do not operate in conflict with alternate service providers. Contractor
must provide all user cellular equipment inclusive of telephones, data modems and associated devices, and
deploy prior to March 14, 2010 in order to insure that the State is not without service for any period of time
during transfer of service from an incumbent Contractor. Additional equipment shall be provided when
requested to support new subscribers after start of Contract. Only services requested and authorized by the
State shall be replaced. The Contractors shall communicate with Agency contacts and State users for
coordination with the distribution of equipment.
2.5     RETENTION OF EXISTING TELEPHONE NUMBERS
The State may retain any existing cellular telephone number currently assigned to a State subscriber.
Contractor shall be responsible to port over numbers to the proposed service as requested by the user. The
porting of numbers shall not delay service installation nor result in a user being without service.
2.6     ORDERING PROCEDURE
2.6.1 Telephoned Requests
State contacts may call the Contractor at any time between 8:00 A.M. and 4:30 P.M. to request services. The
Contractor shall respond and provide appropriate service as requested.
2.6.2 Contractor Signature Documents
Any signature forms used by the Contractor to reflect service requests or delivery of service by the Contractor
to the State shall refer to the Contract resulting from this RFB All other terms and conditions shall be null and
void.
2.6.3 Purchase Orders
The State may mail or fax Purchase Orders as requests for service. Contractor shall accept State Purchase
Orders whenever used.
2.6.4 Initial Service Requests
The Contractor shall contact designated State agencies to perform a service needs analysis as directed by the State at
the initiation of the Contract. The Contractor must meet with each designated agency to determine the count and type of
telephones, service program and delivery of replacement services. The needs analysis must be completed prior to
deployment. A complete deployment plan including agency, end user, telephone number, equipment supplied, cost and
cost plan must be provided to the Department of Administrative Services Telecommunications Section prior to
deployment. Political sub-divisions and authorized non-profit organizations shall utilize their own individually established
ordering procedures.
2.6.5 Addition and Removal of Users
Telephone subscriptions may be added or removed from service at any time during the Contract, with a
maximum requirement of a 30-day or one calendar month activation period with no termination liability. All
service agreements shall be coterminous with the termination date of this Contract. All Contractor provided
equipment (telephone, charger, belt-clip, etc.) shall be returned to the Contractor within 30 days of request of
line removal.
2.6.6 Cancellation of Former Services
Contractor shall be responsible to cancel any previously provided Contractor cell phone services to the State
when replacing services with the newly contracted service. If the previously provided service was provided by
an alternate contractor, the State shall be responsible for canceling service.
2.7     DELIVERY
2.7.1 Delivery and Installation
The complete delivery of all equipment and provisioning of services shall be completed prior to March 14, 2010.
Any new services requested after this date shall be delivered within 7 calendar days of transmission of order by
the State. The installation of all materials and equipment shall be in accordance with the manufacturer’s
instructions. The Contractor shall be responsible to furnish or cause to be furnished, all labor, supervision,
equipment, materials and supplies necessary to install and operate the proposed services and equipment.
2.7.2 FOB Destination
Delivery is to be made FOB destination for any order. All orders shall be shipped prepaid at the
Contractor’s expense. No shipping charges shall be invoiced to the State by an outside carrier. Shipments
must be properly packaged to meet accepted commercial standards, which will prevent damage or
contamination, without extra charge to the State.
2.7.3 Returns
Return Authorization credits shall be provided without penalty for faulty equipment. Contractor shall be
responsible for all shipping charges for equipment returned. Defective telephones, accessories and
associated equipment shall be replaced within three (3) State business days of report of failure. Such
service shall be available to State users by placing a single telephone call to the Contractor.
2.7.4 User Training
The Contractor must train each user on the operation of the individual telephone, service access and
features upon delivery of equipment. Complete training must be available at each individual State office.
Continued support shall be provided to train new users and provide refresher training for others when
requested by the State.
2.7.5 User Documentation
The Contractor shall provide user instruction manuals and associated documentation with each telephone
provided. Manuals shall include detailed operation of all telephones, accessories and system operations.
System operational manuals shall be provided to users who prefer to utilize existing telephones.
2.8    CUSTOMER SUPPORT SERVICES
The Contractor must provide complete customer support inclusive of the following:
2.8.1 Account Management
The Contractor must provide a single point of contact for the State or its representative. Contractor must work in
conjunction with any State Contractor regarding the interface of any and all Contractor or State provided and
supported communications equipment.
2.8.2 Account Team Access
Contractor must provide telephone, facsimile, and Internet e-mail access to each individual on the Contractor
account team. General toll free numbers must be provided for telephone and facsimile services on a statewide
basis.
2.8.3 Billing Support
The Contractor shall assign a dedicated financial representative to the State account that will cooperate with the
State to resolve billing, call detail, equipment programming, data discrepancies and all other aspects of the
Contract. The Contractor shall be capable of receiving electronic payments from the State.
If a Contractor subcontracts any or all of the products and/or services covered by this agreement, the Contractor
must be the central point of contact for all matters of ordering, billing, invoicing, and delivery. Use of a
subcontractor does not release the Contractor of any responsibility. Bills and invoices for products and services
must be issued by and payable to the Contractor. Questions related to billing or delivery of service and/or
equipment will be handled promptly by the Contractor and not the Contractor’s subcontractor(s).
2.8.4 Problem Resolution
The Contractor must have a single contact person, available from 8:00 A.M. to 4:30 P.M. during State workdays for
the resolution of problems. The contact person must be authorized to provide invoice corrections, initiate repair
and equipment replacement processes and expedite services.
The State will designate a Contract Administrator who will work with the Contractor to resolve problems that cannot
be resolved by the agency end-users.
The Contract Administrator may impose a moratorium on the Contract pending resolution of any controversy that
arises regarding services to be provided pursuant to this Contract or take other action deemed necessary.
2.8.5 Training
Contractor shall provide on State site training to insure proper telephone and system operation. Training shall be
available on all State business days, during the hours of 7:30 a.m. to 4:30 p.m.
2.9    REMOVAL AND REPLACEMENT OF EMPLOYEES
The State retains the right to require the Contractor to remove and replace any employee whose actions or
appearance are not consistent with the standards of the State. Such employee shall be replaced with a
different qualified Contractor employee meeting State acceptance.
2.10   TECHNOLOGY UPDATES
The influx and transition of new technology may result in the disconnection or discontinuation of a defined service
and connection to new technology. The Contractor may not impose any charges for a given service when the
State discontinues one service for the replacement of another while remaining a customer of that same
Contractor. Contractor shall replace any interface equipment which becomes obsolete due to Contractor service
updates.
All users shall be notified thirty business days prior to system operation changes, inclusive of equipment updates and
software/firmware updates.
2.11    CONFIDENTIAL INFORMATION
The Contractor agrees that all discussions or information gained during an engagement shall be considered
confidential and that no information gathered by the Contractor shall be released without prior consent of the
State.
2.12    INVOICING AND REPORTING REQUIREMENTS
Invoices and reports are required throughout the duration of the Contract as denoted below.
2.12.1 Monthly Reports
The Contractor must provide a Monthly Summary Report forwarded to Department of Administrative
Services, Telecommunications Section detailing service. Included shall be all cellular telephone numbers
subscribed to the State, billing program used, monthly cost, usage cost, equipment provided, telephone
“owner”, State agency responsible for billing and contact person.
2.12.2 Invoicing Services
Invoicing Services shall consist of the costing of all Contractor-provided services. Contractor shall detail all charges
and provide extended definitions of itemized charges. Bulked charges shall be rejected by the State. A separate
invoice shall be available for each telephone subscribed to each State office. The State may require that multiple
telephones be billed on a single account. In such cases, the Contractor shall place charges on a single master bill.
Invoices shall be submitted on a monthly basis, inclusive of the first bill cycle day of the month to the last bill cycle
day of the month in which the services have been performed. Charges will be pro-rated by the numbers of days in
service when partial month services occur.
2.12.3 Invoice Details
All monthly reports and invoices must be itemized including details for every Contractor billable item, inclusive of
basic monthly charges, minutes of use, excess minutes of use, call detail inclusive of city/town location and
telephone number called. All call records must be listed in sequential order by date of call.
2.12.4 Invoice Corrections
The Contractor shall provide within ten (10) working days, any corrective data requested by the State. This shall
include replacement reports, corrective information on balances and credits and any other change of service
information required for bill back to agencies by the State.
2.12.5 Invoice and Report Delivery Timeframe
Invoices and Reports must be delivered on or prior to 25 days after the end date of the monthly bill cycle. The
State shall not be held liable for service performed without the receipt of properly filed invoices, reports and
supporting information. Invoices shall be forwarded directly to State users, and may be forwarded as a group
to same addresses when directed by the State.
2.12.6 Account Balances
The Contractor shall maintain all records of payments, credits and balances.
2.12.7 Accuracy of Invoices
Contractor shall be responsible to justify all charges to the State. Invoices must be reviewed for accuracy prior
to delivery to the State. All billing information provided to the State must reflect same information. Paper
invoices and electronic reports must reflect same call detail, record count, call and service cost. Conflicts
between support data and paper billing shall be considered incomplete billing and payment held until proper
support information provided by the Contractor.
2.12.8 Paper
Invoices must be provided on paper. It is preferable that invoices also be available electronically, delivered by
e-mail or downloadable from an Internet web site.
2.12.9 Electronic Media
Monthly reports must be provided in electronic PC format with all files provided as Open Data Base Compliant
(ODBC) ASCII flat files. All such reports shall be e-mailed to dennis.leclerc@nh.gov and ellen.charron@nh.gov
or alternate addresses when provided by the State.
2.13   REPAIR AND INSTALLATION SERVICES
The Contractor shall make service available 24 hours per day, seven (7) days per week. The Contractor shall
be responsible to implement appropriate maintenance to insure continued operation of all services throughout
the duration of the Contract.
2.13.1 Toll Free Telephone Number
A toll free telephone number shall be provided for trouble reporting and immediate service assistance.
Personnel must be knowledgeable of the Contract services and devices as configured for the State.
2.13.2 Escalation to Second Level Support
Escalation to second level support shall be provided for trouble reports not addressed and corrected within a
24 hour time period.
2.14   CONTRACTOR AS PRIME CONTRACTOR
Contractor warrants that they are the prime contractor for the business defined within this Contract.
2.14.1 Subcontractor Work
The Contractor shall provide a complete list of all subcontractors and their work performed during the
Contract. This requirement shall be exclusive of network backbone carriers or call completion carriers used
within the Contractor’s network.
2.14.2 Subcontractor Performance
The Contractor shall be responsible for the performance of any subcontractors that are used.
2.15   SPECIFICATION COMPLIANCE
The Contractor may be required to supply proof of compliance with Contract specifications. When requested,
the Contractor must immediately supply the State with certified test results or certificates of compliance unless
such information is confidential and proprietary. If the information is confidential and proprietary it may be
subject to a non disclosure agreement in compliance with RSA 91-A... Where none are available, the State
may require independent testing. All costs for such testing, certified test results, or certificate of compliance
shall be the responsibility of the Contractor.
2.16   INTRODUCTION OF NEW TECHNOLOGY
Contractor shall provide notification to the State prior to technology updates requiring end user equipment
replacement or modifications in methods of calling. Whenever such changes are made, the State may
request sample equipment to verify that services operate within the parameters of the Contract.
The presentation of any new service not current in the Contract will require the issuance of a Contract
amendment.
SECTION 3 SERVICES
3.1    TELEPHONES AND EQUIPMENT
Contractor shall provide a single end user device for each service end user, inclusive of Traditional Cellular
Telephones and Data services. The Contractor shall also offer equipment for purchase at the discretion of the
State. The Contractor must provide cellular service for compatible equipment that is currently owned by the
State. Services for all equipment shall include equipment programming, repair, installation and instructional
assistance. All user equipment shall be retained as property of the State once delivered to end-users.
Equipment may be substituted at any time after equipment review and acceptance by the State.
3.1.1 Accessories
All telephones provided by the Contractor shall be complete and ready to use with the following
attachments:
•        120 Vac battery charger/adaptor
•        12 Vdc battery adaptor/charger
•        Belt clip holster or belt clip carrying case
•        Earpiece (ear-bud)
3.1.2 Hand Held Portable Telephones
Traditional Cellular Telephones shall be new or like new sets. Telephones must allow and use of each service
feature proposed by the Contractor and incorporate hands free talk-back, corded earpiece, graphic display,
internal call directory and other common phone features. A base phone shall be provided at no charge, with
optional feature phones proposed for purchase.
3.1.3 Radio Phone to Phone Telephones
Radio Phone to Phone Telephones shall be new or like new sets. Telephones must include all of the items in
the above listed Hand Held Portable Telephones and use of each service feature proposed for the Radio
Phone to Phone Telephone service. A phone shall be provided at no charge, with optional feature phones
proposed for purchase.
3.1.4 In Vehicle Telephones
Contractor must provide in vehicle service kits enabling telephones to be easily used in State vehicles.
Equipment shall include the following features:
•        Wireless earphone or headphone and microphone;
•        Hands free talk-back capability;
•        12 Vdc vehicle power adaptor;
•        Telephone holder.
3.1.5 ISP Access Data Equipment
Contractor must provide Internet access equipment inclusive of Sierra Wireless Aircards, Novatel Wireless
Ovation or equipment with same performance and similar features, designed for use with laptop, mobile or
stationary data equipment. Contractor must provide a full physical, operating and technical description of
each device offered.
Contractor must provide a full range of “smart” cell phones such as those offered by Palm, Nokia, HTC and
other manufactures.
3.1.6 Existing Equipment
Equipment currently owned by the State, when compatible, must be serviced by the Contractor. Services
shall include equipment programming, repair as available from the Contractor, removal and installation of
equipment in State vehicles, and operational instructional assistance.
3.1.7 Equipment Maintenance and Warranty
The Contractor must have at their disposal installation and support technicians to provide support services for
all equipment supplied by the Contractor. All equipment must remain fully functional throughout the duration
of the Contract. Any equipment purchased by the State shall be provided with a three (3) year warranty,
resulting in free of charge repair or replacement of any device that becomes faulty. Replacement of such
devices may be with a certified like new device. Replacement shall be provided within three (3) State business
days of report. It will be acceptable through the course of the Contract to substitute alternate equipment as
manufacturers discontinue products and introduce new equipment. Acceptance of such equipment shall be
at the discretion of the State.
As an alternative, Contractor may provide one year equipment warranty with equipment replacement
available for each year of the Contract, resulting in complete equipment coverage for the entire duration of
the Contract.
End users will return to the Contractor, any equipment for which the end user receives a replacement device
due to equipment failures as defined within this section 3.1.7.
3.1.8 Quality of Equipment
All equipment provided shall be new or of factory refurbished like new quality. The State retains the right to
reject any equipment which does not provide a showroom appearance and equivalent operation.
3.1.9 Insurance
The Contractor may supply equipment insurance providing replacement of lost, stolen or damaged
telephones. Insurance shall be at the option of the subscriber and not a requirement of the Contractor.
3.2    SECURITY
The Contractor must maintain network security at all times, disallowing network facility access by unauthorized
users. When fraudulent use is detected, the Contractor must contact the State and discontinue service for a
designated telephone number if directed by the State.
3.3    NATIONAL SERVICE COMPATIBILITY
The service shall be interactive with other Contractor services allowing functionality throughout most areas of
the United States.
3.4    911 AND FCC SERVICE COMPATIBILITY
Contractor services and equipment must meet all FCC, State E911 and Federal E911 mandates.
3.5    SERVICE FEATURES
The Contractor shall provide service features as defined below.
3.5.1 Call Forwarding
Forwarding of calls to alternate lines and/or voice mail. Transfer may be invoked either if calls are unanswered
after a designated number of rings or if line is busy.
3.5.2 Voice Mail
An answering system allowing calls to forward to an automated message center for call message recording
and playback.
3.5.3 Text Messaging
Users shall have the ability to send and receive text messages using telephones.
3.5.4 Caller ID:
Display of originating caller telephone number.

                                                    PRICE
Contractors shall provide all services based upon the terms and cost reflected in this section.

1. Promotional Offers
During the Contract period, the Contractor is required to invoice the State according to the pricing provided in
EXHIBIT B. Prices shall be fixed, except the State shall be advised of, and possibly receive benefit of, any
promotional offer or price decrease allotted any other customer of similar size, utilizing a similar number of total
phones. The Contractor shall not discontinue any service pricing once subscription has been accepted and
used by the State, unless resulting in lower prices.
An up-to-date price list, reflecting State pricing and additional discounts must be e-mailed to
dennis.leclerc@nh.gov on a quarterly basis, defined as December 1, March 1, June 1 and September 1 of
each Contract year throughout the duration of the Contract. All state agencies are eligible to make direct
Contractor request for services provided to them, and to receive copy. Price lists must be maintained
current.
2. Additional Discounts
The Contractor may offer additional discounts such as: quantity discounts, percent off or cash discounts for
early payment, educational, special promotions, etc. All such discounts must be made available to all State
Agencies when offered.
3. Payment
Payment shall be due in full within thirty (30) days after receipt of a properly documented invoice and reports.
Invoices shall be forwarded to each office with payments being the responsibility of each office. Late
payment charges shall not apply.
4. Equipment Price
The Contractor shall provide one voice, “Wireless Air Card” or “Smart Phone” device at no charge, one time,
per line or service for each applicable line number or service used by the State. No one-time charge shall
apply unless the State selects to “upgrade” to different equipment.
All Contract users shall be eligible for one free voice, “Wireless Air Card” or “Smart Phone” (telephone, data
interface, etc.) only one time per line; For “Smartphones”, the credit shall be valid for up to 180 days from the
commencement of Contract. End user shall receive credit for future receipt of device if not received at time
of Contract initiation, allowing the user to receive a free device at any time during the Contract.
5. Quoted Prices
Should the Contractor fail to either include in the quoted price, or deliver to the State, any components or
features that are necessary to deliver the wireless communications services as defined within, the Contractor
shall be required to provide same at the Contractor’s expense.
•       There shall be no charge for voice mail services, including delivery and user access minutes.
•       No toll charges will result from calling within geographic New Hampshire for New Hampshire based
        services.
•       No toll charges will result from calling within continental United States for US based services.
•       There shall be no charges for calls between US Cellular customers within the U.S. Cellular owned and
        operated service areas.
6. Quantity
The number of devices or services may vary. Services may be added or deleted from this Contract at any
time, at no penalty to the State, and at the prices listed herein.
7. Price Tables
The following tables shall reflect all additional cost of services. The State shall not be responsible to pay Federal
or State of New Hampshire taxes.
Table 1 Traditional Cellular Telephone, NH Based Service Costs:
Costs of service for traditional cellular telephone calling for phones subscribed to geographic New Hampshire
as the home service area. Calls between telephones subscribed to the same Contractor when used within
New Hampshire shall not incur any usage charges.
Table 2 National Grouped Minute Service Costs
Cost of services where all minutes of use are bundled to be shared between multiple telephones. Services may
be composed of a base phone cost and base grouped minute cost or grouped minutes between phones.
Phones are to be subscribed to the geographic continental United States as the home service area.
Table 3 ISP Access Data Service Costs: Base Service CDMA2000, 1xRTT (or similar)
Costs of service for ISP access and data transfer as available from the equipment types noted.
Table 4 ISP Access Data Service Costs, Advance Data Speeds
Costs of service for ISP access and data transfer as available from the equipment types noted. Speeds must
exceed 1xRTT operation noted in table 3 (EV-DO, 1xRTT, Rev A, Rev B, etc).
Table 5 Supplemental Charges
Additional costs based on Federal and State regulatory rates which may vary with changes by regulating
authority.
Table 6 Telephone and Equipment Costs
One-time costs of telephones and equipment.
Table 7 Balance of Product Line
Additional items, features or services not included in previous tables.
8. Price Table Headings
The following table headings apply. Not all headings are included in each table.
Qty: The quantity of telephones or devices.
Plan Cost per Phone (Device): The fixed monthly charge for plan per phone or device
Outgoing Minutes per Phone: The total call minutes included in the Plan Cost per Phone.
Included Shared Minutes per Phone: The total call minutes included in the Plan Cost per Phone. Total minutes
may be shared between phones.
Cost per Additional Outgoing Minute: The cost of usage per minute for calls in addition to the included
Incoming Minutes per Phone or bulk call minutes.
Cost per US (Non-NH) Minute: The cost of a single out going US call minute (non-NH). Cost shall stand on its
own, and not be in addition to In State Out Going Minute cost.
Cost per Text Message: The cost of sending or receiving a single text message.
Roaming Charges U.S. (per phone per day): The daily surcharge for making calls from outside of New
Hampshire. This heading only applies to Table 1.
Type of Service: Data services as defined within Contract text, summarized as Stationary Data Access Service
provided through wireless devices such as Aircard, Novatel or similar manufacturer equipment; or Smart Phone
devices which include QWERTY data keypad, text messaging, web browsing and e-mail access..
Monthly Data Transfer Allowance: The minimum amount of data that may be transferred within a billing month
as the cost quoted in the Plan cost per Device.
Item Name and Definition: Definition of the specific charge.
Cost: The cost of the item or service.
Item: Type of device to be provided by the Contractor
Category: The service category of the device provided: TCT, Traditional Cellular Telephone; RPP, Radio Phone-
to Phone; MS, Mobile or Stationary data access equipment; or SP, Smart Phone. One device per service user
shall be provided one time per line to the State at no charge.
Manufacturer: The name of the company making or distributing the item.
Model/Version: The model name and number of the item and/or software version if applicable.
One Time cost per Item: The cost to purchase the item for the service. One device will be provided to the
State one time per line at no charge. Contractor may propose additional devices as included within this table.
Description: Description of service or equipment provided.
Label: The label for each assigned cost (i.e. per phone, per service, per month, one time, etc.)

9. No Charge Items
The following items are to be included in the Plan Cost Per Phone as no charge items:
Mobile to mobile calls between Contractor telephones;
Unlimited access, receipt, and use of Voice Mail;
Caller ID;

10. Turn-up Charges
The State shall not be invoiced for telephones distributed during the turn-up phase of service, defined as the
time between governor and Council award through March 14, 2010 unless used by the State.
The invoice for telephones distributed during the turn-up phase of service (i.e., the between Governor and Council
Award (“Effective Date”) and March 14, 2010 in order to allow for efficient turn-up on March 14, 2010, will not be
due from the State until thirty (30) days after March 14, 2010.
