INSTRUCTIONS: 1) Assign a weight (5=vital, 0=not applicable) to each of the attributes and record them in the top row of boxes. 2) List the five or six most subjectively appealing name candidates in the left column. 3) Score each name between 1 and 5 (with 5 being the highest) for each attribute. Enter scores in the top half of the appropriate boxes. Do this for each candidate. 4) Multiple each score by its attribute weight to derive a relative score. Enter resultss in the bottom halves of the boxes. 5) Add the weighted relative scores for each candidate and enter the total in the TOTAL column.
