THE FOLLOWING ITEMS WILL BE CONSIDERED BY
    NORTHEAST ALABAMA COMMUNITY COLLEGE IN DETERMINING
           ELIGIBILITY FOR RESIDENT TUITION RATES.

      CHECKED. MINOR DEPENDENT STUDENTS MAY COMPLETE THE ITEMS ON THE BACK OF THIS PAGE
       AS THEY APPLY TO PARENT(S) OR LEGAL GUARDIAN.

    Non-Resident Student / Minor Dependent Student
THE FOLLOWING ITEMS WILL BE CONSIDERED BY
   NORTHEAST ALABAMA COMMUNITY COLLEGE IN DETERMINING
         ELIGIBILITY FOR RESIDENT TUITION RATES.
                        (A student checking any of these items must attach documentation.)
I submit this application for in-state residency for tuition purposes based on one of the following:
   I am a non-resident minor dependent student*, and my supporting person** is a full-time permanent employee of Northeast.

   I am a non-resident minor dependent student*, and my supporting person** can verify full-time permanent employment in
     Alabama, and said employment will begin within ninety (90) days of my registration.

   I am a non-resident minor dependent student*, and my supporting person** is a member of the United States military on full-
     time active duty stationed in Alabama under orders for duties other than attending school.

   I am a non-resident minor dependent student*, and my supporting person** is an accredited member of a consular staff
     assigned to duties in Alabama.

   I (or my spouse) am a full-time permanent employee of Northeast.

   I (or my spouse) can verify full-time permanent employment in Alabama, and said employment will begin within ninety (90)
      days of my registration.

   I (or my spouse) am a member of the United States military on full-time active duty stationed in Alabama under orders for
      duties other than attending school.

   I (or my spouse) am an accredited member of a consular staff assigned to duties in Alabama.

   I reside in the county of Franklin           Hamilton              Lincoln          Marion          in Tennessee; or
    Bartow            Catoosa             Chattooga          Dade           Floyd           Gordon           Haralson
    Polk             Walker             Whitfield       in Georgia.

        A PERSON IN THIS CATEGORY MUST ALSO CERTIFY AND PROVIDE DOCUMENTATION OF AT
        LEAST FIVE OF THE THIRTEEN SUBSTANTIAL CONNECTIONS LISTED ON PAGE 1 FOR THE
        GEORGIA OR TENNESSEE COUNTY OF RESIDENCE. A PERSON IN THIS CATEGORY MUST MEET
        THE THREE (3) CRITERIA ON THE ADMISSIONS APPLICATION FOR THE COUNTY OF RESIDENCE.


    I understand that in order to be eligible for resident tuition rates, the burden of proof lies with me. Appropriate documentation
    is attached or can be provided in support of my request for eligibility for resident tuition rates. I agree to notify Northeast if
    there are any changes in the information submitted with this form. I understand that an out-of-state student cannot attain
    resident student status simply by attending school for 12 months in the State of Alabama.

    *  A minor is defined as an individual who, because of age, lacks the capacity to contract under Alabama law. Under current
       law, this means a single individual under 19 years of age and a married individual under 18 years of age, but excludes an
       individual whose disabilities of nonage have been removed by a court of competent jurisdiction for a reason other than
       establishing a legal residence in Alabama.
    ** A supporting person is defined as either or both of the parents of the student, if the parents are living together, or if the
       parents are divorced or living separately, then either the parent having legal custody or, if different, the parent providing
       the greater amount of financial support. If both parents are deceased or if neither has legal custody, supporting person
       shall mean, in the following order: the legal custodian of the student, the guardian, and the conservator.
