December 2006 Application Guide for the Doctoral Degree Graduate School of Science and Technology, Keio University Applying for Doctoral Degree If you wish to apply for the doctoral degree, you will need to submit both the documents for Preliminary Review and Formal Review. Please follow the procedure outlined below. 1. Application Fee for Preliminary Review As the Keio University Commencement Regulation article #5 states, application for the doctoral degree by thesis screening only (those not enrolling in doctoral program) requires an application fee. application documents. (1) Applicants who have completed the doctoral degree requirements, but have withdrawn without submitting a thesis: -¥50,000 (2) Applicants who already have either bachelor’s or master’s degree from Keio and to whom does not apply to (1): -¥70,000 (3) Applicants to whom applies to neither (1) nor (2)-¥100,000 (4) Full-time teaching staff at Keio: -¥20,000 If you are applying for the doctoral degree while in the doctoral program, no screening fee is required. Regarding your month of degree conferral, for thesis written in English, TYPE THE MONTH OF YOUR RELEVANT SCHOOL’S FACLUTY MEETING AS: Month 200X. 4. Deadline and Office for Submission (1) Deadline ●Deadline for submitting documents for the Preliminary Review: 2 weeks prior to the faculty meeting of the relevant school. ●Deadline for submitting documents for Formal Review: 2 weeks prior to the faculty meeting of the relevant school. * For details concerning the application and screening deadlines, please refer to the Doctoral Thesis Screening Schedule for Academic Year 2006. (2) Office for Submission -Academic Affairs Office (Faculty of Science and Technology) *If you need to make corrections in any of the above documents you have already submitted, please inform your chief examiner and make the corrections using the form provided by the office. Please make the corrections by the time you submit the bound thesis (2 weeks prior to the faculty meeting) to the Academic Affairs Office. 5. Documents to be Submitted by the Chief Examiner Please check with your supervisor whether he/she has submitted the documents below. The deadline for submission is 2 weeks prior to the faculty meeting of the relevant school. (1) Document required for the Preliminary Review: Thesis Proposal (prescribed form) 1 (2) Document required for Formal Review: Thesis Screening Outline and Result of Academic Achievements (prescribed form) 6. Instructions for Filling Documents for Preliminary Review -Format of the application documents downloaded is based strictly on Keio University’s Commencement Regulations. Please do not delete or add anything to the format. Read the instructions below carefully. ●For documents (1)-(4), the year should be written according to the Western calendar. ●For documents (1)-(5), please make sure that the all the thesis titles written on your documents and Chief Examiner’s documents are the same. ●For the details of each procedure, ask an academic coordinator from your school or the chief examiner. (1) Degree Application Form ● Please write the type of degree (Science or Engineering) you expect to receive in the bracketed space in the first line of the application form. Please check with your advisor. ● The date on top should be the day you submit your Degree Application Form to the Academic Affairs Office. ● Please do not forget to affix your seal where it is required. For those non-Japanese who do not have a seal, please use your signature. (2) Thesis Abstract ●The abstract should be approximately 300 words. (3) Thesis Index ●NOTE 1 and 2 on the prescribed form are instructions for all faculties, but NOTE 1 does not apply to Faculty of Science and Technology theses. ●The sample thesis index shows how to fill out the prescribed form. If you have any problems concerning the original format, please consult your chief examiner. ●Circle either “ KOU ” program or “ OTSU ” program, and do not write anything in the space for the registration number. ●The sub-thesis will not be considered in the thesis-screening process of the Faculty of Science and Technology. Please leave the space for sub-thesis title blank, and the number of items 0. ●For the Supplementary Materials, write 0 for the number of materials and refer to the sample thesis index for the list format. ●If the thesis index exceeds more than one page, write the page number as(1/2) or (2/2) on the bottom of the next page. (5)Supplementary Materials (A4 size) ●File reprints (or photocopies) of published papers, abstracts of papers presented at academic meetings, and other reference materials related to your thesis in the order specified in the Thesis Index in a paper folder. ●Include the thesis title and name on the cover and spine. 7. Archiving of Thesis If you are granted the doctoral degree, your thesis will be stored in the National Diet Library, Faculty of Science and Technology Media Center, and the Chief Examiner’s collection. The thesis in the National Diet Library and Faculty of Science and Technology Media Center will be publicly available as stated in Keio University Commencement Regulation Article 15. The Faculty of Science and Technology Media Center encourages students to submit their theses to the digital library, and contribute to the archiving of the Thesis Search Database. After the screening of the doctoral thesis, please register your thesis to the digital library.
