 Please ensure that you have read the full nomination guidelines to obtaining Queensland
 Government nomination for the Skilled Regional Sponsored visa as detailed at
 www.migration.qld.gov.au.

 This checklist is to assist with collation of documents to be submitted for Queensland State
 Nomination. Submission of complete and accurate documentation will ensure efficient
 processing of your application within the shortest timeframe. Please provide copies only of
 supporting documentation as it is not the policy of this office to return originals.

         are eligible and can meet the Skilled Regional Sponsored Visa criteria – www.immi.gov.au
         Taken the General Skilled Migration Points test - www.immi.gov.au. You must score a
        minimum of 100 points to be eligible (note: the 100 points can include the 10 points for
         State/Territory nomination)
         Have a current positive skills assessment from the relevant Australian Assessing Authority or
        if you are a Student or Working Holiday Maker have submitted an application (see

        Have taken an IELTS test and achieved the required level for visa eligibility.

         Checked that your skill is on the Queensland Skilled Regional Sponsored eligible skills list –
        www.migration.qld.gov.au
         Have made contact with the relevant licensing or registration body for your nominated
        occupation and can meet requirements to allow you to work in Queensland –
         www.immi.gov.au/asri
         Have undertaken research on Queensland and the region where you intend to reside to ensure
        you are aware of costs of living and services available and have sufficient funds to settle –
         see Researching Queensland Regions fact sheet at www.migration.qld.gov.au.

upon receiving nomination from the Department of Tourism, Regional Development and
Industry (the “Queensland Government”) for the purposes of applying for the Skilled
Regional Sponsored (Provisional) visa, declare that:

1. I believe my nominated occupation qualifies me for the Skilled Regional Sponsored
   visa and nomination by the Queensland Government.

2. I understand and acknowledge that the nomination is provided on the basis of the
   details provided in my application for nomination.

3. I understand that the state of Queensland may nominate an applicant to migrate but
   the final decision concerning an application for the grant of a visa rests with the
   Department of Immigration and Citizenship.

4. I understand that the information provided in this nomination application may be
   disclosed to participating government agencies including the Department of
   Immigration and Citizenship.

5. I understand and acknowledge that to be eligible for permanent residence I must
   abide by the conditions of the Skilled Regional Sponsored provisional visa and meet
   the criteria for the grant of the class of permanent visa sought.

6. I understand that Government social security benefits, including Medicare, Higher
   Education Contributions Scheme or Centrelink are NOT available to skilled migrants
   under the Skilled Regional Sponsored Visa. (Information on access to benefits can
   be found on the DIAC website at www.immi.gov.au/allforms/pdf/994i.pdf . It is highly
   recommended that applicants consider obtaining private health insurance).

7. I understand that the Queensland Government has no responsibility for finding
   employment or providing financial or settlement assistance to skilled migrants
   nominated under the Skilled Regional Sponsored Visa.

8. I understand that the Queensland Government or its employees are not liable for any
   inaccuracies or omissions in the information provided to the applicant in the context
   of their application for migration nomination.

9. I understand and acknowledge that the nomination is provided on the following
   conditions:

       (a) The information contained in my application for nomination is complete and
           accurate and I will notify the Queensland Government of any changes to such
           information.

       (b) I have undertaken research of Queensland and the intended settlement region
           and am aware of relevant living costs and services available.

       (c) I have sufficient funds to settle in Queensland and to support myself and my
           family during any period of unemployment.

   (d) I must advise the Queensland Government when my visa is approved.

   (e) I must advise the Queensland Government of my residential address within 28
       days of my arrival into Australia and must continue to notify the Queensland
       Government of subsequent changes to my residential address during the period
       of my visa.

   (f) I must notify the Queensland Government when I commence employment and of
       any subsequent changes of employment.

   (g) I must work and reside in a regional area of Queensland in accordance with the
       visa conditions.

   (h) I must respond to requests for information from the Queensland Government
       during the period of my visa.

   (i) I must notify the Queensland Government of any change to my circumstances
       affecting my ability to meet these conditions.

10. I understand and acknowledge that if I fail to meet the conditions of the nomination,
    the Queensland Government may withdraw the nomination and notify the
    Commonwealth Department of Immigration and Citizenship of such withdrawal.
