Psychotherapy Associates, P.C., 35th Annual Advanced International "Addictive Disorders, Behavioral Health and Mental
Health" Winter Symposium is one of the largest Behavioral Health symposiums in the Midwest. This symposium, located in
Colorado Springs since 1974, attracts a wide variety of attendees from throughout the United States, Canada, and abroad. We
expect over 800 participants to attend the 2009 Winter Symposium. The 2008 program included some 800 participants and 28
exhibitors. Attendees include physicians, psychologists, social workers, private practitioners, family and marriage counselors,
E.A.P. specialists, community agency personnel, counselors, rehabilitation and substance abuse counselors, program
administrators, the military, university faculty, nurses, and LPC’s.

Exhibitors have found that the exposure provided by exhibiting at our Winter Symposium is an excellent method for educating
attendees about their products, programs, and services.

All social breaks are located in the exhibit area so as to provide maximum traffic. Exhibitors are provided with two
complimentary registrations to attend the symposium. As an exhibitor, you will also be provided with a 10' x 8' booth, table, two
chairs, and one wastebasket.

Each year, this internationally recognized symposium brings together a distinguished faculty to address the issues of treatment,
recovery, and research in the Behavioral Health field. Seminars focus on such topics as family therapy, cocaine dependency,
eating disorders, alcoholism, medical issues, differential and dual-diagnosis, domestic violence, compulsive gambling, multi-
cultural issues, women's issues, co-dependency, sexual problems, community corrections, psychopharmacology, sleep disorders,
trauma, ethics, major mental illness, ADD/ADHD, personality disorders, affective illness, opioid/prescription drug abuse,
methamphetamine dependence and many more.
Continuing education credits for physicians, psychologists, nurses, alcohol and drug counselors, and health service providers are
available for symposium participants. Self-Help meetings are also an integral component of the symposium.

As an exhibitor, you will:

1. Receive two complimentary full-week registrations.

2. Receive a 10' x 8' exhibit booth, one draped table, two chairs, and wastebasket.

3. Be able to market your treatment program and services.

4. Interview/hire staff.

   And most importantly,

5. Develop referrals and referral sources.

6. Be listed on Web site

Rental fee for exhibit booths is listed below. A nonrefundable 50% deposit must accompany this contract in order to reserve your
booth space(s). The BALANCE of your fee is due NO LATER THAN December 15, 2008. Exhibitor contracts received
AFTER December 15, 2008, must be accompanied by FULL PAYMENT. Exhibit booth cost is $850.00. Each booth will
consist of 8' back drapery and 3' side panels, one draped table 10' x 8' and two chairs, and one wastebasket. The exhibit color will
be burgundy. A diagram of the exhibit area is printed on a separate sheet. The exhibit hall is carpeted with complimentary colors.

Two complimentary full week symposium registrations are included with each booth rental.

All booths will be assigned on a "first contract in" basis.

Exhibits for the 2009 Winter Symposium will be held in the Crowne Plaza Hotel, Colorado Springs, Colorado. The booth layout shown on the
floor plan of the Crowne Plaza Hotel Ballroom has been depicted as accurately as possible. However, Psychotherapy Associates, P.C., reserves
the right to make such modifications as may be necessary to meet the needs of exhibitors.

Exhibits Open/Registration Wednesday, January 28
                           10:00 a.m. to 2:00 p.m. - Setup
                           2:00 p.m. to 5:30 p.m. - Open

                               Thursday, January 29
                               8:00 a.m. to 7:30 p.m. -Open/ Reception will be held in Exhibit area
                               Friday, January 30
                               8:00 a.m. to 5:00 p.m. - Open

Exhibitors may install as of 10:00 a.m. on Wednesday, January 28, 2009; ALL EXHIBITS must be in place by 2:00 p.m., Wednesday, January
28. ALL EXHIBITS MUST REMAIN INTACT UNTIL 5:00 p.m., Friday, January 30, 2009.

Dismantling may not begin until 5:00 p.m. on Friday, January 30. Exhibit material must be removed from the exhibit area on that day, or they
will be removed at the exhibitor's expense.

Psychotherapy Associates, P.C., and the Crowne Plaza Hotel assume no responsibility whatsoever for any inconvenience, loss, injury, or
damage which may occur to an exhibitor or his representative during, prior to, or subsequent to the Exhibit.

All cancellations must be forwarded in writing to Psychotherapy Associates, P.C. If cancellation is received on or before December 15, 2008, all
monies, less the $350.00 nonrefundable deposit, will be refunded. Exhibitors who cancel after December 15, 2008 will be obligated to make full
payment and will forfeit all monies paid.. Failure to occupy the booth does not release the exhibitor from obligation to pay for the full cost of
booth rental.
