       1977        11 M           Explosion occurred onboard a hovercraft that was used as a "shooter" in the operation. A
                                  charge of 50 m of cord was detonated at the normal timing. At the time of the detonation
                                  the charge was on the deck of the hovercraft instead of in the water at a safe distance
                                  astern from the craft. Two firing lines were in use and this lead to firing the wrong charge.

       1977         1S            A surveyor driving his vehicle at high speed out of camp hit a couple of small gullies and
                                  rolled his vehicle end over end. He was the only person in the vehicle.

               sand. Head slammed into hard sand, died in hospital with head trauma.

1987   2S   Vehicle in which deceased were travelling was involved in an accident with a 12 ton
            company truck.


1987   2S   Four people were digging a water well as a community support project. A petrol water
            pump was used to keep the bottom of the well dry. The person in the well lost conscience
            due to lack of oxygen/CO from petrol engine. A second person went down the well and
            also lost conscience. When the two victims were recovered to surface, they had

            system, the wall of the ditch collapsed and trapped the victim
1987   1S   Trapped between the load and hand rail during a lifting operation.

1987   2S   The victim and his co-worker were on their way to site by motor bike. On arrival at work
            the bike collided with a 4WD vehicle.




1987   1S   The deceased was a local sub contractor who was killed by a falling tree during site
            clearance for a geological survey.
1987   1S   Pressure testing of fireman pipe-work extension resulted in hose blow-off, hitting the
            deceased in the face. Fell backwards hitting head on steelwork. Died two days later.
1987   1S   The accident occurred as a result of an inexperienced driver losing control of his vehicle
            at high speed. One of the passengers died because he was sitting unprotected in the
            back of the pickup when the accident took place.



1988   1S   Follwoing a breakdown of the vehicle in which they were travelling, the four passengers
            disembarked to push the vehicle off the road. The victim was struck by a truck travelling
            at high speed
1988   1S   The victim was pulled overboard when his foot caught in the cable / chain being laid from
            zodiac on fast flowing river during seismic operations
1988   1S   The victim died whilst attempting to recover a third party canoe which had sunk in the
            wake of the contractors’ crew boat. Following several unsuccessful attempts, the victim

1988   4M   A film crew filming vibrator operations from a van was parked on the hard shoulder of a
            black top road. A large truck hit the van for unknown reason killing all four people from
            the film crew.
1988   1S   Collision at night between company and 3rd party boat. Third party passenger drowned.
            Company driver and others injured.

1988   1S   The victim died from drowning whilst swimming from his houseboat.
1988   1S   The victim sustained extensive burns in an explosion in a hyperbaric lifeboat. The
            explosion resulted from an accumulation of combustible gases from a battery charger
            which ignited when the victim operated an electrical switch.
1988   1S   While dismantling a pre-fabricated wood warehouse, contractor fell approximately 12 feet
            landing on the frozen ground below. The contractor landed on the top left side of his
            head.




1989   1S   Land slid on the shooter after shot. Body buried below tons of sand.

1989   1S   A labourer lying on the ground in an area of seismic operations was run over by a vehicle
            carrying seismic equipment. The accident took place at dusk, and the deceased died
            during the med-Medevac
1989   1S   Labourer employed for tent duty slipped into river and drowned, after gaining
            unauthorized access through a fence, instead of using the designated camp jetty to fetch
            water, 65 m away.
1989   1S   Swimming across a river to catch a canoe on the other side of the river

1989   1S   Deceased, engaged in bush clearing activities, was stranded on far side of river when the
            canoe used for ferrying workers stopped for the day. The deceased decided to swim
            rather than wait around and drowned as a result.

1989   1S   The victim, a member of a three man seismic line cutting crew was crossing a river to
            work on the seismic line. The crew was using a rope spanned across the river to cross.
            The first man successfully crossed the river by holding onto the rope. The victim was the
            second man to cross. Halfway across he could not hold onto the rope, as he could not
            swim. Drowned and was found dead by work colleagues.
1989   1S   A dispute arose between a canoe driver and a line labourer who was late reaching the
            canoe at the waterside. A fight broke out in which the canoe driver was struck with a
            machete. The canoe driver dies from his injury.
1989   1S   Geophone man taken ill while waiting to be paid off. Hospitalised for 8 days but died in
            hospital due to Gastro-Enteritis.
1989   1S   Victim was not known to be in area where a 200’ tree was felled. Proper safety and work
            procedures were followed, but due to massive branches on one side the tree fell in the
            wrong direction. Three operators blew warning whistles which the victim should have
            heard.


1989   2S   Truck transporting seismic equipment rolled over into ditch and caught fire. Both drivers
            killed
1989   1S   Driver lost control of Hilux and 100 litre fuel trailer. Both vehicle and trailer rolled over
            injuring 3 men and killing one. All four occupants were thrown out of the vehicle.
1989   1S   A pick up truck carrying three persons in the front and four in the open back turned
            sharply and overturned twice
1989   1S   Deceased fell through open manhole into tank and suffocated


1989   1S   Deceased was stuck by a truck he was running after, presumed horseplay

1989   1S   Victim went to reserve water pit to wash his hands and feet for noontime prayer.
            Accidentally fell in and drowned.
1989   1S   Contractor was installing a new radio antenna . A helper was instructed to cut down some
            cables of the old antenna. On doing this the old pole antenna (2 7/8” tubing) started to
            bend down and the helper, who was not wearing a safety belt dropped to the floor of the
            workshop


1989   1S   Security guard fell from second storey car park whilst on guard duty. Victim was thought
            to have sat on the parapet wall, fallen asleep and overbalanced backwards to the
            concrete floor below.

1989   1S   Victim was working on partially dismantled scaffolding, On disconnecting a joint of
            scaffolding, board on which he was standing gave way and victim fell 12 meters.



1989   1S   Employee mowing grass, tractor turned over and crushed victim
1990   1S     Bulldozer driver clearing a seismic line in Colombia was killed by Africanised bees

1990   1S     Line worker was bitten by snake at night while walking to near by town. He was treated
              but died the next day.
1990   1S     Assistant cook of team preparing a base camp drowned while washing his clothing from
              a 3rd party boat. Victim could not swim.
1990   1S     Incident happened in seismic fly camp, victim slipped into deep water when trying to
              recover a T shirt he was washing and which had drifted away
1990   2S     A surveyor and his helper drowned when crossing a river, no witnesses. The river
              crossing ramp normally was about 50 cm deep.
1990   1S     Although craft and life jackets were in place to assist river crossing, one person decided
              to swim across without lifejacket and drowned.
1990   1S     On a shooting vessel the blasting box was tested before taking a shot. Inadvertently, the
              next charge on the ramp had already been connected to the blaster and exploded. The
              victim holding the charge was killed.
1990   1S     Line labourer became ill but did not seek medical help for 1 week. Crew decided to
              evacuate the man but he died before the plane arrived.

1990   1S     While a tree was being cut during helipad preparation, a second tree unexpectedly fell,
              killing a surveyor present near by
1990   1S     On a track in poor condition a driller and his driver tried to take a dangerous passage.
              The truck slipped and fell into a 30 m ravine next to the road. The driver, who remained
              in his seat and had his seatbelt attached was only lightly injured. The driller tried to jump
              from the truck and broke his neck.
1990   1S     Light van driving at high speed, hit minibus-taxi stopped at the roadside. 1 passenger of
              the van crusehd.
1990   1S     Truck collided with minibus during seismic camp move. A third vehicle was hit and the fire
              from a split petrol tank killed one truck passenger and injured 16 others
1990   1S     On a slippery sloping road surface, a truck skidded into the bank of the road. A
              passenger sleeping in the back of the truck was ejected and crushed between the bank
              of the road and the truck
1990   1S     While driving a quad ATV, the driver was hit by barbed wire across the track which he
              failed to see. Died of severe neck injuries. Exact year ????

1990   11 M   Fixed wing charter aircraft flew into mountain during bad weather.
1990   1S   Bulldozer helper run over by reversing bulldozer
1990   1S   Security guard died when he lost control of a motor scooter
1990   1S   Truck stuck off-road and winching itself free using a tree at the opposite site of the road.
            The cable was attached rather high (above bonnet level). Unfortunately a vehicle passed
            on the usually empty road, did not see the cable, and hit it at full speed (60-80kph). The
            single occupant died on site.
1990   1S   Tyre fitter's helper in temporary repair workshop was inflating a repaired tyre. The tyre
            retaining ring dislodged and stuck the helper on his head.
1991   1S   When a beehive was disturbed the bees attacked a swamp buggy in Nigeria. Four men
            jumped overboard to escape the bees. Three of the men were picked up by a speedboat
            later but the fourth man drowned.
1991   1S   Victim decided to walk to a survey beacon, 3 km away, across a piece of sand dunes that
            was inaccessible to his vehicle. He left alone and without a radio. He was found dead
            next day and autopsy showed he had been bitten by a snake in the left lower leg.

1991   1S   A patrol vehicle was attacked by armed robbers. THe deceased was shot in the head and
            died instantly
1991   1S   Labourer was hit by a passenger vehicle while crossing a road that ran through the camp
            area.



1991   1S   Caught by a sudden wave, washed him over the side. Drowned despite wearing
            protective suit and life vest.
1991   1S   The deceased drowned whilst attempting to recover a piece of seismic equipment from a
            deep mangrove pool. Efforts to locate the body were unsuccessful and it Is assumed that
1991   1S   his body was swept under the to stop operations, fast flowing current at the bottom of the
            Contrary to instructions given mangroves by the a crew of two foremen and 10 labour
            decided to continue and rent a local canoe to cross a river swollen by recent heavy
            rainfall. 5 m from the shore, the canoe capsized and one labour drowned. Crew had no
            lifejackets.
1991   1S   Line labourer drowned while trying to cross a river

1991   1S   The victim together with 2 others were erecting a radio antenna for the seismic base
            camp. The pole touched a 6KV overhead power line electrocuting the victim fatally and
            injuring one of his mates.
1991   1S   While tying to clear the area to get a good GPS signals a line labourer climbed on a tree
            to remove a branch that was obstructing line of sight for the GPS receiver. While doing
            this he lost his balance and fell head first to the ground from about 20 ft.


1991   2S   The men were struck dead by lightning during routine seismic operations. They were
            found close to a tall tree.
1991   1S   Tree felling for a camp clearing. Chainsaw operator was ready to drop a tree and had
            advised his two helpers to move away. When the tree fell, one of its braches hit a near
            by dead tree. This one fell on one of the helpers, killing him.
1991   1S   Some trees were entwined by some creepers. Shortly after one had been cut down,
            these creepers broke off a dead branch. According to the chainsaw operator he had
            warned his helper, but he had not moved away quick enough and was struck by the
            branch and killed. The helper was not wearing his helmet.



1991   1S   A tree that was being felled, rotated twice around its own axis, severely injuring the
            chainsaw operator. The helper was killed by a blow to the head. Both had been standing
            close to the tree when it started toppling.
1991   1S   After a tree had been correctly felled, a creeper attached to this tree caused a dead
            branch to fall. It fell on a worker, crushing his helmet and killing him
1991   1S   Cook cleaning fish at a river bank was killed by a falling dead tree branch (approx 20 cm
            diameter). There had not been any line cutting in the vicinity, the branch fell due to natural
            causes.
1991   1S   Line labourer dies from heat stroke

1991   2S   Personnel carrying vehicle with 17 passengers went off the road and down a 200m drop.
            Two were killed in this RTA others injured
1991   1S   Deceased was a passenger of a car which collided with a Land Rover whilst trying to
            avoid another vehicle.
1991   1S   The deceased lost control of the truck whilst driving over a bridge. The vehicle went off
            the road and crashed into the river below




1991   1S   A patrol vehicle was attacked by armed robbers. The deceased was shot in the head and
            died instantly.
1991   1S   The deceased encountered a robbery in progress and was killed instantly when one of
            the robbers opened fire on the deceased's vehicle.
1991   1S   The deceased was standing with his colleagues at the roadside having a conversation. A
            3d party, driving a defective vehicle and under the influence of alcohol, crashed into the
            men. The deceased received head injuries and died shortly after being admitted to the
            hospital which was only 75 metres away.

1991   1S   Caught by a sudden wave, washing him over the side. Drowned despite wearing
            protective suit and life vest.
1991   1S   When returning from shore visit, fell between dock and vessel.
1991   1S   Entering mud testing cabin which had become electrically alive as a result of faulty
            grounding
1992   1S   Helicopter pilot failed to follow company procedures and attached unweighted straps to
            the hook, during the short flight the straps came into contact with the tail rotor assembly,
            which failed and separated from the aircraft. The helicopter crashed and the pilot was
            killed
1992   1S   Struck by helicopter blade when vessel was heaving in heavy weather.

1992   1S   Contractor fuel clerk killed whe he hit his head on boat during a crash


1992   1S   Speed boat collided at high speed with third party canoe. It was sailing on the left hand
            side of the river, collision took place in bend of the river. One 3rd party died of injuries.
1992   1S   Bus driver loses control while driving out of a village and knocks a woman off her bicycle
            on the right hand side, then overcorrects to the left and knocks down and kills a woman
            on the left hand side. The bus diver then gets severely assaulted by an angry group of
            villagers.
1992   1S   Mechanic crushed whilst 'unstuffing' heavy vibrator components from 40 ft shipping
            container




1992   1S   A man was lost during a man overboard exercise.

1992   1S   The victim, who could swim, was standing in the bow of the boat, not wearing a life jacket.
            Boat (navigating at night) collided with a floating tree trunk and victim was ejected from
            the boat. When the victims body was recovered, there were signs of bruising on his
            forehead, he may have lost consciousness due to hitting the object when being ejected.

1992   1S   Workboat capsized when streamer tangled on aft roller, towing boat backwards. I
            crewperson died as a result of heart failure as result of immersion.
1992   1S   A man died whilst diving.


1992   1S   Line worker stepped into an irrigation canal, which formed a pond right on the line. He
            was walking with a bamboo stick, when he lost his footing and fell in the water. He could
            not swim. His supervisor, who could not swim either, dispatched a vehicle to get help.
            Victim was recovered an hour later, water was about 2.5 m deep in the spot where the
            victim slipped in.

1992   1S   Temporary drill helper drowned while attempting to cross a river
1992   1S   Condition of landing site caused helicopter to hover for disembarkation. Last passenger
            off gave pilot the thumbs-up to depart then tried to retrieve bag from helicopter landing
            skid as aircraft lifted. Pilot was unaware of situation. Passenger lost his grip after 3 km of
            flight and fell to his death.


1992   1S   Whilst the deceased was extinguishing a fire the top of a portable fire extinguisher was
            propelled into his abdomen causing fatal injuries. The initial external diagnosis did not

            stuck (no shovel) and the two decided to walk back to the nearest road, 20 km away.
            After some 15 km one started feeling faint, started to vomit and then collapsed. By the
            time the other had reached the road and returned with help, the victim had died

            lorry reversed slightly, injuring a worker who had been sitting behind the rear wheel of the

            camp. The line crossed a river which normally could be forded (50 cm deep, 8 m wide)
            and the victim had done so the day before. It had been raining heavily the day before.
            Victim was found drowned, 100 m downstream.

            field. Despite wearing a seatbelt, his upper body was flung out of the open driver’s

            Night watchman found dead with head injuries. Possible burglary or murder.



1993   4M   A barge used for the transportation of equipment and drilling equipment sunk and four
            workers were killed.

1993   2S   The two victims performed maintenance work on a lifeboat. The lifeboat was
            unintentionally released and crashed into the sea thirty meters below the lifeboat deck.

            lightning strike killed one of the policemen.

            Reasons unknown, weather and visibility were good.
1994   1S   A Mechanic was deploying gun array down gunslip at night. The Gunslip was made of
            stainless steel. Mechanic fell down gun slip into the sea. After FRC search and rescue
            the body was recovered, drowned.
1994   1S   Drilling crew making a three day trip along river, travelling at night. Crewmember moved
            to back deck (unprotected area) of the boat to relieve himself and fell in the water. Boat
            turned around but too late to rescue the victim.
1994   1S   Died from Overdose of addictive drugs. Stole morphine from Captains safe. Fired and
            sent home. Died from overdose one week later
1994   5M   Vibrator truck on long haul trip, driving behind minibus. In a gentle bend, the minibus
            braked hard at the scene of a road accident. Vibrator could not be stopped in time and
            crashed into vehicles parked along the road and a crowd of onlookers. 5 killed and 12
            injured.

1994   1S   Vehicle he was driving was involved in roll over

1994   1S   Explosion in LQ. An employee was killed by an explosion which seriously damaged the
            living quarters. He was doing ordinary maintenance in a sewage conduit, where there
            was an undetected build up of gas
1995   1S   Transporting materials for being used in seismographic operations, a helicopter, crashed
            against a mountain; in this accident the pilot died due to multiple fractures
1995   1S   Crew transport to camp with military escort was ambushed by insurgents.

1995   1S   Agricultural tractor towing an empty trailer. Suffered brake failure on gentle slope and
            steered of the narrow road to avoid upcoming vehicle. Tractor rolled over in the berm,
            crushing the driver.
1995   1S   Victim was standing next to the right hand side door of a light vehicle parked on the left
            side of the road. He was crushed by a passing truck, even though there was enough
            room on the road.
1995   1S   Assistant mechanic intended to do a repair on the base plate of a vibrator in operation.
            He was thrown off balance when the base plate was lifted and crushed at the next
            vibrator point.
1995   2S   3 men were pushing a grounded boat. When it freed they all jumped onboard. One lost
            his grasp and drowned. A 2nd man tried to save him and drowned also.
1995   1S   Drowned whilst swimming in a river.

1995   1S   During seismographic operation and conducting a diamond drill, a labourer died when
            this tool contacted an electric cable

1995   1S    A local labourer walked out of a line camp situated in a area of karst topography and
            known sink holes. He was never seen again, did not return to village, crew etc. Area
            extensively searched as well as possible on foot and by helicopter. Assumed he fell
            down a sink hole.
1995   1S   Group of local workers noticed a bird nest in the top of a tree. As this was a highly sought
            after species of parrot, they decided to cut down the tree with an axe. When the tree fell,
            it toppled another tree, which killed one of the workers.

1995   1S   A truck fitted to transport personnel dropped down a ravine 30 metres whilst transporting
            site labourers. One was killed and 16 injured.

            drowned.
1996   1S   Chaining crew ahead of survey crew arrived at a river crossing. River was swollen and
            fast flowing after rainfall. Despite clear instructions to the contrary, the victim decided to
            swim across, to demonstrate ha was a strong swimmer. He also challenged two others to
            follow. When the surveyor arrived at the river, he called the swimmers back, but only the
            two followers turned back. In the middle of the river, the victim realized the danger and
            tried to turn back, but failed.
1996   1S   During seismic operations a worker drowned when he tried to cross a river




1996   3S   Boat used for crossing 250 m wide river sank. Craft was overloaded. Only few
            passengers used PFD. 3 drowned
1996   1S   Death from malaria. The deceased had been working on a project in the Congo (Point
            Noire and offshore) and had also spent some time in Gabon.

1996   1S   Bulldozer operator - tree fell on him during road lease seismic clean up

            Congo (Point Noire and offshore) and had also spent some time in Gabon.
1997   3S   Helicopter crashed about 5 minutes after take off from the base, in a jungle area, a few
            hundred meters from a helipad. According to survivors' reports the helicopter was put
            into autorotation by the pilot, tilted over, hitting the trees and finally crashed on its roof. 3
            passengers died from injuries.
1997   1S   While reversing to get closer to a shop the driver intended to visit, he ran over a
            pedestrian behind the vehicle. Pedestrian apparently did not hear reversing alarm (or did
            not know what it was)
1997   1S   This employee worked for seismic operations and returned to his camp by boat. When
            the boat was about to reach the river bank, the passengers started moving towards the
            boat's bow. It became unbalanced and sank.
1997   1S   Barge was passing under seismic line and snagged the cables of the line. The barge
            helper, who was not wearing a life jacket climbed on top of the barge command cabin in
1997   1S   order to release the cable. As he was trying to free the line, it parted
            Barge with explosives container turned on its side (90 degrees). To recover the
            explosives, an access hole was made in the side of the barge and one person entered.
            Half way the offloading, the barge turned over completely (180 degrees). Victim could not
            be rescued in time.

1997   1S   Technician repairing air conditioner on gun boat touched live wires

1997   1S   Victim was dismantling a copper object in his tent on the camp. The object exploded,
            fatally wounding him and injuring a colleague.
1997   1S   Contractor was cutting down a tree to clear brush for laying seismic lines. As tree fell it
            struck contractor in the chest and abdomen.
1997   1S   Chain saw operator sustained compound fractures to right let. Hit by tree he had cut.

1997   1S   Victim was helper on a survey crew. Because of the high temperature (around 50 0C) the
            crew had stopped work between 12 and 14:00 and rested in the shade. At around 16:00
            they stopped to drink water with glucose solution. Briefly after drinking the victim started
            to shiver violently and collapsed. He was taken into a vehicle by the crew and bathed with
            water. Doctor and ambulance arrived after 35 min, but despite the efforts of medical staff
            the victim dies an hour later in base camp
1997   1S   About 1 hour into a 330 km trip on three lane tarmac road, driver dozed off and veered off
            the road (despite passenger attempt to intervene). Hit a horse drawn cart and killed one
            of its passengers.
1997   1S   The engine room water-tight door swung open and struck victim, causing him to strike his
            back on the handrail. Although he was OK at first, he collapsed later & stopped
            breathing.
1997   1S   While placing a container on top of accommodation the assisting banksman was caught
            between the bottom of the container and a storage box for life vests.

1997   1S   Victim was found trapped between a workbench and a tool rack which had fallen over
            onto him
1997   1S   First engineer electrocuted while using portable electrical vacuum pump. Immediate
            cause was an incorrectly wired extension lead, which made the pump casing live.




1997   1S   While changing a bulb lamp in an electrical sub-station, the worker received an electrical
            shock which caused his death.

1997   1S   Vehicle got stuck in wadi and water level rose rapidly. Driver escaped but passenger
            unable to get out of vehicle on time. He drowned and his body was recovered 2 days
            later when wadi was again dry.

            moved
1998   1S   The incident happened at noon (shift change) on the second day on board after coming
            back from break. It was a clear day and slick calm seas. The employee came out of the
            galley after eating lunch and joined in to help the cable hands laying the last of the cables
            of a layout. He was feeding the buoy rope out and accidentally had his foot inside the
            byte of the rope which cinched up on his leg and he was dragged over the stern of the
            vessel and into the water. A co-worker saw all this happen and notified the bridge of a
            MOB. The vessel swung around and went to the last location he was spotted at, which
            was the buoy that was just deployed but did not see him. They quickly retrieved the buoy
            thinking he was still tangled up in the rope but the buoy rope came up without him
            attached to it. A search was conducted with all vessels (4) on the crew and also help from
            other vessels/helicopters that were in the area. The search ended the following morning
            without finding him. Only object that was found was his hard hat

1998   1S   While attempting to move a line of local fish traps from a survey area, contractor was
            forced overboard by the release of a decked fish trap when the recovery rope parted.




1998   2S   Coxswain and Observer died after immersion following workboat capsize. Workboat was
            towing a barovane back towards mother vessel, with tow taken by ‘quick release’ wrap on
            lifting frame. Workboat inverted too quickly to release line, and injured parties became
            trapped under the upturned hull.
1998   1S   Local Fisherman washed overboard when small open fishing canoe was swamped by
            Seismic Support ( Chase Vessel)

1998   2S   One person fell to the sea from a suspected dislodged grating. During the rescue
            attempt a crew member from the standby vessel entered the sea and also died.
1998   1S   While checking light fitting, fell 25 feet from ladder after electrical shock

1998   1S   The deceased was installing a new radio antenna on the main radio tower at a height of
            32 metres. On completion of the work, he fell from the ladder and landed on the concrete

1998   1S   A routine Fast Rescue Craft drill was being conducted. On being retrieved from the sea,
            the lifting frame failed and the boat fell around 18m into the sea. There were 3 crew. One

            was pushed approx 65 metres downstream and finally stopped against some large rocks.

            km/hr, but did not see motorcycle for unknown reason.



1999   1S   Boat driver was shot as he was trying to prevent outboard engine and boat from being

            became loose, and when the victim was trying to secure the machines a welding machine

2000   1S   Chief Engineer on re-supply vessel crushed by tow rope during re-fuelling operation

2000   1S   A policeman who was on night duty was found dead in the swimming pool at a company
            rented residential property.

            vehicle was attached by rocket and rifle fire causing multiple fatal injuries to the driver.

            The assistant was injured by the rotary drill and was taken to the hospital where he died
            later that day
2001   1S   Worker was on the side of the road changing a flat tire and was struck by oncoming

            river. The employee had been instructed numerous times to wait with the crew for the
            scheduled boat to retrieve them

            contracted) Policeman working with the seismic survey team left the party to return to the
            waiting transport. He was found by local villagers in a semi-conscious state hours later
            and died sometime after this. Heat stroke and dehydration are the probable causes of

            suffered fatal injuries and died en route to trauma centre.

            truck which killed him

2002   1S   The victim collected the blaster and decided to start shooting as he complained that the
            shooting rate was too slow. He placed the blaster on top of some wood that had fallen
            across the line 0.7 meters away from the shot point, as the water level was around knee
            deep. He took the first shot and gave details for the next shot point almost immediately.
            The wood was blown up and threw the blaster and the victim away during the explosion.
            At the same time the blaster hit the victim and he sustained fatal injuries in the chest and
            face region of the body.
2002   1S   A line labourer was working on the line, and he was sinking down while crossing a creek
            with 10cm water depth. He was unable to keep his footing and was carried 30 meters.
            Unfortunately there was a 10-meter waterfall at the end of this creek, and the labourer fell
            down to the bottom of the waterfall and consequently died.
2002   1S   An engineer was found dead at the foot of stairs in engine room. Vessel which had been
            standing by in severe weather. The victim had sustained a head wound, but post mortem
            examination revelealed he had suffered a heart failure, probably causing a fall and the
            consequent wound.
2002   1S   Chainsaw operator died as a result of massive trauma to his head, neck and chest
            inflicted by a falling tree. The tree causing the injuries unexpectedly fell as a result of the
            planned felling.


2002   1S   31 year old male crewman found to be missing from onboard seismic vessel in central
            North Sea. Conclusion of investigation was that the person took his own life. The body
            was not found.

2002   1S   Employee was involved in a motor vehicle accident while driving company vehicle on east
            - west pipelines right of way
2002   1S   Worker was walking from camp site to drill rig when he was attacked and killed by a bear

            The captain went into the ballast tank and died instantly because of the lack of oxygen.

            operator had not noticed that the victim was rushing for a water gallon underneath the
            blade, when he was lowering it.
2002   1S   One man on ground was struck by bulldozer and killed.
2002   1S   While an employee was carrying out repair work on a leaking hydraulic system of a
            loader, the bucket hydraulic system failed causing the bucket to suddenly fall and crush
            the employee underneath it.
2002   1S   Caught in power take off shaft while unloading water from truck
2002   1S   During coffee break by roadside, when run over by truck
2002   1S   A contractor, missed a crew boat and boarded (against advice) a third party commercial
            boat. The boat collided with another third party boat and the contractor died.

            generator.
2002   1S   Door return spring came loose and caused a blow to the head, worker fell from ladder

2002   2S   At the project site base smoke was observed around the junior staff accommodation area
            at about 02.00 hrs. It was then discovered the smoke was emanating from a fire located
            in one of the rooms and the fire was immediately extinguished. After the fire was
            extinguished, two bodies were discovered in the room where the fire had occurred.

2002   1S   A contractor participating in basic safety water survival training had successfully
            completed a helicopter escape exercise and emerged from pool. He collapsed shortly
            afterwards. An unknown, pre-existing medical condition meant that participation most
            likely exceeded his cardiovascular capacity.
2002   1S   Swerved to avoid a deer and rolled vehicle over
2002   1S   A helicopter crashed while lifting a sling load (from a hover). The pilot sufferred fatal
            injuries and died on route to the trauma centre

            manoeuvred the boat, intending to reach the shore. The engine was bumped by an
            underwater hazard and the driver lost balance. He was thrown into the water and the
            engine propeller hit his head.

2003   1S   A watchman alone on the line, took the initiative to change a flat tyre of a wheel mounted
            compressor, performing therefore a task far beyond his normal duty. During this
            operation, while the victim was lying underneath to re-position the jack, the compressor
            left in an unstable position felt down and crushed the victim’s chest to death
2003   1S   Mechanic was greasing the chain of drilling machine. He started rotating the chain when
            his turban got caught in the spinning head nipple. This pulled him toward it with full force
            and the chain pulled him upward. He was crushed by the neck and hanged.

2003   1S   A line clearance labourer with his workmate was crossing a river in a canoe (no PPE's
            worn). When the canoe was less than 10metres from the shore, it started taking on
            water. The two people in the canoe panicked and jumped into the river. The victim’s
            workmate was able to swim to the bank but the victim disappeared under the water and
            consequently drowned.
2003   1S   A crew of two seismic line cutters and two cleaners came to a narrow creek. They
            created a small detour to a narrower section of the creek and used a tree trunk to form a
            bridge. Two cutters and one cleaner crossed the bridge to continue work. However, after
            a while the cleaner could not hear the other cleaner. He looked back, but saw no sign of
            him. He then raised the alarm. The dead body of the missing man surfaced close to the
            line from where he disappeared
2003   1S   An employee died while attending a training course which involved snorkelling near the
            Barrier Reef.
2003   1S   A Driver/Helper working on a surveying crew, was carrying a four metre telescopic
            measuring rod. Between the third and fourth measuring location, the rod came in contact
            with a high voltage transmission line and the helper was electrocuted.
2003   1S   An Electrician sustained an electrical shock while repairing a microwave oven in the staff
            mess.
2003   1S   Died from injuries sustained in fall. Working on scree covered hillside. Pneumatic hose
            for drill was dragged across hillside above him dislodging rocks that hit him causing him
            to fall over cliff edge.
2003   1S   Exposed to asbestos from working in camp environment construction

2003   1S   During field sesimic work with explosives, as a result of a shot, a branch fell off a tree and
            hit the workers.
2003   1S   A labourer fell ill and collapsed due to heat stroke while laying seismic line.

2003   1S   Four contractor personnel were stung by bees as they were carrying out right-of-way
            clearing and de-stumping work on site. One of the contractors, who had the worst stings
            inflicted, died on site while the other three were treated successfully in the site clinic.
            Nigeria
2003   1S   The accident happened inside a power pack unit where a generator (Diesel aggregate)
            was placed. The victim's jacket sleeve was "caught" by an uncovered drive band inside
            the unit and he was pulled into it. He died from the injuries after ten days in hospital.

2003   1S   A mechanic was working underneath a large truck unscrewing a bolt from the
            transmission protection case. The engine was running, his coveralls were caught up on
            the rotating drive shaft, and he was fatally injured.
2003   1S   The employee was standing between a third party truck and a company vehicle. The third
            party driver did not notice the person standing between the vehicles before backing up.

2003   1S   A zodiac light boat (being towed by a tug) with two personnel on board was hit by sea
            swell, knocking one of the passengers overboard. The passenger drowned.
2003   1S   An electrician was replacing an electric lamp without isolating the electric power supply
            and came in contact with the electric cable disconnected from the lamp housing. This
            lead to his immediate death.
2003   1S   A field telecommunications technician fell approximately 20 feet from a microwave
            telecommunications tower and subsequently died from injuries suffered from the fall.
2003   1S   A contractor employee was involved in cleaning the exterior of a third floor window of the
            new Project Office building. The operation was being conducted from a large working
            cradle suspended from the whip line hook of the crane. At some point it appears that one
            of the wire suspension slings of the cradle became snagged with the main crane hook.
            During attempts to free the suspension wire one of the wires came free of the crane
            hook, causing the cradle to fall and 3 persons to fall to the ground. The employee
            subsequently died from the injuries sustained
2003   1S   Fell from ladder on oil rig
2003   1S   A surveyor died as a result of lightning striking a pile driving rig. He and four others had
            assembled near the rig to await transport. Although the weather had changed, prior to
            this strike no lightening had occurred and no thunder had been heard.
2003   1S   A worker was struck on the head by a compressed air hose which blew off its fitting on a
            piece of equipment. He received serious head injuries which proved fatal. The air hose
            was held by hose clamp on the fitting with no keeper or restraining chain or sling to
            prevent the hose from thrashing around.
2003   1S   A crewman on a supply boat was struck on the head by a flexible hose when it moved
            violently after pressure was vented through it.
2003   1S   ATV roll over while drive was looking sideways into gravel pit
2003   1S   Loading quad (ATV) on ramp to truck bed, quad fell off ramp on top of worker
2003   1S   Did not stop at uncontrolled railroad crossing
2003   1S   A small boat driver was manouvering his boat intending to reach the shore. The engine
            was bumped by an underwater hazard and the driver lost balance. He was thrown into
            the water and the engine propeller hit his head
2004   4M   A Bell 206L helicopter crashed with a pilot and 2 passengers in a shallow area about 2
            miles from shore. The bodies of the pilot and one passenger were recvoered. The
            helicopter reportedly sent out a mayday call during stormy weather about 25 miles from
            the location it went down.
2004   1S   Whilst attempting to clear the bulldozer exhaust of a colony of "killer" bees, the bees
            became agitated and stung him fatally. The driver's body was found half-naked (without
            PPE) a few metres away from the bulldozer. He was found to be alone. Sudan.

2004   1S   At approximately 9:30 on March 26th, whilst using a shovel to clear drill cuttings from a
            seismic shot hole, the drill helper either slipped or had his clothes caught by the rotating
            auger stem and was pulled in between the auger stem of the hydraulic drill and the
            gearbox. The injuries he received were fatal.




2004   1S   A vibrator convoy was moving to the next VP, and the lead vibrator started to descend a
            slope. With the ground frozen, the vibrator slid down a small gully and fell into a ravine.
            The drivers cabin was severly crushed, and the driver died at the scene from head
            injuries
2004   1S   A front recording crew labourer fell into a flooded creek.

2004   1S   Local villager + donkey killed by ordinance cleared during mine clearance operations
            associated with seismic. Mine was removed (stolen) from guarded/marked ordinance
            bunker at night.
2004   1S   Worker, assisting mountaineer, fell from steep slope in mountainous terrain.

2004   1S   The victim became involved in a fight with a co-worker in the camp. Both men had been
            drinking. In the fight the victim was wounded in the groin area. He was taken to hospital
            but died as a result of his injuries
2004   1S   The deceased, reported sick to medical staff on 12:15 hrs and was medevaced at
            17:10hrs. The deceased was immediately hospitalised, with suspected advanced
            malaria. Company medical staff verified that the deceased received appropriate medical
            care, however he was in critical condition. He died at 03:00hrs from severe cerebral
            malaria with apparent multiple organ failure.
2004   1S   The victim was working on line with other crewmembers. The victim had cut about 10
            trees of 15 to 30cm diameter before the accident occurred. The helper cleared the area
            around the tree, which the victim wanted to cut and the victim started to cut the tree
            following normal procedures. The tree he cut did not fall on him directly; it was the dead
            branch of another tree that was falling with the tree he cut that fell on him. The falling
            branch broke his helmet, inflicting internal injury, leading to his death.
2004   1S   A navigator had been working in the instrument room on the night shift, and just before
            breakfast, announced that he was going on deck for some fresh air. When he failed to
            appear at breakfast, full emergency procedures were initiated and a full scale search and
            muster were carried out. As a result, it was confirmed the navigator was missing and
            presumed lost overboard. Seismic operations were immediately suspended and both the
            vessel, and its attendant five chase vessels, were deployed in a carefully planned search
            pattern. The weather at the time of the incident and during the search was fine. No sign
            was found of the navigator.
2004   1S   While carrying out seismic works a vibrator truck lost control and overturned. The
            mechanical driver was killed.
2004   4M   A contractor had suffered a severe hand injury. The Medevac helicopter, crashed, killing
            all four people on board. Four persons were on board the pilot, the co-pilot, the injured
            rig worker and a medical assistant.
2004   1S   A parked water truck was unable to start because of a defective battery, so the truck crew
            requested a tow from a parked truck. During the setup of the operation, the water truck's
            lorry man was crushed between the 2 trucks.
2004   1S   A ferry was being used by company, contractors and authorised third parties to cross the
            river and gain access to the production block. During a ferry loading operation under
            heavy rain conditions a waiting driver stepped out of his bus and was run over by a
            reversing truck.
2004   1S   A mobile crane, driven by Company driver, failed to climb a slippery frozen hilly road,
            stopped and pulled back. The driver jumped out of the cab, but was hit by the vehicle and
            died.
2004   1S   A contractor bus driver received a head injury resulting in his death, while he was working
            under the bus. The accident took place when the bus was parked outside the plant and
            was being washed, and the driver went under the bus to do some checks. He asked the
            cleaner to start the engine, while he was working under the bus. The cleaner started the
            engine without realizing that the gear was engaged, the movement of bus resulted in
            crushing the driver’s head.
2004   1S   Caught between barge and boat during tie-up operations.
2004   1S   A junior grader operator suffered a fatal injury when the grader he was assigned to
            operate crushed him. The deceased was in the process of placing a 10-gallon water
            cooler into the grader when it came into contact with the grader's gearshift lever,
            engaging it into forward. The grader rolled forward crushing the deceased under the rear
            wheels of the grader
2004   1S   A road was being constructed leading to a drilling pad. A bulldozer used in the
            construction reached difficult terrain, so the operator stepped out to discuss how to safely
            traverse the terrain with supervisor, leaving the bulldozer running and the blade up. 3
            bags of inert material were loaded into the cab on a ledge just to the right and above
            driver's seat. Another worker went into the cab to remove a bag of inert material, which
            caused another bag to move, accidentally releasing the lever brake, which lowered the
            blade whilst another labourer was passing under it. The Labourer died from injuries after
            being struck on the head by the blade of the bulldozer.
2004   1S   A bulldozer driven by the casualty while working in a quarry fell from the height of 4m and
            toppled.
2004   1S   A truck driver fell off a barge and drowned when a tractor-trailer filled with scrap metals
            was being loaded onto barge. The driver, who was not wearing a personal floatation
            device, exited the cab to, in the limited workspace between the truck and the edge of the
            barge.
2004   1S   A contractor died when he was opening a trench in a concrete walkway with a pneumatic
            hammer and made contact with an electricity cable.
2004   1S   A team was erecting steel poles to form "goal post" style warning markers approx 30m
            either side of an uncontrolled crossing at a railway line by digging 4 holes, cementing in
            place 6m vertical scaffold poles and stretching bunting between them. Above the railway
            line, at approx 5.7m, ran the 3000+V DC cable carrying the electric locomotive supply
            line. There was uncertainty about the length of the final pole, so 2 labourers raised it and
            it touched the live line. One labourer died from the resulting injuries.

2004   1S   The deceased person received an electric shock whilst drinking from a water cooler.
2004   2S   Two contractors died when an explosion occurred while they were welding a structure
            over the sump tank of a barge.
2004   1S   The victim lost his balance and fell down from scaffolding while assembling a 4 metre
            long vertical scaffolding pipe to the scaffolding platform. He had not secured his safety
            harness.
2004   1S   The gun of a security guard was accidentally fired when being placed horizontally on a
            ledge, fatally wounding a colleague. The safety catch was not used. No firearms
            procedure was in place.
2004   1S   Developed throat cancer from years of inhaling toxic fumes from welding
2004   1S   The Head of Construction site was driving around in a high dune area, seeking a
            vantage point for a photograph. The vehicle overshot the crest of a dune and rolled over,
            down the slope. The driver was killed (head injuries) and the passenger injured. Seat
            belts were used, but roll over bar was makeshift. Driver was not trained in dune driving.

2004   1S   Died in mva when struck by deer
2005   1S   A recording shooter group was sitting under a mound of earth (slight overhang) to sit in
            the shade. The overhang of earth collapsed and hit the group. Two shooter helper
            labours were buried under the mud and got injured. First shooter received minor injuries,
            second shooter received head injury, compound fracture of left lower leg bone and
            vertebral compression. The second injured person died in hospital unexpectedly due to a
            fungal infection of the brain. Probable cause of the infection was through the open
            wound(s) on the head.
2005   1S   Vehicle reversing hit an elderly person who subsequently died

2005   1S   Operator crushed by generator pack being lifted onto vessel deck.

2005   1S   Chase boat crew member fell overboard, not recovered

2005   1S   Victim drowned during recreational swimming in rest period. Natural pool of water in
            desert.
2005   1S     During operations on a seismic project, a shooter and his helper were detonating 2.5 lb
              charges loaded at a depth of 20 feet. The drill log of the shot hole in question indicated
              that the hole was drilled to depth, loaded and plugged correctly. Conditions at the shot
              point while the charge was being prepped for detonation were normal. The shot hole was
              detonated without delay and recorded by the central unit at time zero. The shot hole did
              not blow out at the time of detonation and the Shooter’s Helper walked to the shot point
              from a distance of approximately 100 feet to clip the leads. At the moment the Shooter’s
              Helper leaned over the hole to cut the leads, the shot hole blew out and hit the Shooter’s
              Helper in his chest area, killing him instantly. The delay between detonation and the
              subsequent blow out is estimated to be in excess of 30 seconds.

2005   1S     Worker had arm outside passenger truck, crushed by oncoming traffic. Poor medical
              treatment 30 days later resulted in his death
2005   1S     A 30-year-old surveyor was struck on the head by a tree felled in the vicinity of a survey
              point. He was taken to a nearby hospital, but died within 2 hours as a result of his injuries.
              The deceased had contracted a chainsaw operator from the local community to fell trees
              near the survey points in order to ensure the GPS equipment would have good satellite
              reception.
2005   15 M   15 members of a seismic party froze to death during a snow storm. Due to lack of
              communication the crew could not be warned about the bad weather approaching and
              evacuation started too late.
2005   1S     Single cab pick up transporting driver and two passengers collided with a 5 ton truck
              travelling in the opposite direction on the public highway



2005   2S     3rd Party 18 wheel Mack truck hit seismic contractor truck with 30 passengers all wearing
              seat belts. Two died


2005   1S     The truck was parked by the side of the road close to the main traffic lanes. The driver
              was inspecting a noise under the truck when another truck collided with a passing fuel
              tanker. The collision caused spillage and as a consequence of the impact, the split fuel
              ignited and exploded immediately. The stream of ignited fuel was blown by the wind
              directly at the contractor’s truck.
2005   3S      Seismic truck was hit by train on unprotected railroad crossing, which was normally in
               disuse
2005   1S      A Surfer (personnel transportation boat) collided with a fishing canoe. One fisherman
               died. Canoe was hard to see - hidden in a trough of a wave, early morning and no
               navigation lights.
2005   1S      A contractor was caught in a moving hydraulic door during completion/commissioning of
               a platform at the construction yard. Unexpected closing of door due to mechanical failure
               of a spring in the opening/closing device. The injured person tried to pass through the
               door when the door was not fully open and got caught.

2005   1S      Steel structure fell of forklift on worker, crushing his head
2005   1S      A tug boat collided with a wooden long boat. 4 people in the fishing boat fell into the
               water. 3 were recovered, 1 drowned.
2005   1S      The worker suffered an electric shock while connecting an electric cable to a plug at the
               generator.
2005   1S      The incident occurred at about 3:00 am. The victim, a 30-year-old worker, was trying to
               get into his upper bunk bed after a card playing and drinking session in his shared room
               in one of the dormitories in the construction camp. He fell, his head hit the floor and his
               room mates found him lying on the floor and unconscious.
2005   1S      The employee, whilst working unsecured at height on a structure under fabrication, fell to
               the ground when attempting to descend, resulting in a fatality.
2005   1   S   Employee fell approximately 10' due to hand rail giving way
2005   3   S   Three fatal cases of Malaria amongst expatriate E&P workers
2005   1   S   A contract staff person was hit by a fire fighting gas bottle.
2005   1   S   An Fast Crew Boat was moored. Its engine was started to test propulsion. The mooring

               half broke off and fell in opposite direction hitting worker.
2005   1S      After a 2 hour walk, exposed to the sun, the victim became lost, dehydrated and
               disorientated in high temperatures without water. The Geologist had lost his way and
               went 500 metres off the original path. He fell down into a river from a vertical cliff 40
               metres high.
2005   1S      Death by drowning while swimming in natural pool during standby period
2005   1S   After 2 hour walk, exposed to the sun, the victim became lost, dehydrated in high
            temperatures without water.

2005   1S   Mooring rope parted and recoiled hitting person on SB who sufferred severe multiple
            fractures to right leg and bruising to forehead. 1st aid and immediate evacuation to
            onshore hospital but after two hours resus by doctor he passed away.


2006   1S   ATV rollover onto land crewman after being released for the day

2006   3S   Helicopter flying under IFR, crashed in fog during landing

2006   1S   Helicopter crash, causes unknown, was involved in sling work

2006   5M   A geomatics crew were attacked by armed robbers. The robbers fired on the crew
            vehicle.


2006   1S   On May 18th 2006 an air-conditioner technician was fatally injured when his shemakh
            (headscarf) got caught in a pump engine fan from a pressure washer, pulling his head
            down hard onto the engine causing extensive internal head injuries.




2006   1S   The incident occurred at the fly camp when the deceased and two other crew were about
            to board a truck. The deceased was trapped between two trucks (that were parked back
            to back) when the front truck rolled back
2006    S   A subcontractor trouble-shooter helper attempted to swim a canal. Trouble shooter helper
            got into difficulties, his colleagues were unable to help and he drowned.
2006   1S   Cable deployment crew employee drowned while attempting to cross a canal

2006   1S   The crew supervisor drove his vehicle past the mine detection specialist who were
            clearing the road for land mines. His vehicle was blown up by an explosive device and he
            died
2006   1S   A mountaineer helper fell approximately 10 metres during seismic survey activity and
            died due to serious injuries

2006   3S   Medic wanted to dispose of out of date medical alcohol and gave this to crew member to
            take to the incinerator. Crew member decided to consume this alcohol, together with two
            others. All three died from alcohol intoxication
2006   1S   Driver took "short cut" across desert between seismic camps. Was found 3 weeks later.

2006   1S   ATV ("Quad" or "Trekker") rolled over when backing up on steep, bulldozed track

            the other way.

2006   1S   A watchman (contractor) was intoxicated by carbon monoxide from his heater.
2006   1S   Contractor was loading empty gas bottles from jetty to barge. He rigged bottles for lifting,
            donned manual inflation life jacket and tried to board barge from unsecured ladder on
            jetty side from where he fell into the water and died.
2006   1S   A mechanic technician was checking a platform lifeboat. During the inspection, the
            hanging hook accidentally opened and the man fell, with the lifeboat, 20 meters.
2006   1S     A logging crew foreman was supervising logging on a pipeline right of way. He was
              standing next to a chainsaw operator who was cutting a tree. The tree fell, struck a free
              standing dead poplar tree which broke into several pieces, one of which struck the
              foreman causing a fatal injury.
2006   1S     Employee killed by CO2 fire extinguisher bottle propelled by its own internal pressure
              when the bottle fell and the top was broken off
2006   1S     Employee killed by exploding fire extinguisher during training
2007   1S     A hired single engine small aircraft carrying 1 pilot, 1 company staff and 1 contractor
              crashed. One company staff dies, the pilot and contractor sustained minor injury.
              Possible fuel starvation.

              A Geophone Layout crew was attacked by spear wielding men, one employee targeted
              and hit with three spear strikes which resulted in him being fatally wounded.

2007   1S     Drilling subcontractor employee was fatally injured when the right sleeve of his jacket
              became caught in a bracket/clamp affixed to the spinning Kelly quill pin and he was
              pulled into the mast framework. The incident occurred while the individual was servicing
              the drill prior to the days work schedule.
2007   3M     Three workers of a seismic crew died due to premature explosion of high explosives used
              for shooting.




2007   1S     A worker employeed by a contractor on a seismic project died as a result of an anti-
              personnel mine.
2007   1S   A contracted water tanker drifted from the highway, dropped it’s wheels and rolled over.
            The driver died at the scene of the accident.




2007   1S   On November 29, 2007, a seismic troubleshooter was involved in a fatal ATV incident.
            The incident occurred on a flat, straight section of a gravel / dirt road. Weather and road
            conditions were favorable, and the operator held valid ATV Operator Certification. The
            speed limiter on the ATV was found to be intact and operational (15 mph
            maximumallowable speed). The Operator was traveling by himself and there were no
            witnesses to the incident. The Operator had several years of industry experience, and
            was considered to be a competent ATV Operator.
2007   3S   Two crew entered the anchor chain locker to investigate a noise. Later, when it had
            become evident the two were in trouble, a third person (with BA) also entered, but also
            collapsed.
2007   2S   Two contractor staff died in armed attack on crew boat. A large sum of money was stolen

            defensive reaction and was shot dead.




2007   1S   A fatality and 4 injuries occurred among a civil works contractor crew. A heavy sand and
            rainstorm caused a number of sleeper portocabins at a civil work camp to roll
            over.:Extreme wind conditions (>30 m/sec) outside expected range, displacing
            transportable accommodation units (standard sea container moved 80 m, rolling several
            times); No barriers to avoid rolling and subsequent movement of units. Loose inventory
            inside units further exacerbated injuries;
2007   1S   Two contractor employees left the area with a bulldozer in order to tract 2 blocked lorries.
            One of them employees jumped on the track, while the bulldozer was moving, trying to
            reach the cabin. In that moment, he slipped and was crushed between the track and
            cradle, having his right leg crushed and arterial bleeding. He died in hospital.

2007   8M   The vessel concerned was one of a number of large anchor handling vessels contracted
            by the company to support a rig moveas part of an appraisal programme. The vessel was
            involved in the deployment of the 8th and final anchor for the drilling unit when it
            experienced significant stability problems causing it to capsize. This tragic accident lead
            to the loss of 8 lives.Government investigations are continuing, but our interim conclusion
2007   1S   is vessel was conveying this tragedy was theproperly stacked on board but one of them

            slightly leaking. During the journey the boat propulsion engine developed a fault and
            stopped; the victim, who had the responsibility of checking and restarting the engine,
            entered the engine room and in an attempt to restart the engine an explosion and fire
            occurred.The victim was extensively burnt and was taken to a Clinic where he remained

            lightning strike.
2007   1S   Victim was killed by failing portable fire extinguisher. Upon activating the extinguisher, the
            bottom of the canister failed. The fire extinguisher propelled upwards and struck the IP in
            the neck and chin area resulting in fatal injuries.
2007   1S   During remediation works in a flooded area, a swamp buggy owned by the contractor
            turned over in deep water. The driver could not get out of the cabin and drowned.



2008   1S   Ground worker mortally injured during the preperation of a truck mounted crane for re-
            location




2008   1S   Contractor water truck after delivery of water to a community, drove over 3 year old child
            which was in blind angle in front of the truck when it moved off.
2008   3S   Bus carrying contractor and company personnel failed to give way and to stop at
            intersection between graded road and hard top main road (despite STOP sign). The bus
            was hit by a tipper truck approaching the intersection on the hard top road. 3 passengers
            were killed, 31 seriously injured and 27 suffered minor injuries..
2008   1S   4 Flush Drilling Contracted Helpers raised a pre-assembled length of 2 x 4m (8m) drill
            pipe to begin flushing a hole in a field of tall, densely planted, maize corn. The team did
            not observe an overhead power line. The drill pipe touched the overhead powerline. All 4
            personnel suffered an electric shock. 3 recovered immediately but unfortunately 1 died of
            electrocution.




2008   1S   A Vibrator Technician was driving a seismic crew pick up along a blacktop highway at
            approx 07:00. The driver saw a vehicle approaching and turned across the oncoming
            lane to make access to an offroad track believeing there was enough time. The
            approaching third party car was travelling well above the speed limit and struck the
            seismic pick up. The third party vehicle airbags deployed. Both drivers suffered minor
            cuts and bruises. The passenger in the third party vehicle was killed.
2008   1S   A Vibrator Technician fixed a problem in a spare vibrator cab. Individual got down from
            the cab through the right side door and told the driver to move up to production. The Vibe
            Technician walked around the rear of the vibrator and when walking past the rear left
            wheel decided to duck under the vibrator to check the rear transmission oil level. In doing
            so the driver moved the vibrator to go up to his production position. The Vibe Tech was
            hit on the head by the metal parts of the underside of the vibrator (skull fracture). The
            supporting hand was pushed into the ground, breaking a wrist. Whilst then laying on the
            ground the inside edge of the Vibrator tyre pinched the side of the individual's torso to the
            floor (internal organs injured and broken ribs. Individual died of his injuries in hospital.




2009   1S   A rig subcontractor suffered fatal injuries from electric shock while performing a welding
            operation. A light used to illuminate the worksite was found to be a potential source of
            the shock as the light's electrical cord was found to have been damaged. The damage
            may have been caused by a pinching action between the loose mounting bracket and the
            light housing which was observed to be in contact with the victim's body.
2009   2M   2 crewman succumbed to methanol and ethanol poisoning after consuming locally
            purchased alcohol while aboard a marine vessel off the coast of Indonesia

1. Employee was not wearing his PPE (life vest) in an Geophysical Marine   Drowning   MOB
area and a job procedure that required it. Employee
was also known to all as a non-swimmer.
2. The job procedures at that time had the cable
hands coiling up the buoy rope on the deck and
feeding it out from the aft deck after the last cable
head was deployed.
 After the investigation of this fatality the use of buoy
rope canisters was initiated so no buoy rope was
ever to be coiled up and fed off of the deck during a
cable deployment again.
