For administrative purposes, OSA requires reliable evidence that the following Copyright License Agreement has been personally accepted by the submitting author as agent on behalf of all contributing authors. Therefore, each Innovation presentation must be submitted by an author of the Presentation; submissions from a non-author cannot be accepted. Copyright License Agreement As Agent on Behalf of all Authors Contributing to The Work Named in the On-line Submission Report, the Author Is Submitting the Indicated Presentation ("Work") To The PhAST/Laser Focus World Innovation Awards Program. The Author(s) hereby grant to the Optical Society of America, Inc., and the other sponsors (collectively "Sponsors") of the Conference named in the On-line Submission Report, a non-exclusive, worldwide, royalty-free, transferable license to reproduce, distribute, create derivative works from, publicly perform, and publicly display the Work in all languages, in whole or in part, in any form or medium now known or hereafter known throughout the world including, but not limited to, all formats of print, electronic, magnetic digital, laser or optical-based media, effective as of the date of acceptance of this Work for publication in the above-named technical digest. The Sponsors shall have the right to register copyright to the Work and the accompanying abstract in their name as claimant as part of the technical digest or other medium in which such the Work is included. Furthermore, the Author(s) agree to execute any documents as the Sponsors may reasonably request in confirmation of the foregoing. The Author(s) hereby agree that the Sponsors may edit or otherwise change the Work, its title and abstract. The Author(s) also hereby agree that the Sponsors may use his/her name, photo, or likeness in connection with promoting or advertising the Work, and any adaptation thereof, in the technical digest or other medium in which the Work is included. The Author(s) warrant that, to the best of the Author(s’) knowledge, belief, and expertise: (1) Author(s) are the sole Author(s) and sole holder(s) of all rights, in both written and electronic form, and in any other form of sending and transmitting data, including those not in existence at the time of this Agreement, in and to the Work; (2) the Work does not violate or infringe on any personal or property rights of others, including, but not limited to, patent and trademark rights, whether common law or statutory; (3) the Work contains nothing libelous or contrary to law; (4) the Work does not violate any other party’s right to privacy or publicity; (5) any scientific knowledge contained in the Work is true and accurate; (6) the Work contains no material from other copyrighted or unpublished works that has been used without the written consent of the copyright owner and/or of the owner of any other rights to or in such other works and that Author(s) will obtain any such written consent as required and will deliver the consent to the Sponsors; and
(7) the Author(s) have the full power to enter into this Agreement and to make the grants contained herein. [The Author(s) agree to reimburse the Sponsors for all losses, damages, and/or expenses (including court costs and reasonable attorney’s fees) that the Sponsors may incur due to any breach of the above warranties.] The Author represents that he/she accepts this Agreement as an authorized agent for and on behalf of all the Author(s) and that this Agreement and authorization is made on behalf of all Author(s). US Government Authors: If the Author(s) are or were officers or employees of the U.S. Government at the time the Work was prepared and the Work was prepared as part of their official duties, the Author represents that the Work is not subject to U.S. copyright protection.
