Request for Report
A bill introduced in the Wisconsin Legislature in 2009 would have required ambulance drivers in Wisconsin to take
and pass an emergency vehicle operator’s course (EVOC) to be certified to operate an ambulance. It would have
also required ambulance drivers to repeat the written exam once every three years and to repeat the entire course
once every six years. The bill was amended to require the Department of Health Services to establish, by
administrative rule, an ambulance operator safety program that features a safety training course/curriculum and a
certification/recertification program for persons who successfully complete the course, which must include actual
practice driving. Currently, Wisconsin does not require ambulance drivers to have any special training beyond that
required to obtain a driver’s license.

The Wisconsin State Patrol requested this report in order to better understand the current laws and practices in other
jurisdictions regarding the legal requirements to operate an ambulance. The report is focused on training or testing
required to simply operate an ambulance, not to serve as an emergency medical technician or paramedic.


Summary
This report contains information from 47 states, the District of Columbia and six Canadian provinces regarding their
licensing requirements for ambulance drivers. Forty-three of 54 jurisdictions identified do not require EVOC
training to operate an ambulance. A large majority of those states and provinces do not require any special training
or certification to operate an ambulance beyond that needed to obtain a basic driver’s license. Several states require
medical training of varying levels for all ambulance personnel, including drivers. Eleven states require drivers to
complete some type of EVOC training.

We surveyed members of the American Association of Motor Vehicle Administrators, seeking responses to the
following questions:

         1.   Do you have specific training or certification requirements for the licensing of ambulance drivers? If
              yes, what are those requirements? Please provide links or citations to relevant state guidelines or
              statutes.
         2.   If you require certification or training for ambulance drivers, do you require or recommend the use of a
              particular training course or curriculum? If yes, which courses or standards do you use/recommend?
         3.   What is the name, e-mail and phone number of the person at your agency to contact who can provide
              additional information?

We received 31 responses from 24 states, the District of Columbia and six Canadian provinces. Next, we sent the
survey to the State Highway Safety Offices in states that did not respond to the initial survey. We received an
additional five responses. Finally, we contacted the appropriate agencies in the remaining states to identify their
licensing, training or certification requirements regarding ambulance drivers. The raw survey responses can be
found in Appendix A and Appendix B of this report.

We gathered information in three topic areas related to EVOC training requirements:
   •    States and provinces that do not require ambulance drivers to complete EVOC training
   •    States that require drivers to complete EVOC training or a similar course
   •    National emergency vehicle operator standards and courses

Following is a summary of findings by topic area.

States and Provinces That Do Not Require Ambulance Drivers to Complete EVOC Training
    •    Forty-three jurisdictions do not require ambulance drivers to complete any form of EVOC training in order
         to legally drive an ambulance.
    •    Of these 43 jurisdictions, seven (Massachusetts, Washington, Alberta, British Columbia, New Brunswick,
         Ontario and Quebec) require some form of license endorsement (such as a chauffeur’s license or an F—for
         hire—endorsement) or medical training before an individual can operate an ambulance.

States That Require Ambulance Drivers to Complete EVOC Training or a Similar Course
    •   Eleven states (Delaware, Florida, Iowa, Louisiana, Maine, Minnesota, Mississippi, Nevada, New Mexico,
        Oregon and West Virginia) identified in this report require ambulance drivers to complete some form of
        EVOC training to drive an ambulance.
    •   Four states (Florida, Iowa, Minnesota and Mississippi) require an on-the-road driving component be part of
        the course.
    •   Seven states (Delaware, Louisiana, Maine, Nevada, New Mexico, Oregon and West Virginia) do not
        specifically require an on-the-road component in the EVOC.
    •   No state provided information indicating that retraining or recertification was required once an individual
        had taken the course.

National Emergency Vehicle Operator Standards and Courses
    •   This section includes the U.S. Department of Transportation EVOC National Standard Curriculum and
        links to the most common, nationally recognized EVOC providers. Programs include those provided by the
        Volunteer Firemen’s Insurance Services, the National Safety Council and Coaching Systems’ Coaching the
        Emergency Vehicle Operator II and CEVO 3 Ambulance courses.


States and Provinces That Do Not Require Ambulance Drivers to Complete EVOC
Training
Forty-three of 54 jurisdictions surveyed do not require drivers to receive EVOC training to operate an ambulance:
Alaska, Alabama, Arizona, Colorado, Connecticut, District of Columbia, Georgia, Hawaii, Idaho, Illinois, Indiana,
Kansas, Maryland, Massachusetts, Michigan, Missouri, Nebraska, New Hampshire, New Jersey, New York, North
Carolina, North Dakota, Ohio, Oklahoma, Oregon, Pennsylvania, Rhode Island, South Carolina, South Dakota,
Tennessee, Texas, Utah, Vermont, Virginia, Washington, Wisconsin, Wyoming, Alberta, British Columbia, New
Brunswick, Ontario, Prince Edward Island and Quebec.

States That Require Ambulance Drivers to Complete EVOC Training or a Similar Course
Delaware
Delaware state law requires that every ambulance driver obtain an Emergency Vehicle Operator card. To obtain an
EVO card, an individual must take a course offered by the Delaware State Fire School that consists of six hours of
classroom training. No road testing is required, and recertification is also not required. The Delaware State Fire
School uses an EVO curriculum developed by the Volunteer Firemen’s Insurance Services.

Florida
Florida law requires that every ambulance driver possess a valid license, be trained in the safe operation of
emergency vehicles, and complete an emergency vehicle operator’s course or the reasonable equivalent as approved
by the department (Florida Statute Title 29, Chapter 401, Section 401.281(1)(e)).

The law requires drivers to complete a 16-hour course of instruction on driving an authorized emergency vehicle.
Drivers are not required to recertify. The course must include, at a minimum, classroom and behind-the-wheel
training as outlined below:
     • Legal aspects of authorized emergency vehicle operators
     • Selecting routes and reporting emergency operation
     • The practice of defensive driving
     • Accident avoidance
     • Principles of vehicle control
     • Routine safety checks of vehicle
     • Braking and control braking
     • Backing: road position, fender judgment and steering technique
     • Slalom: steering technique and chassis set
     • Steering technique during a skid; a skid pad is optional
     • Turn-around steering technique: fender judgment, road position, controlled braking, controlled
          acceleration, understeer, oversteer and chassis set

Iowa
Iowa requires ambulance services to document their drivers’ training in CPR and emergency driving techniques.
According to Chapter 132, Emergency Medical Services—Service Program Authorization, Section 641-
132.8(147A): Service program levels of care and staffing standards (page 7 of the PDF), training in emergency
driving techniques must include:
     • A review of Iowa laws regarding emergency vehicle operations
     • A review of the service program’s driving policy for first response vehicles, ambulances, rescue vehicles or
         personal vehicles of an emergency medical care provider responding as a member of the service; the policy
         shall include, at a minimum:
     o             Frequency and content of driver’s training requirements
     o             Criteria for response with lights or sirens or both
     o             Speed limits when responding with lights or sirens or both
     o             Procedure of approaching intersections with lights or sirens or both
     o             Notification process in the event of a motor vehicle collision involving a first response vehicle,
         ambulance, rescue vehicle or personal vehicle of an emergency medical care provider responding as a
         member of the service
     • Behind-the-wheel driving of the service’s first response vehicles, ambulances and rescue vehicles

Louisiana
Louisiana law requires that every ambulance driver be a certified first responder and obtain a chauffeur’s class D or
above license. In addition, drivers must take some form of defensive driving course approved by the state. Drivers
do not need to recertify once they have completed the class. The class can be any one of six programs, including:
    • National Safety Council course: CEVO course
    • All-Safe: a course used by Acadian Ambulance Service (which provides the majority of ambulance
         services in the state)
    • Mississippi’s EVO course
    • Training provided by the Volunteer Firemen’s Insurance Services
    • Course approved by the National Fire Protection Association and provided by Louisiana State University
    • Peace Officers’ Standards and Training course provide for Louisiana police officers

Maine
According to Title 32, Chapter 2-B, §85.6, drivers who routinely operate an ambulance in emergency mode or
transport a patient must complete an approved program within six months of being hired. Maine offers an internally
developed Ambulance Vehicle Operators Course that fulfills this requirement. There is no requirement to complete a
refresher course or to recertify. The following programs are recognized as being equivalent to Maine’s AVOC basic
course.
     • DOT EVOC-Ambulance
     • Volunteer Firemen’s Insurance Services Emergency Vehicle Driver Training Program
     • Maine Fire Training & Education EVOC Program
     • Maine Criminal Justice Academy EVOC course
     • CEVO II (if practical skills and Title 29-A reviewed as listed in Title 32, Chapter 2-B, §85.6)
     • Department of Defense EVOC-Ambulance Course

Minnesota
An ambulance driver must possess a current driver’s license issued by any state and must have attended an
emergency vehicle driving course approved by the state. The emergency vehicle driving course must include actual
driving experience.

Mississippi
Mississippi law requires that anyone driving an ambulance must possess a valid emergency medical service driver
state certificate in addition to a valid driver’s license. To obtain an emergency medical service driver state
certificate, an individual must receive training in an approved course. According to Title 15, Part III, Subpart 31,
101.01, the emergency medical service driving curriculum must “conform, at minimum, to the National Standard
Emergency Vehicle Operator Curriculum developed by the United States Department of Transportation and all
current revisions as approved for use by the [Bureau of Emergency Medical Services].”

The training course must:
    • Be a minimum of eight hours
    • Include didactic instruction and practical evaluation

Nevada
Nevada law requires every ambulance driver to be a certified EMT-Basic level or higher (subject to some exceptions
made for localities with fewer than 2,000 people). According to Nevada Administrative Code, Chapter 450B.090, to
obtain an EMT-Basic level certification, an individual must “possesses evidence that he has successfully completed
a national standard course for the operation of an emergency vehicle or an equivalent course approved by the Health
Division.”

New Mexico
New Mexico has two classes of emergency medical responders: medical rescue and ambulance services. Medical
rescue—or first responders—provides immediate medical service to an individual but generally does not transport
the individual to a hospital or other facility. Drivers of medical rescue vehicles are required to complete an approved
EVOC to be licensed in New Mexico. Ambulance services, which usually transports an individual to a hospital, is
regulated by the Public Regulation Commission. Ambulance drivers only need to be certified to drive the
appropriate type (size and weight) of commercial vehicle they are driving. New Mexico has adopted the Federal
Motor Carrier Safety Administration guidelines with regard to commercial driver’s licenses.

Oregon
Oregon requires all ambulance personnel, including non-EMT drivers, to complete an EVOC. This is a one-time
requirement; recertification is not needed. The regulations, 333.250 Section 0042: Ambulance Operational
Requirements, state that persons operating an ambulance must “successfully complete an emergency vehicle
operator’s course of instruction prior to independently operating an ambulance. The course must meet or be
equivalent to the 2003 National Safety Council for Emergency Vehicle Operators Course (CEVO 2 or 3) or National
Fire Protection Agency (NFPA) Driver.”

West Virginia
West Virginia requires ambulance drivers to be certified in CPR and successfully complete an approved EVOC.
Currently, while the state requires drivers to take an EVOC, it does not require any certification that they have done
so. After Sept. 1, 2010, new rules will take effect requiring all ambulance drivers to apply for and obtain
certification from the State Office of Emergency Medical Systems. The basic requirements will remain the same, but
all drivers will need state certification showing that they have completed all of the requirements before getting the
appropriate license. All nationally recognized EVOC training will be approved for certification. Relevant regulations

National Emergency Vehicle Operator Standards and Courses

This guide is an update of the 1978 national standard curriculum for emergency vehicle operators and specifically
addresses emergency vehicle operations as they relate to ambulances. According to page 2 of the PDF, “the course is
recommended for anyone who operates an ambulance. The revised curriculum is appropriate for either rural or urban
operations. The curriculum is made up of an instructor guide and participant manual which allow for the inclusion of
local and State laws and organizational operation procedures.” The manual consists of three modules: The first is
approximately 16 hours of classroom teaching; the second allows participants to practice ambulance operator skills

on a driving range; and the last module provides an on-the-job performance assessment of the skills obtained
through the course. This curriculum is the U.S. DOT-recommended best practices for EVOC training throughout the
country.
