All forms must be filled out by parents prior to leaving children:

1.   Child enrollment form
2.   Parental agreement with child care facility
3.   Emergency medical authorization

Forms to be filled out if necessary:

1.   Guide for authorization to administer medication
2.   Infant feeding plan

Additional forms to be placed in child’s file:

1.   Daily attendance record
2.   Report of incident requiring professional medical attention

PARENTS MUST BRING A COPY OF SHOT RECORDS ON FORM 3231
