							WCI supports adults with intellectual and developmental disabilities. These
include physical, sensory, behavioral, and psychiatric needs, as well as
significant medical conditions.


HEALTHCARE COORDINATOR NURSE (RN)

Residential Support Divisions
Schedule: Full-time 40 hour a week position; schedule offers some
flexibility, but includes some evening and weekend hours
Pay level: TBD

WCI is looking for a Full-time Healthcare Coordinator Nurse (RN) for a 40
hour a week position with some flexibility in schedule (some weekend and
evening hours may be required). The Nurse will have a caseload and
perform basic health checks and oversight of the health and safety of the
Individuals in their home, as well as perform initial and ongoing medical
assessments, insure compliance with primarily DDS and MAP as well as
Mass Health Regulations, interface with physicians and clinicians, and act in
a leadership role on the Interdisciplinary Team. Must be able to work with
Deaf Individuals, staff, sign language interpreters, and also be willing to
learn some ASL and other facets of Deaf culture. Must have a professional
and positive approach in the performance of all tasks and interactions.

Other responsibilities include: Medication Administration training and
oversight of supervisory & direct support staff, direct oversight of the
medical needs of the Individuals, including writing of Protocols, Supervisory
and staff training in managing Medical Records and conducting Medical
Record Audits; developing and maintaining Health Care Plans; Training
staff in the medical conditions of the Individuals, their medications, possible
side effects, and agency policies and procedures regarding health related
practices and documentation including Agency-wide medical condition
training; coordinating medical services with PCPs and medical treatment
facilities; coordinating admission and discharge meetings with hospitals;
maintaining contact with all necessary parties as necessary (physicians,
family members, service coordinators); participating in Team meetings; and
all other duties consistent with the responsibilities of an agency Nurse. Must
be able to speak to groups, such as when conducting agency-wide trainings,
and lead group discussions when appropriate.
Minimum requirements include: Current RN Licensure with a BA in
Nursing; valid CPR and First Aid certifications; 3-5 years experience as an
RN preferably working with adults with intellectual and developmental
disabilities (or a similar environment); a warm, caring, and patient
personality with strong administrative, communication, relationship-
building, and report writing skills. Communication and interpersonal skills
with both internal and external parties are essential. Proven ability to work
within a Team process. MAP Trainer Certification preferred.

supporting adults with intellectual and developmental disabilities with
activities of daily life at home and in the community); CPR, First Aid
required; MAP preferred; ASL fluency required; experience in the day to
day running of a residential home for individuals with intellectual and
developmental disabilities preferred; supervisory exp required; Bachelor’s
degree or higher preferred). A commitment to the field; dependable and
