           ANNEX 1 – BASIS OF PAYMENT FOR WORK ITEMS

 BASIS OF PAYMENT FOR WORK ITEMS INCLUDED IN THE
                    PROPOSAL

Project Name      :   Proposed Construction of Traveller’s Hub
                      Port of Zamboanga, Zamboanga City

The work items included in the proposal shall be paid on a basis of per unit of actual quantity
delivered / accomplished duly accepted by the project engineer as stipulated in the Bill of
Quantities multiplied by the Unit Cost agreed upon.

The unit cost per quantity of the work items shall include the use of equipment, labor, materials,
contractors profit and all taxes.

     ANNEX 2 – FACILITIES TO BE PROVIDED FOR THE ENGINEER
                          AND HIS STAFF


            FACILITIES TO BE PROVIDED FOR THE ENGINEER & HIS STAFF


A.     SITE OFFICE

       The Contractor shall provide a one storey building for use of the Engineer and his staff
       with a floor area of at least 48 sq.m. It shall provide with electric power, potable water
       supply and drainage/sewerage system.

       The field office shall be constructed with G.I. roofing (Ga. 26), plywood ceiling, concrete
       hollow block wall, plain concrete flooring, glass jalousie windows on an aluminum
       frame, flush type doors, a kitchen sink with glazed tiles, toilet mechanism and water
       supply and movable partitions with one (1) bedroom.

       The field office shall be located within the area to be determined by the Engineer. The
       contractor shall submit to the Engineer for approval the working drawings of the field
       office prior to its construction. Upon instruction of the Engineer, the field office maybe
       demolished by the Contractor after the completion of the project.


B.     OFFICE EQUIPMENT FOR USE OF THE PPA ENGINEER

       The Contractor shall purchase the following main items of brand new office equipment
       for use of the Engineer and his staff. The Contractor shall make available for use of the
       Engineer other equipment as may be necessary for the proper functioning of the office.
       The equipment shall be the property of PPA. Operation shall be done by PPA but
       maintenance shall be borne by the Contractor until the duration of the project.

              1. 2 units Air conditioning unit, 1.5 HP
              2. 1 unit   (1544 x 612) mm Main Desk and Chair
              3. 2 units Gas Lift Chair without arms
              4. 1 unit   3-Drawer Mobile Pedestal
              5. 2 units Single Bunk Beds with mattress and beddings
              6. 1 unit   Waste paper basket
              7. 1 unit   Filing cabinet, four (4) drawers with lock
              8. 1 set    Gas stove (2 burner with tank)
              9. 1 unit   Electric airpot (2.5 liter capacity)
              10. 2 units Scientific calculator – FX 4600 matrix
              11. 1 unit  Dining table 6 seater with chairs
              12. 1 unit  4 cu. Ft. Refrigerator
              13. 1 unit  Water dispenser hot and cold

D.     PROVISION OF VARIOUS PROTECTIVE EQUIPMENT

       The Contractor shall provide all his/her personnel the following personal protective
       equipment for use in the implementation of the project:

              1.   Hard Hats
              2.   Safety Boots/Shoes
              3.   Goggles
              4.   Safety Gloves
              5.   Masks
              6.   Safety Belt

       These equipment must be used by his/her personnel at all times.
