					This template document, from a Landlord to a Tenant, puts the Tenant on notice that
they are in default and will be evicted, pursuant to the terms of the lease. This eviction
notice requires the Tenant to either pay rent, which is past due, or vacate the premises
and surrender possession. Landlords should use this form to evict tenants and the
provisions for time of notice, fees, and other options are all customizable to fit the
unique needs of a landlord.

required time, a legal action will be filed seeking not only damages and possession, but also

section ____. Landlord declares a forfeiture of the lease if past due rent is not paid and you
continue to occupy the Premises. As required by law, you are hereby notified that a negative
credit report may be submitted to a credit reporting agency if you fail to pay your rent.

This Eviction Notice was served by:

    1. ____ Personal Service. A copy of the Notice was personally delivered to the above-

       and discretion at the Tenant’s residence or place of business and a copy was mailed to the
