    This product recall is to alert you to a potential safety issue with the devices listed below that, in extreme
    situations, could lead to property loss or serious personal injury, including death.


    Subject: Contractors to participate in product recall of Siemens plug-in type “QP” circuit breakers &
    X-Press Pack loadcentres


    Dear Contractor,

    In an effort to implement corrective action in regards to a recent safety notice / product recall issued by
    Siemens, contractors are obligated to take immediate action as listed below:

    1. Identify affected products in your possession; and
    2. Quarantine and return affected material in your possession to Siemens; and
    3. Provide Point-Of-Sale (P.O.S. & Site of install) information on sales of impacted products for the month of
       June 2010, July 2010, and August 2010 to Siemens using the enclosed “Customer Acknowledge Form”.

    Listed below is the step-by-step procedure that you are asked to follow for this product recall:

    Step 1: Identify affected material
    Refer to the enclosed “Product Identification Guide” for details on product identification.

    Step 2: Quarantine, Return and Replacement procedure
    Isolate and quarantine any affected products you may have in your inventory immediately.
    Complete the enclosed “RMA” form and call the Recall Hotline 1-888-557-5588 to finalize the return and
    replacement procedure.

    Step 3: Customer Identification
    A product recall obligates installers and contractors to provide the vendor, Siemens Canada Limited in this
    case, with a list of all customers you have sold to or installed the products affected at. Please complete the
    enclosed “Customer Response Form” and send it to Siemens either via email (q.recall.ca@siemens.com) or
    fax (1-905-335-3098) as soon as possible.

    Siemens will contact your customers via registered mail to advise them of the product recall. Siemens will ask
    them to contact our recall hotline and arrange for a service call to conduct an inspection and correction to their
    panels.

    If you wish to participate in replacement activities on behalf of Siemens Canada limited for the duration of the
    recall, kindly call the Recall Hotline 1-888-557-5588 and ask the operator to send you a copy of the “Recall
    Service Contract”.

    Service calls will require a certified electrician (Note: Ontario requires that electrical work be done with
    licensed electrical contractors that utilize certified electricians. The other provinces and territories may have
    similar requirements) to examine installed breakers and verify whether they are subject to this recall. Breakers
    affected by this recall need to be replaced and then returned to Siemens. The service call will also require an
    inspection of the loadcentre and surrounding breakers to ensure that their functionality has not been
    compromised by the affected breakers. If this is the case, these too need to be replaced and returned to
    Siemens. Further information on what substantiates compromised functionality of loadcentres and
    surrounding breakers is contained within the "Siemens Breaker Recall Service Contract".

    Further information about product replacement procedures for end customer / home owner and payment for
    service calls is contained in the “Recall Service Contract”.

    If you do not wish to participate in the product recall service calls program, Siemens will dispatch a licensed
    electrician to perform the required corrective actions.

    I, the respondent, acknowledge receipt of the Siemens Type “QP” Plug-In Circuit Breakers
    and X-Press Pack loadcentres Product Recall / Safety Notice and I hereby confirm that:


    A) For those products that my company has already sold (please check the appropriate box)

        A1) I will supply Siemens Canada Limited with a list of the customers that may have the
          potentially affected products, and authorize Siemens Canada Limited to contact them directly.
          My customer list is attached in table 1 below. This list covers in total _______ products sold.

        A2) I do not have a complete list of customers who have these products in their facilities and I
           am unable to locate these remaining products. No further action is required by Siemens
           Canada Limited for unlisted customers.


    B) For those products that my company has not yet sold (please check the appropriate box)

        B1) I have already quarantined our inventory and processed our return.

        B2) I have quarantined the affected products from our inventory and will return them to Siemens
           Canada Limited by following the RMA procedure outlined in this letter.
