EXECUTIVE SUMMARY

An asbestos survey was conducted on October 9 & 13, 2008 of the Williams building
located on the South Carolina Department of Mental Health (SCDMH) Bull Street
Campus in Columbia, South Carolina. The purpose of the assessment was to verify the
presence of asbestos-containing materials (ACMs) in the building as determined in a
previous asbestos survey report by Davis & Floyd (1990), and to perform supplemental
bulk sampling and analysis to conform with the South Carolina Department of Health and
Environmental Control (SCDHEC) Regulation 61-86.1 Standards of Performance for
Asbestos Projects effective June 27, 2008.

The building is a two-story masonry block structure on a pillar foundation with a finished
basement and crawlspace. The building was reportedly constructed in 1938 and contains
approximately 79,366 square feet of interior space. This structure was formerly used as a
patient ward and hospital with various offices and storage rooms and a kitchen. The
Williams building is currently vacant.

Bulk samples were collected from suspect ACMs that were observed and the samples
were subsequently analyzed for asbestos content by an accredited laboratory. These
materials included: various acoustic ceiling tiles with mastic, sheetrock, joint compound,
various floor tiles with mastic, vinyl baseboards and mastic, vinyl stair treads and mastic,
window glazing, thermal systems insulation (TSI) and mastic, fire door insulation, mastic
on HVAC ducts, roofing materials, roof flashing and roof sealant. The Environmental
Protection Agency (EPA) and South Carolina Department of Health & Environmental
Control (SCDHEC) define materials as asbestos-containing if an asbestos content of
greater than one percent (>1%) is detected in a representative sample. Asbestos, in
concentrations >1%, was identified as a result of the assessment. The following table
summarizes the ACMs identified:

The identified asbestos-containing roofing materials/sealants, floor tiles and floor tile
mastic are classified as Category I non-friable materials, in good condition. The
identified window glazing, ceiling tile mastics, arc shields, elevator brake pads, TSI
mastics and duct mastic are classified as Category II non-friable materials, in good
condition. The identified, thermal system insulation (debris, wraps, and hard joints), fire
door insulation and joint compound are classified as friable materials in good condition.
All of the identified ACM are considered to have a significant potential for disturbance

should the structure be subject to demolition or renovation. No asbestos in
concentrations >1% was detected in the remaining samples of suspect materials collected
from the building.

The EPA and SCDHEC require proper removal and disposal of ACMs that will be
destructively affected by renovation and/or demolition activities, therefore we
recommend the identified ACMs be properly removed and disposed by a qualified
abatement contractor licensed by SCDHEC prior to any renovation or demolition
activities. A copy of this report should also be provided to the contractor(s) to assist with
compliance with the applicable State and Federal regulations. If additional suspect
materials are discovered during future renovation or demolition activities, bulk samples
must be collected and analyzed for asbestos content.

A copy of this assessment report must be submitted to SCDHEC, along with a demolition
license application, 10 weekdays prior to demolition activities.

1.      BACKGROUND

S&ME was contracted to perform an asbestos survey of the Williams building located on
the SCDMH Bull Street Campus in Columbia, South Carolina. The purpose of the
assessment was to identify asbestos-containing materials (ACMs) in the building.

The Williams building is a two-story masonry block structure on a pillar foundation with
a finished basement and crawlspace. The building was reportedly constructed in 1938
and contains approximately 79,366 square feet of interior space. The building has a flat,
built-up bituminous roof. Interior finishes include concrete floors that are mostly
covered with terracotta tiles or ceramic tiles. Other areas are finished with nine-inch or
2-inch vinyl floor tiles. Walls are generally plaster, with some renovated areas having
painted sheetrock walls. Ceilings are finished with a mixture of adhered 12-inch acoustic
tiles, 12-inch acoustic tiles adhered to a dropped ceiling in the hallways and suspended 2’
x 4’ or 2’ x 2’ acoustic tiles. Thermal system insulation (TSI) is present throughout the
building on the pipes associated with the heating and cooling system. This structure was
formerly used as a hospital and a patient ward and is currently vacant.

S&ME reviewed an asbestos survey report of this building prepared by Davis & Floyd,
Inc. dated January 1990. A copy of this report is presented in Appendix V. The January
1990 report discusses the June 1988 and December 1989 asbestos surveys of the subject
building that included the collection of approximately 50 bulk samples from various
materials throughout the building. Of the samples of suspect ACM collected by Davis &
Floyd in 1988 and 1989, asbestos was determined to be present in the following
materials:

        •   White and green floor tile in Rooms 101 and 102 (currently Rooms 042 and
            043)
        •   Black bituminous duct insulation sealant and pipe sealant throughout the
            building
        •   12-inch ceiling tiles on the first and second floors
        •   General debris in the crawlspace on the western end of the building and below
            the front entrance
        •   Pipe insulation and hard joint insulation in the boiler room and Room 100
        •   Insulation on the surge tank and condensate tank in the boiler room
        •   Roof flashing on the roof

The purpose of the assessment was to verify the presence of asbestos-containing
materials (ACMs) in the building as reported by Davis & Floyd (1990), and to perform
supplemental bulk sampling and analysis to conform with the South Carolina Department

of Health and Environmental Control (SCDHEC) Regulation 61-86.1 Standards of
Performance for Asbestos Projects effective June 27, 2008. The identification of ACMs
will aid in the prevention of occupational exposures and/or environmental releases of
airborne asbestos. Identification of ACMs also complies with Title 40 Code of the
Federal Regulations, part 61, and State regulation 61-86.1 enforced by the SCDHEC,
along with Title 29 Code of Federal Regulations, part 1926 enforced by OSHA. The
following sections describe the investigative procedures used, results of the suspect
ACMs sampled and analyzed, and conclusions and recommendations regarding the
structure as related to ACMs.

2.      ASSESSMENT PROCEDURES

The assessment was performed by observing and collecting statistically random samples
of suspect asbestos-containing materials associated with the interior and exterior of the
structure. The possibility exists that suspect materials were not detected in inaccessible
areas such as flooring overlays, pipe chases or wall voids. If additional suspect materials
are discovered during future renovation or demolition activities, bulk samples should be
collected and analyzed for asbestos content.

A sampling strategy was developed to provide representative samples in accordance with
OSHA, SCDHEC and EPA. Bulk samples were then extracted from suspect ACMs and
recorded on a chain of custody record and submitted to our in house Polarized Light
Microscopy (PLM) laboratory. Non-friable organically bound materials requiring
confirmation by Transmission Electron Microscopy (TEM) were submitted to EMSL
Analytical for analysis. The laboratories are accredited by the National Voluntary
Laboratory Accreditation Program (NVLAP), which is administered by the National
Institute of Standards and Technology.

Polarized Light Microscopy (PLM)
The suspect materials were analyzed by trained microscopists using PLM techniques
coupled with dispersion staining in accordance with EPA Test Method Title 40 Code of
Federal Regulations, Chapter I (1-1-87 edition), Part 763, Subpart F-APPENDIX A. This
method identifies asbestos mineral fibers based on six optical characteristics:
morphology, birefringence, refractive index, extinction angle, sign of elongation and
dispersion staining colors. The laboratory analysis reports the specific type of asbestos
identified (there are six asbestos minerals) and the percentage of asbestos present.

Transmission Electron Microscopy (TEM)
Suspect non-friable organically bound materials, exhibiting negative results by PLM
analysis, were analyzed by trained microscopists by TEM using EPA 600 Method in
accordance with ASTM E2356.

3.         FINDINGS AND RESULTS

The asbestos assessment conducted on October 9 & 13, 2008, included the quantification
and statically random bulk sampling of various suspect asbestos-containing materials
located on the interior and exterior of the structure. These materials include: various
acoustic ceiling tiles, plaster and skim coat, thermal system insulation (TSI), hard joint
material, window glazing compounds, sheetrock, joint compound, various vinyl floor
tiles and mastics, electrical arc shields, bituminous sealants on ductwork and TSI, various
vinyl baseboards and mastics, fire door insulation and roofing materials. Of the
representative materials sampled and analyzed, asbestos in concentrations >1% was
identified in the following materials summarized below:

The EPA classifies ACMs into two categories: friable and non-friable. A friable material
creates a greater health hazard due to the fact that it may be "crumbled, pulverized or
reduced to powder by the forces expected to act upon it in the course of demolition or
renovation operations." The identified asbestos-containing roofing materials/sealants,
floor tiles and floor tile mastic are classified as Category I non-friable materials, in good
condition. The identified window glazing, ceiling tile mastics, arc shields, elevator brake
pads, TSI mastics and duct mastic are classified as Category II non-friable materials, in
good condition. The identified, thermal system insulation (debris, wraps, and hard
joints), fire door insulation and joint compound are classified as friable materials in good
condition. All of the identified asbestos-containing materials are considered to have a
significant potential for disturbance during future renovation/demolition activities. No
asbestos in concentrations >1% was detected in the remaining suspect materials sampled
and analyzed from the structure.

TEM analysis was performed on eight samples of non-friable, organically-bound
materials that tested negative for asbestos via PLM analysis. These materials are:

    •      Brown mastic associated with 12-inch textured ceiling tiles
    •      Brown vinyl baseboard and mastic
    •      Dark brown vinyl baseboard and mastic
    •      Black six-inch vinyl baseboard and mastic
    •      Dark brown vinyl stairtread and mastic
    •      Green 12-inch vinyl floor tile and mastic from the elevators
    •      Beige vinyl baseboard and mastic

    •   Roofing paper

The TEM analysis confirmed that no asbestos is present in the samples that were
analyzed. No additional TEM analysis was warranted in this assessment for the other
negative PLM results obtained for organically bound materials because they were
reported in materials associated with materials that tested positive for the presence of
asbestos. These materials (e.g. mastics associated with positive floor tiles or multiple
layers of flooring in the elevator lobby) should be considered to be asbestos-containing
due to their association with confirmed ACM.

Rooms 042 and 043 (formerly rooms 101 and 102) were inspected for the presence of the
asbestos-containing floor tiles identified by Davis & Floyd in 1990. Nine-inch white
floor tiles with green streaks are present in both the rooms. Three bulk samples of the
floor tiles and mastic were collected as part of this assessment. The floor tiles and mastic
were reported to contain 7% and 6% chrysotile, respectively.

The crawlspaces and areas above the suspended or dropped ceilings throughout the
building were inspected for the presence of the asbestos-containing bituminous pipe
insulation sealant and duct insulation sealant identified by Davis & Floyd in 1990. The
bituminous sealant was observed on ductwork and pipe insulation above the suspended
ceiling in the basement. The bituminous sealant on ductwork was also noted above the
suspended ceilings near the nurses’ stations. The duct sealant was sampled and reported
to contain 6% chrysotile.

The building was inspected for the presence of the asbestos-containing 12-inch ceiling
tiles reported by Davis & Floyd in 1990 to be present on the first and second floors. At
least three varieties of 12-inch ceiling tiles were identified and sampled in the building
and none of the tiles were reported to contain asbestos. However, the brown mastic
associated with the smooth and stippled 12-inch ceiling tiles that are present in the
dayrooms, meeting rooms and front lobby was reported to contain 6% chrysotile.
Additionally, brown mastic is present on the original ceilings in the hallways above the
current dropped ceiling. This mastic was also reported to contain 6% chrysotile.

The boiler room was inspected for the presence of the asbestos-containing equipment
insulation (on a surge tank and condensate tank) as well as asbestos-containing pipe
insulation and hard joint material. No suspect equipment insulation or TSI was observed
in the boiler room. The boiler room appears to have been abated and the insulation that
was observed appeared to be non-suspect fiberglass.

The roof was inspected for the presence of the asbestos-containing roof flashing
identified by Davis & Floyd in 1990. Four bulk samples of the roofing paper were
collected and none of the samples were reported to contain asbestos. Four samples of

roofing sealant were collected from the edges of the roof of the Williams building. One
of the samples was reported to contain 5% chrysotile.

The crawlspaces of the building were inspected for the presence of the asbestos-
containing TSI debris reported by Davis & Floyd in 1990. No evidence of pipe TSI, hard
joint material or debris was observed in the western crawlspace. Apparent Aircell pipe
insulation debris was observed in the crawlspace under the front steps.

The following summary table exhibits the sample number, location, type of material
tested, approximate quantity of the material sampled, condition of the material, and
corresponding result for each sample. A diagram exhibiting the bulk sample locations
and locations of the confirmed asbestos containing materials is provided in Appendix I.
Photographs of building conditions and select ACM are included in Appendix II. A copy
of the inspector’s SCDHEC license is provided in Appendix III, and the laboratory
analyses and chain-of-custody records are provided in Appendix IV.

4.      ABBREVIATIONS AND HAZARD ASSESSMENT KEY

In accordance with the EPA and SCDHEC, a confirmed ACM is assigned a hazard
assessment based on its present condition and potential for disturbance. The hazard
assessment is used as a tool for prioritization in remedial actions regarding the identified
ACM(s). The following key exhibits the criteria that compose the hazard assessment.

5.       CONCLUSIONS AND RECOMMENDATIONS

The asbestos survey conducted on the Williams building located on the SCDMH Bull
Street campus in Columbia, South Carolina, identified the presence of asbestos-
containing building materials as follows:

     •   Joint compound (2% chrysotile) associated with non-asbestos containing
         sheetrock walls in the basement. The samples were collected from the office area,
         records room and post office located in the eastern wing and from the salon
         located in the north wing – approximately 1,500 square feet.
     •   Dark brown mastic (6% chrysotile) associated with the non-asbestos containing
         12-inch smooth and stippled ceiling tiles located in the lobby, dayrooms and
         meeting rooms on the first and second floor – approximately 10,000 square feet of
         ceiling tile.
     •   Dark brown mastic (6% chrysotile) from former ceiling tiles located on the
         original ceiling above the dropped ceiling in the hallways on the first and second
         floor – approximately 6,000 square feet.
     •   Hard joint material (2% amosite) and black mastic/wrap (6% chrysotile) on
         elbows of water pipes with fiberglass insulation located above the suspended
         ceiling in the basement – approximately 300 joints.
     •   Window glazing compound (2% chrysotile) located between the metal frame and
         the glass on the exterior of the windows throughout the building – approximately
         278 windows.
     •   Nine-inch cream-colored vinyl floor tiles and nine-inch green vinyl floor tiles
         (2% chrysotile) located in offices, stairwells and under carpets throughout the
         building – approximately 9,000 square feet.
     •   Brown vinyl floor tiles (2% chrysotile) and black mastic (6% chrysotile) located
         under non-asbestos containing off-white 12-inch vinyl floor tiles in the first floor
         elevator lobby and adjoining stairwell landings – approximately 196 square feet.
     •   Fire door insulation (25% chrysotile, 2% amosite). Fire doors are located
         throughout the building on the stairwells and elevator lobbies – approximately 20
         doors.
     •   Black bituminous roofing sealant (5% chrysotile) on the joints and edges of the
         building’s roof – approximately 1,240 square feet.
     •   Black bituminous sealant (8% chrysotile) on the joints and edges of the covered
         walkway roof on the north side of the building – approximately 1,200 square feet.
     •   Brown Bakelite arc shields (5% chrysotile) located on the elevator control panels
         in the elevator equipment room on the roof – approximately 2 control panels.

    •   Two white brake pads were observed on each of the two elevator motors in the
        elevator equipment room on the roof of the building. The pads were not sampled
        because the elevators were active at the time of the assessment. The pads are
        presumed to be asbestos-containing – 4 brake pads.
    •   Black insulation sealant/mastic (7% chrysotile) located on the non-asbestos
        containing sulfur rock insulation covering the steam pipe above the ceiling in the
        basement – approximately 200 linear feet.
    •   Nine-inch white vinyl floor tiles with green streaks (7% chrysotile) and associated
        black mastic (6% chrysotile) located in rooms 101 and 102 in the basement –
        approximately 808 square feet.
    •   Black mastic/sealant (6% chrysotile) on HVAC duct insulation above the
        suspended ceilings throughout the building – approximately 2,500 linear feet.
    •   Previously identified black mastic/sealant on fiberglass pipe insulation located
        above the suspended ceilings throughout the building – approximately 2,000
        linear feet.
    •   Apparent Aircell pipe TSI debris was observed in the crawlspace under the front
        porch. The surficial soil located in this crawlspace is considered to be
        contaminated with ACM – approximately 450 square feet.

The EPA and SCDHEC require proper removal and disposal of ACMs that will be
destructively affected by renovation and/or demolition activities, therefore we
recommend the identified ACMs be properly removed, transported, and disposed of by a
qualified abatement contractor licensed by SCDHEC, Asbestos Section, prior to any
renovation/demolition activities. If additional suspect materials are discovered during
future renovation/demolition activities, bulk samples must be collected and analyzed for
asbestos content. Access to areas with significantly damaged ACM should be limited to
asbestos trained personnel with proper personal protective equipment (PPE).

A copy of this assessment report must be submitted to SCDHEC, along with a demolition
license application 10 weekdays prior to demolition activities.
