                                         Introducing Dental Optima™
                                          Premera Blue Cross Blue Shield of Alaska has a new group dental product portfolio designed
                                          to bring bright smiles to employers and healthy teeth to employees and their families. With
                                          Dental Optima, employers choose from a range of deductibles, annual coverage maximums
                                         and cost shares – working within their budget and still providing coverage to ensure their em-
                                      ployees’ dental health.
These products are available to new and renewing groups with effective dates beginning July 1, 2005. Renewing groups with
Premera dental coverage will need to select from the new products.

Groups with 2-9 employees
Two plans are available to employers with 2-9 employees – both offering individual deductibles of $50 or $100 and annual maxi-
mum allowances of $1,000 or $1,500.
The difference in the two plans is the coverage level for Diagnostic and Preventive Services. Employers can choose to have these
services covered in full or have members pay 20% of the cost. With either choice, the annual deductible is waived for Diagnostic
and Preventive services.
Basic services are covered at 80% after a six-month waiting period. Major services are covered at 50% after a 12-month waiting
period.
These dental plans are available as a supplemental option when the employer also purchases a Premera Blue Cross Blue Shield of
Alaska medical benefits plan.

Groups with 10 or more employees
Employers with ten or more employees have the same two plans options to choose from, but also have an additional deductible
and maximum allowance choice. Employers can choose to offer a no-deductible plan and can increase the annual maximum al-
lowance to $2,000.
Basic services do not have a waiting period. Major services are covered after a six-month waiting period.
These dental plans are available on a stand-alone basis or as a supplemental option to a Premera Blue Cross Blue Shield of Alaska
medical plan.

Additional Dental Options
Employers with 26 or more employees have two optional benefits they can add to enhance their dental benefits coverage – a
Benefit Enhancement Rider and Orthodontia.
 The Benefit Enhancement Rider can be purchased to cover Endodontic (Root Canal) and Surgical & Nonsurgical
  Periodontal Treatment under Basic Services rather than Major Services.
 Orthodontia coverage has been enhanced to allow employers more choice in lifetime maximums and member coverage.
Employers can choose from a lifetime maximum of $1,000 or $1,500 and can choose to allow coverage for all members or limit
coverage to members 19 years and younger.
As before, diagnostic care coverage is covered in full up to a $250 maximum and monthly orthodontic adjustments are covered in
full up to the lifetime maximum.
Employers with 100 or more employees can still purchase TMJ Dental Services, with an annual benefits maximum of $1,000 and
a lifetime maximum of $5,000.

Premera Blue Cross Blue Shield of Alaska Dental Network

Building on our commitment to provide members with access to affordable coverage, we are launching a
campaign next month to build a dental provider network in Alaska. By this time next year, our goal is to
have a comprehensive dental network in place to bring even brighter smiles to employers and members!

                     Dental Care – On the Road!

                                     Effective immediately, Premera dental members have access to a network of dental service providers when
                                           traveling or residing outside of Alaska. This network offers members access to more than 52,000
                                              offices nationwide, putting members close to quality dental services almost anywhere in the U.S.
                                             Plan members may still see any dentist they choose but the fees charged by in-network dentists
                                             are often lower, so out-of-pocket expenses are reduced and members are able to obtain more ser-
                                            vices under their annual maximum.
To find an in-network dentist nationwide, simply visit www.premera.com. The provider directory will give you a list of the closest
dentist offices. To pay for in-network services, members simply show their ID card. The dentist office will take care of filing the
claim while members may be responsible for any applicable cost shares at the time of service.
