Service Type Asset Information via Internet Asset Tagging - Bidder administration of State provided tag (with number)
Description and Service Name Website for asset inquiry including information on: Origin configuration as shipped; Warranty expiration date. This is a fixed fee charged per system ordered. Receive customer provided tag; Affix tag on hardware components; Supply electronic report to State Agency on daily or less frequent basis; Maintain control of all tags issued, lost or destroyed (desirable).This is a fixed fee charged per system ordered. Deployment and Logistics Management Offspec Value Added Services: This is an hourly baseline rate to be used for evaluation purposes only and will be applied to negotiate scope of work that may include the following value added services: Just in time delivery (no PC boxes on-site); Secure Cart Delivery; Move and place equipment within the same building and for use by a new user; Data transfer/migration (amount of data and access) between old and new system; Storage applicable to special delivery and handling; Special handling; Staggered deliveries; Staging; Additional handling and transportation beyond the contract minimum contract requirements; Training at time of system implementation; Only services desired beyond the minimum performance requirements of the contract. Enter the maximum hourly rate that would apply as a baseline to negotiate Deployment and Logistic Services. This hourly rate will be used to negotiate these services and establish a schedule of activities and charges to be applied on a per system basis.

Disposition/Disposal Services
Disposition and Disposal is is a fixed fee charged per component ordered as priced for each common configuration. It includes: 1)De-installation service: Removal and recycle of equipment and packaging materials at end of life cycle, 2) Disposal of IT State equipment shall be in accordance with the State Administrative Manual, Section 5900, Information Technology Disposal of IT Equipment and 3) Disposal according to U.S. EPA standards with electronic certification. The bidder may also commit to End of Life Management as covered in Technical Requirement, Non-Mandatory Scored item 2) where the RFP response provides a plan narrative included with a ‘YES’ response. This requirement will be assessed in addition and in conjunction with Exhibit F – Worksheet Disposition/Disposal Services for a fee, whereby, the bidder will perform the End of Life Management actions for items that are covered and included under the paid VAS fee ass
Emergency Services
Ability to provide on-site user assistance within four (4) hours of a service call for emergency services. Emergencies are defined by users that have a significant impact on their business operations. This is a fixed horuly rate. Create and install custom images; Describe and recommend process for image update and rebuild. This is a fixed fee charged per system ordered. Review of hardware shipment and components; Unpack and setup new system components; Connect to existing network; Perform systems test; Removal of excess packaging.

Radio Frequency Indentification (RFID)
Systems that can carry data and retrieve data via readers from active or passive tags. This is a fixed rate for the applicable items to obtain and perform RFID on a single system ordered. The RFP requests applicable RFID tags, services, reader, scanner, software, and other components be identified and priced within the bid response. RFID component brand, discounts and pricing, where applicable, should be shown in the Exhibit F, Worksheet 7, Offspec Pricing. Testing and certification including reimbursement offered for self-service warranty repairs. This is a fixed fee charged for the training and certification of each state staff person. 1) 24x7 telephone based technical support service, 2) Provide performance reporting. This is a fixed hourly rate.
