             Description
To provide mobile dental services to low-
income children and adults in rural areas
of the Bootheel.

To provide computer workstations,
software and training to increase the
efficiency of public services to a 23-county
service area in southeast Missouri.
To implement an innovative teen
pregnancy prevention program in seven
high schools in Dunklin and Pemiscot
counties.

To provide further development of the
infrastructure of the organization,
specifically training and strategic planning.
To provide nurse home visits and
mentoring to pregnant or parenting teens
and their infants.
This 3-year grant will help establish and
operate a new Diabetes Management
Education Center.
This 12-month grant will enable Dunklin
County Caring Council to purchase
medical diagnostic equipment to properly
diagnose child sexual abuse.
This 2-year grant will fund salaries for the
child advocacy center, a center that
provides care and support for abused
children.
This 4-year project will provide free
assistance to addicted individuals and
their families through outreach, case
management, education and mentoring.
The program will aid individuals who have
been in rehabilitation by creating a safety
net to facilitate recovery and a productive
re-engagement into society.
This 2-year project will be an expansion of
a community-based mentoring program
that matches volunteer mothers with
pregnant or parenting teens.
This 2-year project will improve the quality
of emergency medical services through
the purchase of two ambulances and two
automated external defibrillators.
This 2-year project will improve the
center's information technology systems
and includes the purchase of a new server
and the hiring of an IT person to update
the system.
This 12-month grant will be used to
purchase a new portable x-ray machine
and provide partial reimbursement to the
hospital for a CT scanner.

This 9-month grant will provide a new
health curriculum to the education
program at Stoddard County Alternative
School. Components of the program
include smoking cessation, individual
counseling, hygiene, nutrition, infant care
and the promotion of deterrents to teen
parenthood.
This 3-year project is enhancing health
care services for up to 400 uninsured
women in Butler County. implementation of
This 3-year project is an
phase III of the department's action plan to
address diabetes and high blood pressure
in low-income or uninsured patients. It
also allows for equipment for blood testing
for patients, staff education to become
Certified Diabetes Educators, and
provides the center with the opportunity to
become recognized by the American
Diabetes Association (ADA) for diabetes
education.
This 12-month project provides a new
service system to improve scheduling and
data input regarding patient information.
This 3-year project is providing
comprehensive women's wellness
services including pap smears, breast
exams and birth control.
This 12-month project replaces out-dated
equipment and expands health services.
This 3-year project is expanding the
organization's perinatal care program to
New Madrid and Mississippi Counties.

This 2-year project provides a psychiatrist,
nurse and two therapists to serve youth
with severe emotional disturbances.
This 2-year project is providing funding to
staff a new crisis shelter. The shelter will
serve persons who are experiencing a
crisis situation such as family violence or
homelessness.
This 2-year project improves the current
infrastructure of the emergency medical
response system.
This 3-year project is establishing a
program to aid uninsured and
underinsured individuals in completing
necessary forms to receive medications
and health services from patient
assistance programs.
This 2-year project is replacing diagnostic
and medical equipment and a computer
system for the school health office which
provides direct service to 540 preschool-
12th grade students.
This 12-month project is being used to
purchase necessary equipment for the
center to continue its work providing
health education, services, information,
transportation and meals to seniors.

This 12-month project is being used to
replace inadequate kitchen equipment in
an effort to more effectively serve
nutritious meals to seniors and those with
handicaps.
This 2-year project continues to support
the services of the center, which includes
developmental and therapeutic services
for children and their families regardless of
their ability to pay.
This 12-month project enhances the
center's current services of providing hot
meals to the elderly and the disabled.
This 12-month project is allowing a dental
program to increase services, eliminating
the waiting lists for dental services in the
area. This is also improving access to
quality oral health care to underserved
residents.
This 3-year project expands mentoring
services to pregnant teens and parenting
group sessions for at-risk parents.
The two-year grant will provide operational
expenses to the Qulin Nutrition Center so
that it can continue meal preparation and
food delivery to home-bound and
underserved elderly and disabled in Butler
County.
This 2-year project will allow the
organization to provide essential health
services to low-income infants and
children.
This two-year grant serves the Dunklin
and Stoddard counties' drug court
program, to help offenders with substance
and mental health problems break the
cycle of crimes. Monies will be used to
provide medical, dental and mental health
examinations to ensure more effective
treatment, judicial supervision and case
management.
This two-year grant targets adults at risk
for or already diagnosed with heart
disease and diabetes in Missouri's
Bootheel. It will provide essential primary
and preventive services, nutrition and
lifestyle health education, along with
appropriate medical referrals.
This 2-year project will allow the
organization to deliver nutritious meals to
elderly and disabled individuals.
As part of its service to a five-county area
in the Ozark Foothills, the Commission will
use this two-year grant to continue to
operate a medical clinic in Naylor, and
provide needed programs including
childhood immunizations, child abuse
prevent, dental services and general
health support.

Thist wo-year support grant will enable the
agency to continue providing 24-hour
staffing and ensuring prompt responses to
emergency requests to residents in Butler
and Ripley counties.
This 2-year project will replace the
nutrition center's vehicle and some
equipment.
Adults and children in a four-county region
of southeast Missouri will receive mental
health education services, transportation
services, access to psychological help and
out-patient services through this two-year
grant. It is estimated that more than 800
area clients will be helpd each year.
This 2-year project will enable the
organization to offer health and social
support services to senior residents.
More than 400 area low-income women
will have acess to health services,
screenings and counseling services
through this two-year grant. The health
center collaborates with local churches,
schools and other providers to provide
comprehensive education and information
on women's health issues.
Regional Grant:
American Lung Association - 04-0640-TR-
04

In an area with a much higher-than-
average smoking rate, this grant will
provide two programs - one aimed at
helping adults quit smoking and the other
focusing on preventing children from
beginning tobacco use. Over the course
of the two-year grant, more than 3,000
area residents will be touched.
Regional Grant:
American Lung Association - 04-0640-TR-
04

Through the Health Department, the
smoking cessation efforts will be focused
on adults and in the workplace, through
two American Lung Association-created
programs. The two-year community grant
will reach a 17-county area in southeast
Missouri.
Requested funds to provide medical,
dental and vision services as well as
medication assistance to over 1750
Hurricane Katrina evacuees.
Requested funding to provide medical and
dental care to Hurricane Katrina evacuees
in the Bootheel region.
Requested funding to provide medical
services to evacuees and volunteers at
nearby shelters as well as conduct
environmental inspections of the shelter
campgrounds.

Requested funds to provide counseling
and resource coordination to both
evacuees and those serving them in New
Madrid, Scott, Stoddard, and Mississippi
counties.

This grant will cover travel costs,
equipment needs and other operations
expenses to enable the center to maintain
its meal delivery program to seniors in the
southeast Missouri community.
Funding through this 2-year grant provides
salary support for the southern Missouri
ambulance district so that it can continue
to provide emergency medical care to
county residents.
This 2-year basic support grant funds
salaries and supplies for this southern
Missouri ambulance district which
provides transportation to medical
services for county residents.
Funding supports the organization's
salary, food and hygiene supply costs tp
provide assistance to low-income families
in Dunklin and Pemiscot counties.
Funding allows for the continuation of on-
site counseling and support services to
victims of domestic violence and sexual
attack in Mississippi County.

Funding maintains the health department's
family planning services, which serve low-
income women in the Bootheel.
This basic support grant provides salary
and operational support the Bootheel
health organization which works with
families and individuals to provide health-
related assessments, education and
services.
In order tto maintain women's health
services in the Bootheel, this grant
supports the health center's family
planning and reproductive health services
for low-income, uninsured and
underinsured women.
This grant supports operational expenses
related to the department's family planning
program (screenings, referrals, exams,
education and counseling) for low-income,
uninsured women in this southeast
Missouri county.

In Wayne County, the district is using this
2-year grant to support its 'Tiger Fitness
Challenge,' which includes constructin of a
student fitness center, assessment of
student fitness levels and nutrition
education for almost 1,200 students age 5-
18.
In the Charleston and East Prairie school
districts, the TATU program reaches 500
elementary students with information to
discourage tobacco use. This is part of
MFH's Tobacco Prevention and Cessation
Initiative.
This three-year grant supports the health
department in providing case management
and patient education to people living with
diabetes.
This three-year grant enables the
organization to enhance current diabetic
care programs in order to serve 1,500
individuals in the southeast Missouri.
The expansion of the organization's
current dental program expects to treat
more than 2,700 elementary school
students, in 15 schools in six southeast
Missouri counties.
Using the 'Recovery Limited' program, this
3-year grant targets adults with co-
occurring disorders who live in the
Bootheel, where access to integrated
treatment is limited. Funding supports
expansion of programs, strengthening of
collaborations and improved tracking of
outcomes.

Recovery Unlimited Provider Consortium,
a new collaboration with area
