                              LETTER OF INSTRUCTION (LOI)

1. Welcome to the Fort McCoy MSTC Combat Lifesavers Course (CLC).

2. Your course training dates are: 8/7/2009 TO 8/7/2009

3. Location of training is the Medical Simulation Training Center (MSTC), Bldg. 1024 S 11th
Ave., Fort McCoy, WI.

4. Fort McCoy MSTC provides the training only. Transportation, meals, and lodging are an
individual unit responsibility. POC for units wishing to reserve barracks is Mr. Mark Stelzner of
Fort McCoy DPTMS, phone: 608-388-3721 or Fort McCoy Lodging, phone: 608-388-2107/3646.

5. MISSION - Immediate far-forward first aid is essential on a widely dispersed and fluid
battlefield to prevent Soldiers from dying of wounds. Medical personnel may not be able to
reach Soldiers at all points on the battlefield in a timely manner. The MSTC’s CLC mission is to
train non-medical Soldiers, both Enlisted and Officers, with advanced first aid/lifesaving
procedures beyond the level of self-aid or buddy aid.

6. The proponent for the CLC, the US Army Medical Department Center and School
(AMEDDC&S), recently updated the CLC to include skills that were recommended from lessons
learned in Operations Iraqi Freedom and Enduring Freedom. Nine (9) lessons will be taught:

             LESSON 1 - PERFORM TACTICAL COMBAT CASUALTY CARE
             LESSON 2 - EVALUATE A CASUALTY
             LESSON 3 - OPEN AND MANAGE A CASUALTY'S AIRWAY
             LESSON 4 - TREAT PENETRATING CHEST TRAUMA AND DECOMPRESS A
                         TENSION PNEUMOTHORAX
             LESSON 5 - CONTROLLING BLEEDING
             LESSON 6 – INITIATE A SALINE LOCK AND INTRAVENOUS INFUSION
             LESSON 7 - INITIATE A FIELD MEDICAL CARD
             LESSON 8 - REQUEST MEDICAL EVACUATION (MEDEVAC)
             LESSON 9 - EVACUATE A CASUALTY USING A SKED OR IMPROVISED LITTER
             DAY FOUR – TRAUMA LANE

7. Practical skill stations and human patient simulator mannequins will be utilized for hands-on-
training.

8. Soldiers are required to take and pass a student examination at the end of the course.
Retests are authorized for Soldiers who fail the exam.

9. Soldiers are encouraged to bring an old or unserviceable uniform to use on day four trauma
LTX.

10. Soldiers who successfully complete the written and performance tests will receive promotion
points (based on 40 training hours) and be certified as a COMBAT LIFESAVER.

2. You are attending this school for TRAINING PURPOSES ONLY. Your unit is responsible for
individual orders and travel.

3. Potential trainees with any of the following conditions should not attend this course:
       (a) Fractures, with or without cast
       (b) Recent Surgery
       (c) Upper respiratory infections
       (d) Any medical condition that may impair performance
       (e) Profiles need to be reviewed by MSTC NCOIC prior to class seat reservation

4. The MSTC is located at 1024 S 11th Ave., Fort McCoy, Wisconsin. Driving directions to Ft
McCoy can be found on pages 3 and 4. Map of Ft McCoy cantonment area is shown on page 5.

6. Report to the MSTC, Building 1024 NLT 0830 hrs on class start date. It is located on the
northeast corner of the Ft McCoy cantonment area (see Cantonment Area – Ft McCoy Map,
pg. 5). If you anticipate arriving late, please notify SGM Bruce Kaufman (Phone: 608-633-1952-

7. Report with the following items in your possession:

8. Lodging and meals are an individual unit responsibility.

9. Duty uniform is ACU with Patrol Cap. Uniform will be worn in accordance with AR 670-1.

10. Washers and dryers (no charge) are available at locations throughout Fort McCoy.
Individuals must supply laundry soap, dryer sheets and other laundry sundries.

11. Please refer to attached PACKING LIST for individual items, equipment, documents, and
identifications required for the course (page 4).

12. PHYSICAL TRAINING – No structured PT will be conducted. However, Soldiers are
encouraged to perform individual physical fitness activities.

13. Be prepared and pack gear for seasonal weather.

14. DRIVING INSTRUCTIONS TO FORT MCCOY :

        (a) Fort McCoy is located in west-central Wisconsin, between the communities of Sparta
and Tomah, and approximately 35 miles east of the city of La Crosse. Entrances to the
installation are located off of State Highway 21. Exits off Interstate 90 (at Sparta) or Interstate 94
(at Tomah) provide access to Highway 21. Visitors should enter the installation through the Main
Gate.

        (b) If traveling by Privately Owned Vehicle (POV) you must have a valid Department of
Defense (DOD) decal OR pass in order to gain access to Fort McCoy. Passes can be obtained
at the Main Gate area using a valid DOD identification card. You must also provide a valid
driver’s license and vehicle registration, along with proof of automobile insurance to obtain a
pass. Otherwise, you may not be allowed entry on the post.

      (c) See Page 5 for driving instructions to Fort McCoy MSTC once inside cantonment
area. Detailed map of cantonment area and facilities listing can be picked up at Main Gate.
