                           INSTRUCTIONS FOR FILLING OUT ATTENDANCE REPORT
                                    FOR SEMI-MONTHLY EMPLOYEES

     Who fills out attendance reports? All non-exempt employees.

     Who are non-exempt employees? Anyone who has not been specifically identified and told they are exempt.
     Examples of non-exempt employees (may not be a complete listing-If in doubt, call payroll office):
     Clerical, Secretarial, Paraprofessionals, Custodians, Maintenance Workers, Groundsmen, Warehousemen,
     Bus Drivers, Mechanics, Route Supervisors, Computer Technicians, Computer Engineers, etc.)

     Examples of personnel that are exempt employees who are not required to fill out attendance reports:
     All certificated personnel unless working in position not requiring a certificate, Administrative Cabinet Members,
     Directors, and some Supervisors meeting certain criteria as established by the Department of Labor.

         Before filling out an attendance report always look at the Semi-Monthly Payroll Calendar to see if the
         Pay Period will be a 2 week or a 3 week Pay Period. There is an attendance report specifically for the
          2 week period on regular size paper and a report specifically for the 3 week period on legal paper.

                              Use the Proper Form for the Current Pay Period
 1 Employee Name:              Employee Name as shown on Social Security Card
 2 Social Security #:          Social Security Number
 3 Pay Period                  The start and end date of the two or three week pay period that corresponds to the
                               Semi-Monthly Payroll Calendar; this will be filled out if you enter the starting date in cell C9.
 4 Facility:                   The school or department where the employee works
 5 Account Number:             If this attendance report is for any position other than the regular position (I.e. service contract,
                               extra duty) the account number to be charged must be entered.
 6   Position:                 Enter the position for which this report is being filed. (I.e. Paraprofessional, custodian, clerk, etc)
 7   Date:                     The daily date (I.e. 7/31/00)
 8   Time In:                  The actual time that employee reports to work for each date
 9   Time Out:                 The actual time that employee leaves work (I.e. for lunch) for each date
10   Time In:                  The actual time that employee returns to work on the same date.
11   Time Out:                 The actual time that employee leaves work on the same date. (I.e. for doctor's appt.)
12   Time In:                  The actual time that employee returns to work on the same date.
13   Time Out:                 The actual time that employee leaves work at the end of the day.
14   Total Hours Worked:       The total hours worked should be added down the column for each date. Round to
                               nearest quarter of an hour. (15 minutes = .25 hr 30 min =.5 hr. 45 min = .75 hr
                               60 min = 1 hr.) Round up or down as appropriate.
15 Week Total                  The total hours worked for each week should be totaled across the row titled
                               "Total Hours Worked" No totals would be in the "Time In" Time Out" Rows.
16 Overtime:                   Any hours worked over the employee's "Standard" hours would be listed here.
                               Payroll will calculate actual overtime hours to be paid at time and a half based
                               on 40 hour work week.
                               All overtime hours are to be PRE-APPROVED by supervisor.
17   Sick:                     Any hours, or portions thereof, that an employee is out for sick leave.
18   Sick Personal:            Any hours, or portions thereof, that an employee is out on sick personal leave.
19   Annual Leave              Any hours, or portions thereof, that an employee is out on annual leave.
20   Signature of Employee:    The employee must sign the attendance report attesting to this report being a true and
                               accurate record of his attendance.
21 Signature of Principal      The Principal or Director must sign the attendance report attesting to this report
   or Director:                being a true and accurate record of the employee's attendance.

     The Secretary of Each School or Department will collect the Attendance Reports from everyone in her facility or
     department and send them in one batch to the Payroll Office by the Due Date.
     It is VITAL that this attendance report be received in the Payroll Office by the Due Date listed
     on the Semi-Monthly Payroll Calendar in order for the employee to be paid on time.
          If the attendance reports are not received on time, payment will not be processed until the next Exceptions
                         Payroll, which will be run on the third business day after each regular payroll.
