I.       Only authorized purchasers included in the State of Washington Purchasing Cooperative (WSPC)

II.      Contract Terms: This Document includes by reference all terms and conditions published in the original
         IFB, including Standard Terms and Conditions, and Definitions, included in the Competitive
         Procurement Standards published by OSP (as Amended).


1. 04/07/08: With this CCI, contract is allowed to expire May 31, 2008 without being replaced. Customers
   may use their authority in accordance with the Washington State Purchasing Manual.
2. 03/12/08: This CCI is being issued to update OSP contact information.
3. 04/04/05: per customer request, contract has been extended for an additional two year term contingent with
   a six percent price increase as reflected on the attached price sheet. The remainder of the contract terms,
   conditions and specifications are unchanged.

4. Training Requirements: supplier is required, to the extent possible and based upon mutual agreement with
   the purchaser(s), to provide training on how to utilize the products specified herein. This can be
   accomplished by a method as simple as a video or well detailed instruction manual, or if deemed necessary,
   an on-site, hands on training of agency personnel. Training shall be provided at no additional cost.
   Supplier has noted the following regarding training: “the manual is very user friendly and in most cases
   should be adequate. If this is not sufficient, our regional sales manager, Nelson Chui, will visit your facility
   to supplement the information in the technical manual”.
5. Emergency Purchases: supplier agrees, to the extent possible, to honor requests for “emergency purchases”.
   Any additional costs incurred for special delivery will be borne by the purchaser. In the event supplier is
   unable to meet such requests, purchaser(s) will be authorized to purchased their immediate requirements
   from non-contract suppliers.
