I, the purchaser named above, claim an exemption from payment of sales taxes for the purchase of taxable items on the attached invoice (receive on the day of the event). Purchaser claims this exemption under the rules outlined in the State of Texas Tax Code Sec 151.309 & 310 which exempts Churches, Governments Entities, Educational, and Public Service organizations from the taxes normally imposed under that chapter. I understand that I will be liable for payment of Sales Tax which may become due for failure to comply with the laws of the State of Texas, City of Houston, and the rules set by the State Comptroller regarding exempt purchases.
