Introduction
The Harris Team is currently in the process of identifying healthcare provider networks to be early
participants in the Florida Health Information Exchange (HIE) patient look-up service. The patient look-
up service enables an authorized health care provider to search for and retrieve his/her patient’s
clinical data from other network participants at point of care for treatment purposes. There are other
HIE services that will be offered through the Florida HIE (e.g., public health reporting, provider to
provider messaging, and provider directory), however, this On-Boarding Readiness Questionnaire is
focused on prospective provider networks who wish to participate in the Florida HIE patient look-up
service. Examples of provider networks include operational Regional Health Information Organizations
(RHIOs), Integrated Delivery Networks (IDNs), health systems, and Clinical HIEs.

The Harris Team recognizes that provider networks and healthcare organizations are at varying levels
of adoption and use of health information technology. Therefore, the Florida HIE is designed to be
flexible to accommodate and leverage existing healthcare information systems and processes. The
Florida HIE deploys a federated data architecture model. This model provides centralized services
connected through a Florida HIE gateway at each participant site’s edge system and includes an
integration engine/data connectors that leverage the site’s local Master Patient Index (MPI). Further
technical information will be provided during the on-boarding process.

Participation in the Florida HIE includes a commitment from your organization to provide the resources
for enabling a defined set of inbound and outbound interfaces, to sign a Florida HIE participation
agreement, and to conduct other key on-boarding activities. It is important that prospective
participants demonstrate both technical readiness and a willingness and ability to commit resources to
the implementation process. To assist the Harris Team in assessing your organization’s readiness and
willingness to be an early Florida HIE participant, please complete and submit this Florida HIE On-
boarding Readiness Questionnaire and e-mail it to FloridaHIE@harris.com. A Harris Team
representative will then contact you for follow-up information. Please note that this questionnaire is
not intended to imply minimum criteria for on-boarding to the Florida HIE. The responses received will
be used to help assess current and/or future readiness of your organization and to understand various
deployment needs.

 Answers to this questionnaire are subject to Florida public records law. If your organization will be
disclosing trade secret information, you will need to designate which sections are considered trade
secrets by marking each page “Trade Secret as defined in Section 812.081, Florida Statutes” upon
which such information appears. Information specifically identified as a trade secret under Section
812.081, Florida Statutes, will be kept confidential to the extent provided by law. Designating material
simply as “proprietary” will not necessarily protect it from disclosure under Chapter 119, Florida
Statutes.

Attachment A
Estimated Implementation Work Effort for Provider Network to On-Board with
the Florida HIE for Patient Look-Up Service
To on-board to the Florida HIE, your organization will be required to commit resources and personnel
to work alongside the Harris Team. Deployment activities will span over approximately a two month
period, where various levels of support will be required from your technical personnel. The table
below identifies the tasks that will need to be accomplished by your organization’s staff in support of
the Harris deployment team activities.

Express System Requirements
The Express package consists of a Mirth Virtual Appliance with all required software pre-installed and
pre-configured. Options for running the Express package with various levels of functionality provide
flexibility in accommodating a wide variety of deployment scenarios. Tools, resources, and assistance
will be made available to ease the task of interfacing the participating organizations back-end systems
with the Express package.

Site Hosting Option

The Express package is made available for download as a VMware disk image in .ova format, which is
compatible with the free VMware Player as well as VMware ESX-based products. The following
minimum system resources should be allocated to the virtual machine running the Express package:

            •    100G available disk space

Sites that will be using the extended functionality of the Express package including Mirth Match (a
master patient index) and Mirth Results (a clinical data repository) (i.e., sites that do not have an MPI
or a clinical data repository already) should allocate system resources as follows:

      MINIMUM SYSTEM RESOURCES, FULL EXPRESS
            •    8 CPU cores
            •    16G RAM
            •    500G available disk space

For either Express Lite or Full Express, allocating additional CPU cores will allow for better threading of
transactions in high volume environments. Allocating additional RAM will allow for better database
performance for larger sites. System CPU and RAM resources can be easily scaled post-installation
using the server management dashboard included with a participating organization's VMware system.

The Harris Team can assist in disk allocation planning in advance, as changing the maximum size of a
virtual server can be a time consuming task requiring system downtime to complete. VMware disk
image files are small initially and grow to the maximum size specified as they are used. The Express
package download site contains multiple disk image options with various maximum sizes. It is
recommended that participating organizations using the extended functionality of the Express
package--which means that MPI and/or clinical data will be accumulated on the virtual appliance--
should select the largest disk image file available that their disk storage system can support. It is

important to ensure that the underlying storage system for the VMware server has sufficient capacity
to accommodate the virtual Express appliance at its largest size.

Remote Hosting Option (Service Fee Required)

Participating organizations that do not wish to run the Express package on their own infrastructure
may select from a variety of hosting options available from the Harris Team as a fee-based service. In
these cases, Harris will work with the organization to select the appropriate size and options for the
Express package and it will be hosted in a secure, HIPAA-compliant data center managed by the Harris
Team. In cases where the Express package is hosted, it will be necessary to provide a secure network
connection between the data center and site where the participating organization's clinical data
systems are stored. The form of this secure connection is typically an IPSec VPN tunnel. The VPN
tunnel may terminate on the participating organization's existing network infrastructure, or Harris can
provide a VPN device that may be installed at the participating site. There is also an option for VPN
connectivity using a software-based client, if the hardware solution is unavailable or unfeasible.

Clinical System Integration

Whether the participating organization uses the Harris hosted option, or has the Express package
installed at its own site, it will be necessary to allow for network access between the Express system
and the HIE system. Access to clinical system data may be via standard HL7 (v2 or v3) interfaces,
published web services, direct database connection, file import/export, or other means of data
exchange. The Harris Team will make available a library of adapters for the CONNECT Gateway
component of the Express package, as well as a library of Mirth Connect channels for connecting to the
data sources. In cases where appropriate adapters or channels are not available in the library, the
Harris Team will work with the software vendor or developer to create new adapters and channels as
necessary to facility connectivity to the participating organization's system. These adapters and
channels will then be added to the libraries and made available for use by future sites.

The nature of the integration will vary depending on how the participating site uses the Express
package. In its most basic form, the CONNECT Gateway component of the Express package will be
integrated directly with the HIE system to provide data for the patient look-up service. The patient
look-up services include patient discovery functionality, document query functionality, and document
retrieve functionality. For organizations identified which do not have a unified patient identifier
available, or cannot create appropriately formatted clinical data, the Mirth Match (MPI) and Mirth
Results (clinical data repository) components of the Full Express package would provide this
functionality.

Initiating outbound requests to other sites across the Florida HIE will require either the Mirth Results
component of the Express package or customization and integration of their clinical application to
allow for making outbound requests. In cases of integrating this functionality with existing systems, the
Harris Team will work with the software vendor or developer to provide user interface specifications
and also make available specific Mirth Connect channels to simplify the process of integrating with the
CONNECT Gateway component of the Express package.
