    Student Responsibilities: (Failure to comply with any of the following may result in termination from the program.)
    1.    Keep regular attendance at school and on the job, notifying the Site Supervisor of any anticipated absences.
    2.    Abide by all state, federal, business site, and school rules and regulations.
    3.    Demonstrate honesty, punctuality, cooperation, confidentiality, and respect for others.
    4.    Submit verified documentation of hours at the internship site to the Site Supervisor and Internship Site Visitor.
    5.    Inform the internship Site Supervisor and Internship Advisor or Site Visitor of any problems, concerns, accidents/injuries immediately.
    6.    Abide by the dress code of the work site.
    7.    Provide proof of health and emergency medical insurance coverage to the Internship Advisor.
    Internship Site Supervisor Responsibilities:
    1.    Comply with Federal and State Labor and Industry regulations.
    2.    Conform to federal laws prohibiting discrimination on the basis of race, color, national origin, sex, or disability.
    3.    Provide a safe working environment and report any student accidents and injuries.
    4.    Consult with, and provide feedback to, the Internship Advisor concerning the student’s Internship Learning Experience Plan.
    5.    Verify attendance and/or time records.
    6.    Complete a Site Supervisor’s Evaluation Form of the Intern at the end of the Internship.
    7.    Maintain liability insurance.
    8.    Supervise students while on business premises and monitor employees who have direct contact with students.
    Internship Advisor Responsibilities:
    1.    Supervise student placement in an appropriate internship site.
    2.    Consult with the internship site to develop an Internship Learning Experience Plan.
    3.    Maintain regular communication with the Intern and with the Site Supervisor as needed.
    Internship Site Visitor Responsibilities:
    1.    Secure all paperwork, including the Internship Learning experience Plan, Week 5 Evaluation, Final Evaluation and the Site Supervisor’s Evaluation.
    2.    Contact the company and arrange a site visitor appointment with the Site Supervisor and the Student Intern.
    3.    Visit the company and talk with the student and Site Supervisor.
    4.    Act as a liaison between the Student, the Site Supervisor and the Internship Advisor, should any issues or problems occur at the Site.

    The parties agree to comply with all laws, ordinances and regulations of both organizations involved in this internship site experience and further agree to keep all records
    and information confidential as related to the evaluation of students in the program. Each party shall defend, indemnify and hold the other party, its officers, officials,
    employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of injuries and damages caused by each
    party’s own negligence.

    Relationship of Parties. The Intern enters into this Agreement as, and shall continue to be, an Intern. The Intern shall not look to the Company as its employer, nor as a
    partner, agent or principal. The Intern shall not be entitled to any benefits accorded to Company employees including, without limitation, compensation, workers’
    compensation, disability insurance, or vacation or paid time off days.

    This Agreement shall be governed by and construed under the laws of the State of Washington. In accordance with WAC 392.190.056 Bellevue Community College does
    not discriminate on the basis of race or ethnicity; color; national origin; sex; marital status; sexual orientation; age; religion or creed; the presence of any sensory, mental or
    physical disability; or veteran status in its educational programs and activities. Harassment of any employee/student with regard to race, religion, ethnicity, age, disability,
    gender, marital or veteran status, or sexual orientation is strictly prohibited.

    This agreement shall remain in effect until either party provides the other with thirty (30) days written notice of its intention to terminate this agreement.
