This section ensures that historical records are updated in a timely and orderly manner. Required actions will be
accomplished before the VIDS/ MAF is forwarded to data services for data entry. Maintenance control/logs and records
personnel should screen all VIDS/MAFs, checking the appropriate blocks, and then enter their name/rate in the signature
portion of this block. This signature certifies that no entries are required or all applicable logs/records have had the
appropriate entries made.
This block contains the supply reference to help the material control division requisition failed or required material.
The name and or shift of personnel performing the work.
Toolbox/tool container inventory verification. This column is used to help ensure that tools are not left on the job. Each
worker having a tool container on the job enters the number of the container in this column. Upon return to the work
center, a sight inventory of the tool container is made by the work center supervisor/quality assurance inspector and
initialed or stamped to the right of the tool container number.
The Julian date on which the action takes place is entered in this column.
The number of man-hours that were expended to correct the discrepancy (in hours and tenths of hours) is entered.
The number of clock hours involved in making the repair (in hours and tenths of hours) is entered. For example, if three
people worked together for 2.5 hours to make a repair, the total man-hours would be 7.5, and the elapsed M/T would be
2.5 hours.
This block is used to record the hours accumulated during the SCIR-related time of a discrepancy. This block is best
used by recording the beginning date and time of the AWM period with the proper reason code. At the end of the AWM
period, calculate the accumulated AWM hours and enter in the hours section of this block. NOTE: A complete list of AWM
codes are contained in an appendix to the latest edition of OPNAVINST 4790.2.
Enter letters H - Z. These letters represent a specific record type to be extracted from the MAF by the DSF for failed parts,
AWP, and engine identification reporting. Index letters H - Z shall be assigned to block 79 in alphabetical order. This allows
the 19 most significant failed parts to
be reported against a specific maintenance actions, for example, assignment of index H in block 79 indicates the first failed
part record, Z indicates the last and 19th failed parts record against the maintenance action. The purpose of block 79 is to
flag engineering data items only, not supply usage data. Therefore, only significant failed parts will be annotated with H - Z
in this block, such as, those items which are known or suspected to have contributed to the discrepancy reported in the
discrepancy block of the MAF.
Enter an (x) to denote a failed part if the failed material or parts replaced during the repair are piece parts that have failed
in a major component. Common hardware, nuts, screws, safety wire, seals, gaskets, washers, fittings, etc., that are
routinely replaced during a maintenance actions will be documented only if their failure is known or suspected to have
contributed to the discrepancy. Data blocks 79 through 41 must be documented to indicate failed parts information.

Enter the one-character alpha or numeric code which describes the action taken against the removed module,
subassemblies, or significant failed parts required. AT codes are listed in Appendix E. For engine identification, enter O for
installed, P for uninstalled, or S for removal and reinstallation.
Enter the code that best describes the malfunction occurring within the removed subassembly. MAL description codes are
listed in Appendix I, 4790. For engine identification, enter 000.
Enter the manufacturer's code of failed part or required material. For engine identification, enter the engine TEC followed
by the numeric digit indicating the engine position.
Enter the manufacturer's part number of the failed or required material. For engine identification, enter the engine serial
number and the engine time (prefixed with an E). Use time since overhaul if known, otherwise use time since new (whole
hours only).
Leave blank. (Used at the I-level only).
Enter the quantity of failed or required material. For engine identification, enter 0.
Enter project code (as applicable).
Enter the MILSTRIP priority assigned to the material requisition.
Enter the Julian date the material was requisitioned.
Enter the MILSTRIP (Military Standard Requisitioning and Issue Procedure) requisition number of the material required to
complete the maintenance actions.
Enter the Julian date that requisitioned material is received.
The WUC identifying the system, subsystem, or component on which work is being performed is entered in this block.
The organization code of the organization accomplishing the work is entered.
A two-character numeric transaction code that describes the type of action accom-plished on the document.
A numeric 1, 2, or 3 that describes the level of maintenance being performed (not necessarily the level assigned to the
activity).
The one-character alpha or numeric code that describes the action taken to correct the discrepancy.
The three character alpha/numeric code that best describes the trouble or cause of trouble in the system or component
identified in the WUC block (A22).
The number of times the action indicated in block A35 (action taken) was taken against the item described in the WUC
block A22. VIDS/MAFs submitted for close outs by work centers at the end of or during a reporting period will indicate 0
items processed.
The number of man-hours that were expended to correct the discrepancy (in hours and tenths of hours).

The when discovered code is a single alpha character that identifies when the need for maintenance was discovered.
The one character alpha or numeric code that describes the type of maintenance being performed.
The two-digit PSI of a component that has been identified in the applicable work unit code (WUC) manual by a double
asterisk.

Block 10 of this form identifies the apparent failure code at time of removal. Upon further disassembly and
exploration, the root cause for failure may be different from that observed at time of removal. The
overhaul/repair facility will enter the actual failure code in this block. See table 1-2 or 1-3 at the end of chapter
1 for the failure code.
If the item in block 1 was repaired using a special repair authorization from AMCOM or the item was repaired
by an Enhanced Special Repair Activity enter "Y," all others enter "N."
Provide brief pertinent remarks for the action checked in block 43.
The reverse side of Copy 2 is used to list repair parts replaced during repair, overhaul, or rebuild of the item in
block 1. It will be completed by the maintenance activity including depot level maintenance, organic, or
contractor support that completes the maintenance. When repair parts with serial numbers are reported, parts
will be listed individually as a quantity of 1. All parts used except common, bulk issued hardware will be listed
as follows:
The failure code that best describes why the item was removed and replaced (see table 1-2 or 1-3 ).
The total quantity for each part.
The correct Maintenance Action Code, such as "A" for replaced, "B" for adjusted, "C" for repaired
(nonserialized) (see table 1-9 ).

number and PID will be used at the national level to clarify entries.
Enter the name of the DRMO that the item was shipped to.
Enter the address of the activity identified in block 72.
Enter the Unit Identification Code for the activity in block 72. If a contractor enter the CAGE code prefixed with
a "K.
Enter the Julian date that the item was shipped.
Enter the mutilation statement, verbatim, required by TM 1- 1500-328-23 then sign and date the statement.
