PART 2: Provide the following information one time.

Additional offeror information required, but not project specific. This information need only be
provided once in your submission package.

2 A. Applicable to all offerors - Describe your Compliance with the requirements of FAR clause 52.219-8,
Utilization of Small Business Concerns (May 2004). Describe below the methods you have used to comply
with this policy on prior federal contracts. Include any evidence, or a point of contact to verify this
information, if available.

Even if you are a Small or Small Disadvantaged Business, this clause in Federal Contracts applies. It is the
policy of the United States that small business concerns, veteran-owned small business concerns, service-
disabled veteran-owned small business concerns, HUBZone small business concerns, small disadvantaged
business concerns, and women-owned small business concerns shall have the maximum practicable
opportunity to participate in performing contracts let by any Federal agency, including contracts and
subcontracts for subsystems, assemblies, components, and related services for major systems.) It is also
policy to establish procedures to ensure the timely payment of amounts due pursuant to the terms of their
subcontracts.

have worked for. References for subcontractors from the proposing offeror are not acceptable. See
Exhibit “B” format for information to be provided. Written references for subcontractors may be included
and are encouraged. However, subcontractors do not need to send out Exhibit C questionnaires.

2C. Discuss assessment of liquidated damages on any contracts within the last five years. (frequency,
circumstances, severity of problem, etc.)

2D: Offerors are strongly encouraged to include with their proposals information on problems encountered
on relevant projects and the specific corrective actions taken to remedy the problems.

2E. (OPTIONAL) Other Past Performance Considerations. Information, if provided, shall be limited to the
period starting 5 years prior to the solicitation release date.

EXHIBIT B – SUBCONTRACTOR INFORMATION AND CONSENT FORM

Subcontractor and/or Teaming Partner Consent for the Release of Past Performance Information to the
Prime Contractor and Reference Information

Past performance information concerning subcontractors and teaming partners cannot be disclosed to a
private party without the subcontractor’s or teaming partner’s consent. Because a prime contractor is a
private party, the Government will need that consent before disclosing subcontractor/teaming partner past
and present performance information to the prime during exchanges. In an effort to assist the Government
in assessing your past performance relevancy we request that the following consent form be completed by
the major subcontractors/teaming partners identified in your proposal. The completed consent forms
should be submitted to the offering contractor for submission with past performance volume.

Instructions to Offeror for sending Reference Questionnaire Forms: Prepare and send a reference

You are encouraged to send a questionnaire to other clients of contracts listed in your “List of 10 other

                                EXHIBIT D – TECHNICAL PROPOSAL DATA

Offerors are cautioned to read sections 00800 through 01700 of the solicitation documents before
completing Exhibit D as your answers must meet the minimum requirements found in these sections.

Offerors are strongly encouraged to review the evaluation information found in Section 00120 as they
complete their Exhibit D.

Each question should be covered in sufficient detail to clearly address all required information. Answer
questions in order presented. You are NOT limited to the space provided; use as much space as necessary
to provide answers. It is advisable to state the question then give the answer. This will aid in the
evaluation.

In the event you reference another document or part of the proposal you must be clear as to where the
information can be found to answer the particular question(s). Such statements as see Company profile or
see Safety Plan are not acceptable. You must indicate on what page of the proposal the answer can be
found in the referenced document. Failure to do so may make your proposal non-compliant.

The Project Management data is obtained for the purpose of evaluating technical capability in performance
of future task order projects. The offeror is required to provide required information to show experience
and capability to manage multiple disciplined projects.

1. CAPABILITY AND EXPERIENCE:

1A. Describe your experience and capability in managing construction projects for the dollar levels,
disciplines, and locations you are requesting in “Areas of Consideration”. (Why are you a good candidate
for receiving an award?)

1B This contract requires short notice site visits/proposal meetings, normally within 2 calendar days, but
can be as short as 6 hours under emergency conditions. (Consider reporting to165th Civil Engineers
Conference Room or 116th Civil Engineers Conference Room. How will you ensure compliance with these
requirements?

1C. Discuss experience with constructing to United States Green Building Council (USGBC) LEED®
standards. If no experience so state and indicate how you plan to overcome this weakness should a task
order have these requirements.

1 D. Explain in detail how you will perform the minimum percentage required, 15%, with your own
forces. (See E-1 forms for what can/cannot be included.) Note: An offeror who has the capability to
perform both as a General contractor and self perform a major element of construction, for example,
Electrical, or Mechanical or Roofing, may be afforded additional consideration in the evaluation. You
should address your general self performance capability in narrative form as well as demonstrate it in the
calculations on the Exhibit(s) E-1.

2. ORGANIZATION:

2A. Provide as an attachment an Organizational Chart listing company personnel and their roles. (Titles)
(Attachment 1)

2B. Provide names, duties and levels of responsibility of key personnel to include the Project Manager,
Site Superintendent(s), Quality Control Manager and Safety Manager. Indicate to whom the Quality
Control and Safety Managers report. (Lines of authority)

2C. Provide a list of the in-house trades that you employ. Provide the number (not names) of these
employees and if they are full-time, part- time etc.

2D. If applicable, describe the support and interface with your home office or corporate headquarters for
such aspects as financial, management and technical support.

3. SPECIFIC PERSONNEL:

3A Discuss the education, training and experience in managing construction projects of the key personnel
listed in 2B. (A resume may be submitted if it provides this information. Attachment # 2))

3B Provide list of major subcontractors and their trades that you intend to use on these projects. ( These
also should have been listed on Exhibit A and there must be an Exhibit B in the proposal for the named
subcontractors or they will not be considered.)

3C Tell us if you have worked together in the past. If yes, on what kind of jobs?

4. SCHEDULING METHODOLOGY Adequate scheduling processes are necessary to ensure completion
and control of the project from beginning to the end of the project.

4A. Discuss your scheduling capabilities. Include such things as programs and staff to do scheduling.

4B. Include a schedule for the prototypical project(s). Use a Notice to proceed date of 12 Feb 2010,
Offerors not being considered for task orders over $1M may provide schedule in any acceptable format,
offerors being evaluated for orders over $1 Million should provide the schedule in Critical Path
Method (CPM) format to demonstrate capability. (This includes prototypical/seed projects valued at
less than $1M.) If the project is less than $1M an offeror may provide a sample CPM from a
previously completed project to demonstrate capability but must also provide a schedule for the
prototypical project.

5. QUALITY CONTROL- (In addition to the information required in this question, information on
personnel, duties, responsibilities and lines of authority provided in response to questions 2 and 3 will be
used in the evaluation of Quality Control.)

5A. Discuss how quality issues will be dealt with on various types of construction projects. Include in the
discussion as a minimum, meetings, inspections, submittal reviews, correction of non-compliant work and
how you intend to ensure non-reoccurrence and reporting of quality problems to Government officials.
(Note: A full Quality Control Plan will be required if you are awarded a contract and is NOT required with
the proposal. If you do provide a plan, indicate if it is a draft for this contract or a sample from another
project. Also, provide in the answer to this question the page numbers where the required information (list
of minimum information above) can be found. If you fail to follow these instructions, your proposal may
not be adequately evaluated.)

5B. Provide the names of testing laboratories to be used for projects requiring testing and the procedures
for test data reporting.

5C. Discuss your procedures for contract closeout (Punch list, as builts, etc.).

6. SAFETY

6A: Discuss your Safety Program in general, and provide as a minimum, details on training,
documentation, and your plan to ensure adherence to OSHA Standards, and the USACOE EM 385-1-1
(Safety Manual). (Note: A full Safety Plan will be required if you are awarded a contract and is NOT
required with the proposal. If you do provide a plan, indicate if it is a draft for this contract, a sample from
another project or a corporate plan. Also, provide in the answer to this question the page numbers where
the required information (listed above) can be found. If you fail to follow these instructions, your proposal
may not be adequately evaluated.)

6B: Discuss the steps you took to promote safety during construction during the past three (3) years.

Binder # 1 - VOLUME 1, PRICE/COST INFORMATION

( ) Offer and Section 00010 – Complete in its entirety the “Offeror” portion of the Standard Form (SF)
1442. An official having the authority to contractually bind your company must sign the SF 1442 in
accordance with FAR 4.102. Submit an original and the number of copies required by solicitation. An
original signature is required on the original document.

( ) A completed Section 00010, Areas of Consideration,

Prototypical project, Exhibit E

( ) Insert the price/cost associated with the prototypical project on the Prototypical Proposal Offer
Schedule(s)
( ) Complete the Self-performance Exhibit E-1 for each Prototypical Project

( ) Representations and Certifications- Section 00600- Complete the Annual Certification on ORCA.
Insert the required certifications and representations in section 00600 and return with proposal.

Binder # 2 - VOLUME 2, PAST PERFORMANCE INFORMATION- REMINDER- Send out
Questionnaires to your former clients so responses reach Contracting Officer NLT solicitation closing
date!!!

( ) Exhibit A, Part 1: Performance Relevancy Questionnaire format, for five (5) general construction
projects and two (2) design build projects, if applicable. Follow special instructions for IDIQ Contracts.

(   ) Exhibit A, Part 2: Answer all questions.

( ) Exhibit B “Subcontractor Consent and Information Form” for each major subcontractor/ Teaming
Partner listed in Exhibit A Part 2.

than the closing date for the solicitation. Follow-up with clients as necessary.
